A.1 RFI QUESTIONS & ANSWERS

RFI QUESTION & ANSWERS

Q1: Factor 3: Tab B. Past Performance & Experience, Section B: Identify a minimum of 3 projects, but no more than 5 projects from the following Experience categories Do we need to provide 3-5 projects for each category (1-3), OR do we provide 3-5 projects in total?

ANSWER: See revised Evaluation Criteria (Submit a minimum of 1 but no more than 3 references.)

Q2: Factor 3: Tab B. Past Performance & Experience, Section B, part 1:

Team Experience: Identify any projects that the designer and (prime) constructor have previously contractually teamed together to accomplish.

0. Constructor/Key Subs and/or Designer: Include examples, within the last 3 years (5 years for design firms), of design/build projects, in which the designer and/or constructor or key trade subcontractors have been involved, if any.

0. Teaming Arrangements: If Offeror represents the combining of two or more companies for the purpose of this RFP, then each company must list project examples, related to its role for this project.

When submitting as a Joint Venture, do we need to show work together as a team (Prime/Sub) on a similar project?

ANSWER: Show work together as a team or individually.

Q3: When showing work together as a prime, do we also need to display a project that shows experience working with our key subconsultants?

ANSWER: If the same key subconsultants are being used for this project, then yes.

Q4: Phase 1 requires a 24” x 36” schematic design of the Mental Health department. Although the solicitation references several VA Design Guidelines – Mental Health and the PACT Module Design, it is not clear which components of a Mental Health Outpatient Clinic are desired. Besides Mental Health Clinic, the Design Guide lists Substance Abuse Clinic, Psychosocial Rehabilitation and Recovery and Psychology Services. Are these components desired?

ANSWER: Schematic Design requirement removed. See revised Evaluation Criteria.

Q5: There is no detailed Statement of Work provided in this Solicitation. Can the VA provide one? The real issue is that our Architect is having difficulty as the statement of work does not define specifics related to what you want in the project. We are to provide a schematic layout with the Phase 1 proposal but there does not seem to be enough information to accurately provide this layout.

ANSWER: SOW is included in the Solicitation (A.4 DESIGN BUILD STATEMENT OF WORK). For additional space requirement please see attachment “LB Space Worksheet”. Schematic Design requirement removed. See revised Evaluation Criteria.

Q6: There is no general Program for this Solicitation. Can the VA provide a general program of what they are looking for within these two floors?

ANSWER: See attached “LB Space Worksheet”

Q7: Is this Mental Health Facility considered an Outpatient Facility?

ANSWER: Yes Outpatient only!

Q8: If this is considered an Outpatient facility, does the VA require the four-primary mental health outpatient components – Mental Health Clinic, Substance Abuse Clinic, Psychosocial Rehabilitation Recovery Center (PRRC), and Psychological Services?

ANSWER: Yes, plus several others.

1. Behavioral Health Lab (BHL)

2. Neuropsychology

3. Vocational Rehabilitation

4. Behavioral Health Interdisciplinary Teams (BHIP)

5. Psychosocial Rehabilitation and Recovery Center (PRRC) & Peer Support Program

6. Substance Use Disorder/Intensive Outpatient Program (SUD-IOP)

7. Administrative Space

8. DOM Clinician/Admin Overflow

9. VITAL PROGRAM

Q9: If these 4 components are required for this outpatient facility, how many Exam Rooms, Individual and Group Therapy Rooms, Staff Offices, Labs, Conference Rooms, Group Rooms, Classrooms, Observation Rooms, Clinics, Multi-purpose Rooms, etc. are required?

ANSWER: see attached “LB Space Worksheet”

Q10: If there is not a set program, is it the design intent to get as much into these 2 floors as possible and the DB team determines that in the Schematic Layout provided in Phase 1?

ANSWER: See attached “LB Space Worksheet”

Q11: Will the design include a more detailed programming phase, then a schematic floor plan layout phase, then Design Development, and then Construction Documents?

ANSWER: Please refer to Solicitation section VIII DESIGN AND DOCUMENTS REQUIREMENTS (Program Guide, PG-18-15, Volume C)

Q12: Will CAD of Floors 3 and 4 be available for the Phase 1 submittal?

ANSWER: CAD drawings may be provided during Phase 2 if necessary.

