About this file
This is Amendment 0002 to Solicitation 36C24926R0006 issued by the Department of Veterans Affairs Veterans Health Administration, Network Contracting Office 9, located in Murfreesboro, Tennessee. The amendment provides answers to vendor questions and updates the Performance Work Statement (PWS) Section 4.4.1 regarding the scope of cardiovascular and vascular physician services. The procurement requires contractors to provide on-call and callback-only coverage for cardiothoracic and vascular surgical services with no standing onsite requirement; any onsite presence occurs solely in response to callbacks. The Government requires two providers proposed for each specialty to ensure adequate coverage, though only one provider per specialty will be on-call at any given time. Cardiothoracic services require on-call coverage every week including nights, weekends, and holidays, with approximately 5,020 annual on-call hours, while vascular services require as-needed coverage with approximately 1,930 annual hours, with the vascular surgeon serving as backup to the existing VA provider.
The updated scope of care specifies that contractor physicians shall provide cardiothoracic and vascular services including consultation, operative intervention, and perioperative management. Thoracic surgery encompasses procedures involving the chest cavity, lungs, esophagus, mediastinum, and chest wall. Vascular surgery covers procedures involving arterial and venous systems (excluding intracranial and coronary vessels), including open and endovascular interventions for peripheral vascular disease, aneurysms, and vascular access. The amendment clarifies that callback hours are low because surgical procedures rarely occur during on-call shifts, and staffing levels currently consist of one full-time vascular surgeon with no cardiothoracic surgeon on staff. No extension to the submission due date is anticipated, and all proposals must be emailed to olivia.cloutier@va.gov.

Vendor Questions and Answers
1. Can the Government please clarify the required services with respect to onsite and on-call coverage. Please provide a sample schedule to show onsite and on-call requirements.
0. Services required under this contract are on-call and call-back only. There is no standing onsite requirement; any onsite presence would occur solely in response to a callback.
1. Can the Government please confirm the descriptions of the CLIN lines? The PWS indicates an onsite requirement, but there is no clear CLIN line to provide pricing for onsite services. All CLIN lines reference on-call related services.
1. Services required under this contract are on-call and call-back only. There is no standing onsite requirement; any onsite presence would occur solely in response to a callback.
1. Can the Government confirm how many providers are required for onsite services?
2. Only one provider per specialty is required to be on-call at any given time. Onsite presence is only required when responding to a callback request.
1. For on-call purposes, does the Government anticipate the contractor will need to provide on-call coverage every week?
3. Yes. On-call coverage is required every week, including nights, weekends, and holidays for cardiothoracic services, and as needed for vascular services.
1. For Call Back purposes, can the Government provide an idea of how often a provider may be called back to the facility, as the allotment for call-back hours is very low (10 hours according to the CLIN line)?
4. Callback hours are low because surgical procedures rarely occur during on-call shifts. As a result, the need for providers to return onsite is minimal.
1. Can the Government indicate how many providers are currently on staff, and if they are going to rotate the call schedules?
5. Staffing levels fluctuate. Currently, there is one full-time vascular surgeon available for on-call coverage. There is no cardiothoracic surgeon on staff to take call at this time.
1. For cardiothoracic surgery - Are cardiac and thoracic procedures only required as there is a separate need for vascular surgery? In other words, the CT surgeon would not need to provide vascular services, as well?
6. Yes, the VA requires both a cardiothoracic surgeon and a vascular surgeon to be on call. The cardiothoracic surgeon is not expected to provide vascular services, as these are covered by the vascular surgeon.
1. For CT surgery, it says approximately 5,020 hours of service needed, if my math is correct, this means about 14 days of coverage per month?
7. On-call coverage is required for nights, weekends, and holidays. The total hours listed (approximately 5,020 annually) reflect the cumulative on-call availability, not just onsite presence. This does not equate to 14 full days of onsite coverage per month, as onsite time only occurs when responding to a callback.
1. For vascular surgery, it says approximately 1,930 hours of service needed, if my math is correct, this means about 5 days of coverage per month?
· The vascular surgeon serves as backup to the VA provider who is on call. The actual time required will vary based on clinical needs, which is why the estimated hours do not translate directly into a fixed number of coverage days per month.
1. Are you looking for 2 providers for each specialty?
9. The Government is requesting two providers to be proposed for each specialty to ensure adequate coverage. However, only one provider per specialty will be required to be on call at any given time.
1. Does the Government anticipate an extension to the due date of the submission?
10. No extension to the submission due date is anticipated.
Section 4.4.1 is updated and replaced with the following:
4.4.1 Scope of Care: Contractor’s physician(s) shall provide cardiothoracic and vascular services, including consultation, operative intervention, and perioperative management. Services shall include, but are not limited to:
· Thoracic Surgery: Procedures involving the chest cavity, lungs, esophagus, mediastinum, and chest wall.
· Vascular Surgery: Procedures involving arterial and venous systems (excluding intracranial and coronary vessels), including open and endovascular interventions for peripheral vascular disease, aneurysms, and vascular access.
