CONTINUATION PAGE
RFI #3 - For the current fleet that the Tennessee Valley Healthcare System (TVHS) operates, what volume do you anticipate for VA units will complete over time?
VA Answer #3 - The purpose of the foreseeable contract is only for what the successful awardee would provide in terms of ambulance services for the TVHS not ambulance services the TVHS provides internally. The TVHS has already provided what ambulance services are estimated to be needed from the successful awardee over the next four years. Further, the TVHS has already provided extensive data on what ambulance services were provided over the past twelve months via Amendment 0002. This is all of the information the TVHS is prepared to provide at this time.
RFI #4 - Does TVHS intend to expand your fleet in the future?
VA Answer #4 – Yes.
RFI #5 - Reference page 51 - What is the maximum file size for submission of proposal documents via email?
VA Answer #5 – The largest sized email our server can receive at any given time is 5 megabytes (mb). That is the only file size restriction. For example, if your offer was 11 mbs in size, the offer would need to be sent over a minimum of three total emails (i.e. two emails at 5 mbs each and a third email sized at 1 megabyte).
RFI #6 - Reference page 51 - In what format do you accept final proposal submissions? (Word vs. PDF, etc)
VA Answer #6 – Word or PDF only.
RFI #7 - Reference page 51 of the solicitation – Regarding the maximum number of pages at 35, in consideration of the required ancillary documentation listed below, would the restriction apply solely to the main 3 section responses to the request for proposal (RFP)? Allowing for additional reference “addendums” beyond the 35 page count?
VA Answer #7 – In consideration of the edits made below, the page count will only be increased by 15 pages, from 35 to 50 pages. This includes any and all pages, except spreadsheets, unless stated otherwise within the RFP or previous amendments.
RFI #8 - Reference page 16-17 of the RFP – 6 ATTENDANTS
0. New driver records shall be made available to the Contracting Officer and/or COR along with proposal. Any offeror failing to submit proof of these records with its proposal shall be deemed technically unacceptable and that offeror shall not be considered for the award. An actual document or other proof is required. Any mere reference by an offeror in its proposal stating that it has such certification is not proof and the proposal shall still be rendered technically unacceptable and shall not be considered for award.
Contractor’s Recommendation: We would recommend a spreadsheet listing our staff who would operate vehicles in performance of this contract, with the expiration of their driver’s license, will this meet the requirement?
VA Answer #8 – Yes, this would be in lieu of the provision of driving record copies applicable to RFI #8. Be advised, all driving records may be required to be provided upon request by VA personnel during administration of the potential contract. This answer applies to sub-factors 1 and 3 found within Factor 2 that is located on page 57 and 58 of the solicitation if the information being provided pertains to offeror employees NOT the offeror and potential subcontractors.
RFI # 9 - Reference page 17-18 – EMERGENCY MEDICAL TECHNICIANS (EMT):
0. Contractor Emergency Medical Technicians (EMTs) who perform emergency services on ambulances servicing the Nashville and Murfreesboro VAMCs and CBOCs shall have the following qualifications. Any offeror failing to submit proof of these certifications with its proposal shall be deemed technically unacceptable and that offeror shall not be considered for the award. An actual document or other proof is required. Any mere reference by an offeror in its proposal stating that it has such certification is not proof and the proposal shall still be rendered technically unacceptable and shall not be considered for award:
1. EMTs must have completed training in accordance with the standards published by the Tennessee Department of Public Health, Department of Health and Human Services. Such training programs must be acceptable under the regulating requirements for local EMS Systems supported by DHHS under PL 93-154.
1. The “equivalent” training program successfully completed by the EMT must be submitted to the Contracting Officer for appropriate action/review with the proposal.
1. The EMT must be certified, licensed or otherwise officially recognized by the Local, State, or Regional Government or public entity where the emergency ambulance service is operated, or by which it is governed. Certificate shall be submitted with proposal.
