Solicitation 36C24919R0075 RFI’s Project # 626-18-101
Corrections for Life Safety Compliance Nashville TVHS MATOC “Seed Project”
Question #1: Reference paragraph 3.1 on pages 14 & 15. The first paragraph says there are two volumes, Technical Proposal and Price Proposal. The third and fourth paragraphs at the top of page 15 refer to a Past Performance Volume and a Technical Approach Volume. We believe the Technical Proposal Volume includes both past performance and our technical approach. Can you please clarify?
Answer #1: Correct, Volume I “Technical Proposal” includes past performance and technical approach. Volume I is to consist of three (3) tabs as stated in the base solicitation;
Tab (A) Technical Proposal Cover Sheet
Tab (B) Factor 1 – Past Performance - Not to Exceed six (6) pages total, Excluding Attachment A, B and C. The six (6) page limit is for any other Past performance Document a company submits other than Attachment A, B and C. Attachment A, B and C does not count towards the six (6) page limit.
Tab (C) Factor 2 – Technical Approach - Not to Exceed fifty (50) pages total, Excluding Attachment 1. The fifty (50) page limit is for everything provided in Tab (C) except for Attachment 1, “Organization & Key Personnel Self performed Trades/Capabilities”, which will be submitted under Tab (C), Sub-Factor 2. Attachment 1 does not count towards the fifty (50) page limit.
Question #2: Based on the requirements of Sub-Factors 4, 5, & 6, we believe it is your intention that we are to provide our master plans for Quality Control, Safety, and Infection Control. These plans are 36, 146, and 9 pages long, respectively. They are not listed as excluded pages at the top of page 15. We believe these should not be counted as part of the page-limited information.
Can you confirm?
Answer #2: It is not the VA’s intention to receive full copies of those items from each company.
In the Volume I “Technical Proposal” submission, Tab C (Factor 2) is not to exceed fifty
(50) pages total. Attachment 1 “Organization & Key Personnel Self performed Trades/Capabilities”, DOES NOT count towards the fifty (50) page total, as noted if
Question/Answer #1. This means that documents would have to be significantly reduced or synopsized as each company sees fit to comply with all page limitations.
Question #3: On page 16 at paragraph 5.1, we are told to label the cover sheet in Volume I as “MATOC Volume I: Price Proposal Submission –“36C24919R0075_1” – Seed Project Price Proposal.” Can you confirm that you want us to label the Volume I (Technical Proposal) cover sheet as the Price Proposal Submission and Seed Project Price Proposal?
Answer #3: No, Volume I is to be labeled as “MATOC Volume I: Technical Proposal Submission – 36C24919R0075” and include the Station Location, Offeror Name, Address, Point of Contact, Phone Number, Fax Number, and Email Address.
Volume II is to be labeled as “MATOC Volume II: Price Proposal Submission – 36C24919R0075 – Seed Project Price Proposal” and include the Project Location, Offeror Name, Address, Point of Contact, Phone Number, Fax Number, and Email Address.
Question #4: On page 16 at “Tab (B) Past Performance – Factor 1”, we believe there are no offeror’s submission items that accompany the Technical Proposal for Factor 1. That is, all inputs to this factor are provided directly to your office as feedback from the person completing the Past Performance Relevancy Questionnaires (PPRQ). Can you confirm? If this is not true, can you please provide a list of what information is to be included in this factor?
Answer #4: You must carefully read all requirements of this Factor stated in the
“GENERAL INSTRUCTIONS, CONDITIONS AND OTHER STATEMENTS TO
OFFERORS”. Refer to Tab (B), Factor 1, Attachments A, B and C and the requirements for each.
Question #5: Can you confirm that, for relevancy, there is a 3-year timeframe on Past Performance and a 5-year timeframe for the Sub-Factor 1 experience? If true, for simplicity in providing past performance information with PPRQs for each experience item, can you make both items for projects that were completed no more than five years before the proposal due date?
Answer #5: The Past Performance requirements will remain as currently stated in the
“GENERAL INSTRUCTIONS, CONDITIONS AND OTHER STATEMENTS TO
OFFERORS”
Question #6: The Offeror Instructions for Attachment 1 and Attachment C are identical. The only difference in the provided forms is the additional word “Organization” in Attachment 1.
Can you please clarify what we are to provide differently in these two attachments?
Answer #6: Even though they look identical, you must still complete each. Complete what is requested in the forms and refer to the “GENERAL INSTRUCTIONS, CONDITIONS AND OTHER STATEMENTS TO OFFERORS”.
Question #7: There are two attachments labeled “Attachment A.” One is titled “Past Performance Questionnaire” and the other is titled “Performance Relevancy Survey.” On page 16 of 100, we are told to use the first version for our Past Performance. There is no reference to use the second version of Attachment A. On page 20/21 of 100, “Sub-Factor 1 Capability and Experience” lists a number of things we are to provide for our project experience. Are we to (1) provide only the information on page 20/21, (2) provide that information plus the second version of Attachment A, or (3) provide only the second version of Attachment A?
Answer #7: There are two (2) separate documents, Attachment A and Attachment 2, that are the same exact document. As Attachment 2 was added inadvertently, please ignore Attachment 2 “VA PAST PERFORMANCE QUESTIONAIRE” that was embedded into the base solicitation. Attachment A “PAST PERFORMANCE RELEVANCY SURVEY”, is to be submitted in Volume I under Tab (B). Attachment 2 is to be ignored and will not be submitted.
