About this file
This is an Experience Information Sheet template used to document contractor past performance on relevant projects as part of a federal solicitation response.
The form requires offerors to submit exactly four (4) completed project examples, with submissions exceeding this limit not evaluated and fewer than four potentially resulting in lower ratings. For each project, offerors must provide comprehensive contractor information including firm name, address, DUNS number, phone, and points of contact with email addresses. Required contract details include the contract/task order number, award date, project title/name and location, and identification of work performed as either prime contractor or subcontractor with the percentage of work performed by the offeror's company. If performing as a subcontractor, complete prime contractor information with DUNS and CAGE codes must be provided, along with the government agency that awarded the contract and relevant contracting officer or procurement official contact information including name, title, email, and phone number.
The form requires detailed narrative descriptions of the project scope, offeror's specific roles and responsibilities, and how the work performed relates to the current solicitation requirements. Offerors must also describe any difficulties, obstacles, or differing site conditions encountered and the resolution steps taken. Performance period documentation must include contract start date, original completion date, actual completion date, and certification that the project is 100% complete, with explanation if incomplete. Additional required information includes notation of any contract termination with reasons, original contract price (award amount), and final contract price including all modifications with explanation of differences. Continuation pages are available for additional information as needed.