1. The EMT must be enrolled periodically in “refresher” continuing education, or advanced training programs as required by the Local or State government entity in which the service is rendered to veterans, but in no instance shall this be less frequently than every two years. Such “refresher” training must be equivalent of that developed by the Department of Transportation, National Highway Safety Administration. Proof of such refresher training must be submitted to the COR when updated periodically as required by the governmental entity.
Contractor’s Recommendation: Ambulance services are not required to maintain copies of original EMT school diplomas, equivalent training, nor evidence of refresher training. These are required by, and maintained by the State Department of Public Health EMS Bureau as part of the licensing process. We do maintain copies of mandated on-going training such as Emergency Vehicle Operator Course, CPR, etc. We would recommend a spreadsheet listing our staff who have a clinical role in performance of this contract, with expiration dates of these certifications along with their license number, will this meet the requirement?
VA Answer #9 – Yes, this would be in lieu of the provision of certifications/certificates, licensing, and training applicable to RFI #9. Be advised, all EMT certifications/licensure and proof of completed training shall be provided to VA personnel after contract award, if requested. This answer applies to sub-factors 1 and 3 found within Factor 2 that is located on page 57 and 58 of the solicitation if the information being provided pertains to offeror employees NOT the offeror and potential subcontractors.
RFI #10 - Reference page 18 - PARAMEDICS
0. Paramedics providing emergency services on ambulances servicing the Nashville or Murfreesboro VAMCs and CBOCs shall have the following qualifications.
2. Complete training in accordance with the standards published by the Tennessee Department of Public Health to be licensed in the state of Tennessee.
2. The paramedic must be certified, licensed or otherwise officially recognized by the Local, State, or Regional Government or public entity where the emergency ambulance service is operated, or by which it is governed. Proof of such certification must be submitted with proposal.
2. The paramedic must be enrolled periodically in “refresher” continuing education, or advanced training programs as required by the local or state government entity in which the service is rendered to veterans, but in no instance shall this be less frequently than every three years. Such “refresher” training must be equivalent to that developed by the Department of Transportation, National Highway Safety Administration. Proof of such refresher training must be submitted to the COR for appropriate action / review as required by governmental entity.
Contractor’s Recommendation: Same recommendation as above, ambulance services are not required to maintain copies of original Paramedic school diplomas, equivalent training, nor evidence of refresher training. These are required by, and maintained by the State Department of Public Health EMS Bureau as part of the licensing process. We do maintain copies of mandated on-going training such as Emergency Vehicle Operator Course, CPR, ACLS, etc. We would recommend a spreadsheet listing our staff who have a clinical role in performance of this contract, with expiration dates of these certifications along with their license number, will this meet the requirement?
VA Answer #10 – Yes, this would be in lieu of the provision of certifications/certificates, licensing, and training applicable to RFI #10. Be advised, all paramedic certifications/licensure and proof of completed training shall be provided to VA personnel after contract award, if requested. This answer applies to sub-factors 1 and 3 found within Factor 2 that is located on page 57 and 58 of the solicitation if the information being provided pertains to offeror employees NOT the offeror and potential subcontractors.
RFI #11 - Reference page 23 – TRAINING PROGRAM

Any offeror failing to submit proof of these certifications with its proposal shall be deemed technically unacceptable and that offeror shall not be considered for the award. An actual document or other proof is required. Any mere reference by an offeror in its proposal stating that it has such certification is not proof and the proposal shall still be rendered technically unacceptable and shall not be considered for award Contractor’s Recommendation: Much of this training is covered during our New Employee Orientation Program or as part of the basic EMT curriculum (body mechanics, transporting and assisting disabled passengers and age related personal interactions), and Blood Borne Pathogens is completed annually along with Work Place Violence Prevention, Fire Safety, Sexual Harassment Prevention, and Compliance. Would an attestation from senior leadership stating that we have met this requirement, along with records available upon demand, meet this requirement?