Q13: Referencing RFP proposal requirement - Factor 3: PAST PERFORMANCE AND EXPERIENCE (Tab D)

A. Relevant and present Past Performance of the offeror’s team (including the architect-engineerand construction members); Relevant performance means same or similar projects in any type ofclinical area which require surrounding medical functions remain active and uninterrupted, and similar price range. Recent is defined as performance within the last five (5) years throughthe solicitation release date. Submit a minimum of 3 but no more than 5 references.
B. Identify a minimum of 3 projects, but no more than 5 projects from the following Experiencecategories:

Question: Typically, a “Contractor Experience Form” is utilized to provide the information requested. Can the VA provide this form or state how to submit this information with our proposal?

ANSWER: We don’t have a particular form to provide. Please submit in narrative form identifying the information referenced in this paragraph.

Q14: Please issue the site visit attendance roster.

ANSWER: See attached ”Attendance Roster”

Q15: Factor 3, Part C (page 10) of this solicitation requires past performance questionnaires to be completed for each project submitted. As part of our close-out procedures, we acquire CPARs for each of our projects from the CORs. Would CPARS be accepted in lieu of PPQs?

ANSWER: You may submit a copy of CPARS if you prefer, however, the PPQ’s address different questions than CPARS.

Q16: I wanted to also inquire to see if we can have access to the as-built drawings for this project?

ANSWER: As-built drawings were attached in original solicitation labeled 3rd Floor Plan and 4th Floor Plan

Q17: Factor 4, pg. 11 – […] The information shall contain, at a minimum, a certification that the bidder/offeror has no more than three (3) serious, or one (1) repeat or one (1) willful OSHA or any EPA violation(s) in the past three years. […] Is it acceptable for offerors to include this certification on the Past Safety and Environmental Record Form?

ANSWER: Yes. The sample form was attached on page 12 of the solicitation.

Q18: Factor 4, page 12 -- Past Safety and Environmental Record Form. Does the VA want OSHA 300 and 300a Forms for 2014 through 2017 or just for the most recent year?

ANSWER: One for each year.

Q19: Factor 1: TECHNICAL APPROACH A (page 9) – are the D-sized layout pages exempt from the 5-page limit? If not, how many pages does this count for?

ANSWER: Requirement removed. See revised Evaluation Criteria.

Q20: Factor 1: TECHNICAL APPROACH B (page 9) – is the CPM schedule exempt from the page count, as this could easily be two pages? Are offerors allowed to use 11x17 pages to present the schedule, and how many pages is this against the page count?

ANSWER: Requirement removed from Phase 1. See revised Evaluation Criteria.

Q21: Factor 1: TECHNICAL APPROACH C (page 9) – is this written section what is subject to the 5-page limit in this Tab?

ANSWER: See revised Evaluation Criteria.

Q22: Sub Factor 2(b): LICENSING C (page 10) – Is this reporting for licensing and litigation information in the last 5 years, like Project Performance?

ANSWER: See revised Evaluation Criteria.

Q23: Factor 3: PAST PERFORMANCE AND EXPERIENCE – Section A defines “Recent” is defined as performance within the last five years for both constructor and designer, but B has recent performance for the Designer at 5 years and constructor as 3 years. Please consider clarifying this requirement and make it 5 years for Designer and Constructor. For Design/Build teams that have previous experience together, the shorter time frame for the constructor can eliminate some experience of the Constructor working with this designer.

ANSWER: See revised Evaluation Criteria.

Q24: What is the Mental Health IPT? (Is this the Mental Health Design Guide Document, or something else?)

ANSWER: IPT: Integrated Project Team

Q25: Does the VA have a specific program for the needs of this building? The Design Guide’s project is geared towards if we were doing a new free-standing building. Most of the questions below are all related to if we had a program of what exactly the VA wants and sizes then we can design it within the SF we are given on both floors. Additionally, the TIL has general information not specifics which would be provided by the individual VA Hospitals.

ANSWER: see attached “LB Space Worksheet”

Q26: Are administration areas served in other floors of the building? As well as cafeteria, kitchen, lockers, changing rooms or anything else that may be a requirement for the staffs? Or do we need to include that type of spaces in the proposal?

ANSWER: Please use reference PG-18-9: Space Planning Criteria Chapter 260, 272, & 202 and LB space worksheet for required functional and supporting spaces.

Q27: What type of rooms are required? (Examination, consultation, procedure) and approx. how many of each?

ANSWER: Refer to LB Space Work Sheet!

Q28: Do we need rooms for inpatients? The Design Guide references ‘Inpatient’ and ‘Outpatient’ services, are we to provide both in this project?

ANSWER: No, this space is strictly outpatient.

Q29: Are the group care rooms a necessity? How big do they like them to be? And how many of them would be required?

ANSWER: Yes, they are required. They are specified in the Design Guide.

Q30: Do we need space for a lobby/reception per floor?

