About this file

This file is an amendment to a federal solicitation providing responses to vendor inquiries regarding Medical Physicist Services.

The solicitation seeks locum medical physicist services on-site at the James H. Quillen VA Medical Center in Mt. Home, Tennessee, under a one-year emergency/relief (ER) contract. The facility operates two Varian Truebeam linear accelerators and a Phillips 64-slice big bore CT scanner, with treatment planning and record-and-verify systems from the same manufacturers. The radiation oncology department treats approximately 200 patients annually and currently performs SBRT, IGRT, IMRT/VMAT, and 3D conformal radiation therapy. The amendment clarifies that no vacation coverage dates have been predetermined at the solicitation stage; however, the contracting officer will provide eight weeks' notice before coverage is needed. This amendment was issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 9.

This amendment is to answer vendor questions as follows:

1. Past performance: Element 1 asks for three references supporting the contractor's experience. May an offeror satisfy this element with references for its proposed Therapeutic Medical Physicist (key personnel), and how will an offeror without directly relevant corporate past performance be rated?

Response: References for the proposed candidate are great. We want to see references for the offeror to evaluate the offeror’s past performance. If there is no direct relevant past performance the offeror is neither negatively nor positively rated, rather they receive a neutral rating.

2. Qualifications: CLIN 0001 reads "Board Certified/Board Eligible," while the PWS states the physicist must be Board Certified. Is a board-eligible physicist acceptable?

Response: No. The physicist must be Board Certified. The price schedule has been corrected and the corrected price schedule is included in this modification.

3. Scope: Should CLIN 0002 (monthly and annual QA on two linacs and one CT, 12 each) be priced as work performed every month, separate from the 240 as-needed hours in CLIN 0001? Please also confirm the linac and treatment planning system models.

Response: The line items are correct as written. Equipment is: Varian Truebeam linear accelerators, Phillips 64 slice big bore scanner. Treatment planning system is: The treatment planning system/record and verify system is Eclipse/ARIA.

4. Submission: Should quotes be emailed to you, and is there a page or file size limit?

Response: Quotes should be e-mailed. There is a size limit of approx. 25mb. If your file is larger and it cannot be compressed, please send in multiple e-mails.

5. CLIN 0001 – 240 Hours / Vacation Coverage: Should offerors assume a single hourly rate applies regardless of how the 240 hours are scheduled? If available, could the Government provide historical utilization, including whether coverage has typically been required in full-week blocks, individual days, or another pattern?

Response: The offeror should assume a single hourly rate would be applied when covering. The underlying assumption was that whatever company got the contract would prefer full week blocks, but if they are willing to cover individual days that would be great. This VAMC has never done this before in radiation oncology, so there is no historical utilization.

6. Minimum Coverage Period: When vacation coverage is requested, is there an anticipated minimum number of hours per day or consecutive days per assignment? For example, should offerors anticipate an 8-hour minimum for each day of requested onsite coverage?

Response: The contract would be for 8 hours of coverage per day. The VAMC is open Monday-Friday, except holidays. The consecutive days question is answered in question number 5 above.

7. CLIN 0002 – 12 EA: Please clarify what constitutes one “EA” for the monthly and annual QA requirements. Does each EA represent one onsite visit, one day of onsite service, or completion of the required monthly QA regardless of the number of hours required?

Response: EA = Each. The EA would be inclusive of all time necessary to complete the monthly QA and it requires an onsite visit. The quantity of CLIN 0002 will be reduced to 11. The one annual QA per year will include a monthly QA as well as the annual QA and will require a prolonged onsite visit. The Annual QA is being separated into a new CLIN 0003 that will be priced as one (1) job (JB) and the price will be all-inclusive of the number of hours worked to complete both the monthly and annual QA tasks. See corrected price schedule included in this modification.

8. Historical QA Level of Effort: What has been the historical or anticipated duration of the monthly QA visit (hours or days onsite)? Additionally, approximately how many hours/days have historically been required to complete the annual QA?

Response: This service has not been previously procured via contract. The VA staff Physicist has always done the QA when he has time available which rarely allows him to finish it all at one time, so it is hard to say how long that would take. After consulting with our physicist, the monthly QA could probably be done in one day, and the annual QA is estimated to take 1 week (40 hours) to complete and will include on-site work.

9. Annual QA: Is the annual QA requirement included within one of the 12 EA monthly visits, or should offerors anticipate the annual QA as an additional level of effort within the 12-EA quantity?

Response: As noted in the response to question 8 above, we have separated out the annual QA visit into a separate CLIN. The 12th monthly visit will be priced in conjunction with the annual QA, but they may be done on different visits at the contractor’s discretion.

10. After-Hours Requirements: Should offerors anticipate that monthly or annual QA activities will routinely require evening, weekend, or other after-hours work due to equipment availability? If so, can the Government provide the typical hours during which this work has historically been performed?

Response: The VAMC staff have always done all of our QA during the working day (7:30am-4pm). There are 2 linear accelerators; due to volume only one is used full-time. When the VAMC needs to do the QA on the machine we use daily, we move the patients to the other machine. If one machine goes down for an extended period of time and we need to do monthly QA that may require some of the work be done after hours.

11. Potential Full-Time Coverage: The requirement indicates that the current need is primarily vacation coverage but may change during the contract term to include full-time onsite coverage. If the Government's requirement changes to full-time coverage, would those services continue to be ordered against the 240-hour quantity under CLIN 0001, would the CLIN quantity be increased through modification, or does the Government anticipate establishing a separate pricing structure?

Response: The hourly rate should not change, as the services offered are the same. We would modify the contract to increase the hours to a full-time coverage amount for however long full-time coverage would be needed.

12. Travel and Related Costs: Should all travel, lodging, per diem, and other costs associated with onsite vacation and QA coverage be incorporated into the respective CLIN 0001 hourly rate and CLIN 0002 EA rate, or may these costs be separately identified/reimbursed?

Response: All-inclusive pricing is preferred, however the offeror may propose these costs separately if so desired.

13. Alternate Pricing: May offerors submit the requested pricing schedule along with an alternate pricing approach that reflects different levels of effort or scheduling scenarios, provided the Government's required CLIN pricing is also completed?

Response: Alternate pricing may be submitted but may not be considered. The pricing structure in the solicitation must be submitted for consideration.
