About this file
This is an Interim Life Safety Measures (ILSM) Check Sheet from the Department of Veterans Affairs for facility construction projects.
The check sheet is a pre-project assessment tool that the Contracting Officer's Technical Representative (COTR) must complete prior to project initiation to identify and implement necessary life safety measures. The COTR is responsible for ensuring ILSM are in place before work begins and for conducting daily work site inspections. The form evaluates eight specific areas: potential exit obstructions requiring alternative exit training; obstruction of emergency service access; impairment of fire alarm, detection, or suppression systems requiring temporary equivalent systems with daily inspections; necessity of smoke-tight, non-combustible temporary construction partitions; adequacy of fire-fighting equipment and employee training; reduction of Life Safety Code compliance requiring two fire drills per shift per quarter; compromise of structural or fire compartmentation features requiring alternate safety procedure training; and overall ILSM implementation requiring Director notification memorandum to all employees. For each item, the COTR must indicate "Yes" or "No" and document completion dates and responsible personnel. This specific form is for an AHU Refurbish Fan Room project (15/6/12) with COTR Clint Roberts, with all work to be performed inside the actual air handling unit. The memorandum initiation date is 4/29/2026, and all eight ILSM assessment questions were marked "No," indicating no life safety measures were required for this particular project.
