CONTINUATION PAGE

1. We respectfully request an additional extension (i.e. at least seven days) from the current deadline of July 28, 2020 based on the date of issue of Amendment 2 and complications related to the current COVID-19 pandemic. The COVID-19 pandemic has created additional obstacles related to the production and shipping of paper proposals, and we request that this be taken into account. For example, FedEx does not currently guarantee overnight shipping. This means that, in order to guarantee arrival on Tuesday July 28, the Contractor must ship proposals on Friday July 24, a full four (4) calendar days before proposals are due to the Government. This request is also based on the fact that Amendment 2 includes extensive information related to all aspects of the offeror’s proposal, including site design and staffing plans. These proposal items require coordination with outside vendors and local Government entities. We request an appropriate time frame for all offerors to update their proposals, produce the required binders, and ship them to the VA.

We are extending the due date through August 4, 2020.

2. Questions 29, 30, 73, 74, 75, 84, 85, 86 request information about the required quantities of exam rooms, Women’s Health exam rooms, tele-health rooms, and group rooms, and the answer provided to each of these questions is to refer to the Design Guide. We have not been able to identify any portion of the Design guide that provides guidance on the desired or required quantity of these rooms types. For example, page 56 of the Design Guide states, “Provide the appropriate number of Patient Care Rooms per teamlet so there is no back log and limited waiting.” However, the “appropriate number” is not defined, and therefore the answers to questions to 29, 30, 73, 74, 75, 84, 85, 86 do not appear provide the information requested in the questions.

Could the Government reference the section(s) of the design guide that provide information on desired or required quantities of different room types? Or, alternately, could the Government confirm that there are no minimum required quantities of exam rooms, Women’s Health exam rooms, tele-health rooms, and group rooms and that these quantities are up to the contractor’s discretion?

PACT Space Module Design Guide. page 34, Section 4.3.4.A

Each PACT Space Module utilizes a ratio of approximately 3 Exam Rooms per teamlet. All Patient Care Rooms should use a standard layout, with equipment and supplies planned in standard quantities and locations

1. 29. Are there any specific telehealth or tele-mental room requirements; including quantity, dimensions, lighting, etc.?

See section 4.8.6.2 and also new Space Module Design Guide.

See Page 36 and 39 PACT Space Module Design Guide

1. 30. Is there a minimum number of Women’s Health rooms being requested?

See Section 4.8.1 and also new Space Module Design Guide.

Every PACT exam room can be considered women’s health as long as the exam table has foot rests, a privacy curtain, a light source, locking door, feminine hygiene products and adjacent bathroom (so a toilet in close proximity), where the Veteran can use a restroom without walking in a public space in a gown. Women’s Exam rooms do not have to be exclusive for women only. “

Advise contractor to consult the design guide See Page 36 and 39 PACT Space Module Design Guide

1. 74. How many procedures rooms does the contract require?

3. Page 145 of the Solicitation requires “Key Personnel, including salary range worksheet and all other supporting documents” to be submitted in Tab E. This is the only reference we have found to a “salary range worksheet”. In the interest of resolving the ambiguity of this requirement, please indicate the information that is required to be included in this salary range worksheet.

Key personnel Section has been replaced with the following language.

SUB-FACTOR E – KEY PERSONNEL

This sub-factor will be used to determine each Offeror’s ability to recruit and retain qualified staff. This sub-factor should identify that the Offeror has qualified staff that can provide primary services in the disciplines indicated in the solicitation that offers a successful contract performance. PWS Reference: Section 2, Staffing and Qualifications.

· Describe your management strategy, organizational structure, critical management support personnel, and key and support personnel at the CBOC level:

· For each CBOC, provide an organizational chart with a full description of each major organizational and management support element, along with key personnel and roles supporting each element. Describe the mechanisms you will employ to recruit and retain key personnel. Key Personnel shall have experience and qualifications that demonstrates the ability to successfully lead and manage their proposed roles.

· For each CBOC, provide a list of the titles, roles, and qualifications of all clinical staff members intended to be utilized in the performance of this contract, taking into consideration the enrollment numbers for the clinic and the minimum staffing requirements; and describe if each staff member is full-time (FT) or part-time (PT) and, if PT, list the number of hours the staff member will work each week. Also, provide the title, and qualifications of the Medical Director who will oversee the clinic, including whether s/he will be FT or PT, with the number of hours per week if PT. Please list PHC staff together, and MHC staff together.

· For each CBOC, provide a list of administrative support staff intended to be utilized in the operation of the CBOC, whether they will be FT or PT (including number of hours per week if PT), and describe the level of training and experience that will be utilized to meet contract requirements, taking into consideration the enrollment numbers for the clinic and the minimum staffing requirements.

· Describe the employment relationship between key personnel and the Contractor, and the arrangement between the parent organization(s) and the Contractor that enables the Key Personnel to participate in the management and operation of the CBOC, including any limitations to their participation in the management and operation of the CBOC (i.e. identify locum providers vs. employees and subcontracted staff/services).

· Describe contingency plan for covering enrolled patient's appointments due to scheduled or unscheduled vacancies to ensure that full patient grid of appointments is covered. The plan should demonstrate how Contractor shall provide seamless or minimal disruption in services. Include identified locum and/or permanent staff that may be privileged in advance to cover routine and emergency vacancies.

· Describe the Contractor’s vetting process for personnel prior to submitting personnel for VA credentialing. Also, describe the process established to ensure timely completion of VA background checks on employees providing services under this contract. The Contractor's approach should demonstrate to the VA that it has an active program to conduct background checks prior to being submitted to the VA, and to demonstrate Contractor cooperation to ensure completion and submission of documents to the VA for completion of VA background check on a timely basis.

4. Paragraph 4.12.1 requires, “Contractor shall provide completed appointments for new PC patients <=20 days from the create date.” However, the QASP in Attachment 1 requires, “Contractor shall provide completed appointments for new PC patients <=30 days from the create date.” In the interest of resolving this ambiguity for all offerors, please indicate which metric is correct.

Section 4.12.1 in the PWS is correct, I will attach the appropriate QASP that matches the PWS.
