About this file

This document is a Standard Form 30 (SF-30) Amendment of Solicitation/Modification of Contract for a Durable Medical Equipment (DME) contract with the Department of Veterans Affairs Network Contracting Office 8 in the Southern Zone, covering Miami and West Palm Beach VA Healthcare Systems. The amendment modifies the original solicitation (36C24825Q0251) by updating the Price Schedule, Performance Work Statement, and providing answers to potential vendor questions. Key changes include revising tuberculosis testing requirements to be performed only upon hire unless there is a known exposure, clarifying driver's license documentation requirements, and extending the offer submission deadline from March 21, 2025, to March 26, 2025, at 4:00 PM EST.

The contract covers a base period from October 1, 2025, to September 30, 2026, with four additional one-year option periods through September 30, 2030. The estimated contract value is $3,000,000, with services including storage, delivery, setup, repairs, and pickup of durable medical equipment for VA beneficiaries. Pricing is structured around various line items such as emergency and non-emergency trips, equipment maintenance, repair rates, and grab bar installations. The current contractor is Warrior Service Company, LLC, and the contract is for the Southern Zone of VISN 8, with a Principal NAICS Code of 621610 (Home Health Care Services).

Q1. QUESTIONS AND ANSWERS

1. In looking at the old contract from FY 2000, all funding sources (SAM.gov, FPDS.gov, and

USASpending.gov) point to only $52,910 being paid against the contract. What has been the total spend on the current contract?

Answer: The estimated contract value is $3,000,000.00. Orders were placed by Government Purchase Cards.

2. CLINS 0001/1001/2001/3001/4001. Please split this line item into 2 line items – one line item for each VAMC. This will help to determine proper staffing levels for each warehouse.

Answer: Reference Section B.2 Price Schedule for changes.

3. CLINS 0001, 1001, 2001, 3001, and 4001 – the unit of measure shows “MO”, but the description of supplies/services clearly shows it is a job. Recommend changing the unit of measure to “JB” or “EA.”

Answer: Reference Section B.2 Price Schedule for changes.

4. CLINS 0005, 1005, 2005, 3005, and 4005

a. The unit of measure shows “EA,” but the description of supplies/services clearly shows it as an hourly charge. Recommend changing the unit of measure to “HR.”

b. The quantity shows 17. Is this correct? The quantity seems very low to repair DME for two separate VAMCs.

Answer: a. Reference section B.2 Price Schedule for changes.

b. The quantities are correct, the line items are specifically for complex repairs which are infrequent.

5. CLINS 0007, 1007, 2007, 3007, and 4007 – the unit of measure shows “EA”, but the description of supplies/services clearly shows it is a job. Recommend changing the unit of measure to “JB.”

Answer: Reference section B.2 Price Schedule for changes.

6. Will the contractor be required to do a fall risk assessment? (As a point of reference, the James A Haley does, while the CW Bill Young, Lake Nona, and Malcom Randall VAMCs do not.)

Answer: No, the contractor will be required to conduct and document a Basic Home Safety assessment.

7. Recommend changing the 24-hour requirement in paragraph 13.1 on page 21 to 1 business day.

This will allow compliance with the business hours stated in paragraph 7 since it would cover orders received on Friday or the day before a federal holiday to be made the next business day.

Answer: Requirement stands as is. Afterhours, weekend and/or holiday are considered when monitoring for service timeline compliance.

8. Regarding Key West, does this location still fall under the four (4) hour emergency response or the eight (8) hour emergency response?

Answer: The Emergency trip line item specifies a delivery timeline of 4-hours up to 8-hours, this line item and specified timeline does apply to Key West.

13. 3.1 Delivery charges under this contract shall be based on a flat rate zone charge. The zone radiates out from each VAMC/VHCS Facility in the VISN 8 Southern zone and are based on the counties surrounding each facility. Refer to Attachment A located in Section D.

Question: This statement in the PWS is suggesting we are to charge a flat rate charge per zone.

Since there are 2 VAMCs associated with this contract, there should be 2 line items, one for each zone. Please clarify.

Answer: Contract is for the Southern Zone of VISN 8 which encompasses the Miami and West Palm Beach VA Healthcare Systems therefore the one flat rate will apply to both facilities and their respective catchment areas (counties).

14. 3.3 Contractors who provide DME contract coverage for the southern zone may have limited situations where they shall need to pick-up/deliver outside of their service area(s). There shall be NO extra charge for pick-up/delivery with the exception of additional mileage charges (Example:

Delivery/pick-up for a patient who may live in the central zone but receives services from a southern zone contractor). There shall be NO charge for pick-up/transfer of equipment from a northern zone vendor facility to a central or southern zone vendor facility and vice versa regardless of if it is the same or a different Contractor.