ANSWER: Yes.

Q31: Do we need a waiting area?

ANSWER: Yes, each floor.

Q32: How much support spaces for storage and medical equipment required?

ANSWER: Please use reference PG-18-9: Space Planning Criteria Chapter 260, 202, & 272 and LB space worksheet for required functional and supporting spaces.

Q33: We would like to see if we can eliminate some of the mechanical chases on the 2 floors, is this an option we can pursue if we can verify with Engineers that any items in the chase can in fact, be moved. Or do we need to design around all existing chases to remain as is?

ANSWER: Since this is a design build project, any design options will be reviewed for consideration during the schematic design phase.

Q34: Please provide all AS‐builds for the floors affected to properly ascertain a layout based on MEP constraints.

ANSWER: As-built drawings were attached in original solicitation labeled 3rd Floor Plan and 4th Floor Plan

Q35: Can you please confirm that the Phase I and Phase II proposal requirements aren’t reversed? You are asking for a lot of upfront work for a phase I proposal with no guarantee of making phase II. To provide a proposed design schematic for a project of this magnitude in a Phase I proposal will cost each contractor a substantial amount of money. Typically phase I am more of a qualification phase.

ANSWER: See revised Evaluation Criteria.

Q36: Regarding " Factor 4: SAFETY/ENVIRONMENTAL VIOLATIONS AND EXPERIENCE MODIFICATION RATE"

1) If a Joint Venture is submitting the proposal, please confirm if both members of the JV need to submit the (EMR) for the past 3 years.

ANSWER: A JV is considered 1 entity, therefore, EMR is needed for the JV. Suggest contacting the insurance company. It’s possible Joint ventures are eligible for experience rating based on the mathematical averages of the experience modification factors of the member companies in the beginning. Contractors that cannot provide their EMR rating on insurance letterhead must obtain a rating from the National Council on Compensation Insurance, Inc.

2) If one of the members of the JV has an EMR greater then 1, please confirm if you would accept an explanation letter from the offeror's insurance agent's company explaining why the EMR is above 1 and not disqualify the offeror.

ANSWER: No.

Q37: For mechanical utilities, please clarify/define the extent of renovation beyond the physical work space required to tie into existing building systems. For example, shall all ductwork be replaced and reconfigured back to existing VAV’s, yet existing VAVs re-used or shall all VAVs be replaced with new or shall all ductwork be replaced back to dedicated AHUs/RTUs, etc.?

ANSWER: All VAVs will be new including ductwork and associated devices.

Q38: In Factor 1: Technical Approach and Sub Factor 2(a): Specialized Experience and Technical Competence that there will be a narrative page restriction.

Do charts/tables count towards the narrative page count?

ANSWER: See revised Evaluation Criteria.

Q39: Factor 2 – Technical Qualifications; sub factor 2(b) Licensing: A. requires for a current General Contractor’s License to be submitted. Our firm is based in Kentucky which is a state that does not have a General Contractor’s Licensing program. We only perform GOV contracts within Federal properties and in the past, we have not been required to have any type of license when performing at any VAMC. We are currently performing work at Federal installations in 5 different states. Our firm provides construction services to VA at several VISN’s and it’s qualified to perform the SOW for this RFP.

Question/Request: Semper Tek respectfully requests that Factor 2, sub factor 2(b)A. be reconsidered, since work is being performed within a Federal property which does not require to have a GC License, thus providing an open and fair competition approach for SDVOSB companies that are based in states which do not provide a way for firms to obtain a General Contractors License.

ANSWER: Requirement removed. See revised Evaluation Criteria.

Q40: Is there a separate page limit for Sub factor 2b? Sub factor 2a lists a 3-page maximum. Is Sub factor 2b meant to fit into that page limit? Based on what is requested, that seems like too few pages. Please clarify.

ANSWER: See revised Evaluation Criteria.

Q41: Is there a Page Limit for Tab D – Past Performance?

ANSWER: See revised Evaluation Criteria.

Q42: Within Tab D, is the relevant work requested in A. (3-5 projects from past five years) the same or in addition to the 3 to 5 projects requested in B. from the three different Experience Categories. Should there be a total or 6 to 10 projects submitted? Or is it 3 to 5 projects total?

ANSWER: See revised Evaluation Criteria.

Q43: In Tab D, C. May CPARS be included in the proposal as an example of the client’s satisfaction instead of a PPQ that would be emailed to you?

ANSWER: You may submit a copy of CPARS if you prefer, however, the PPQ’s address different questions than CPARS.

Q44: At site visit, it was mentioned that CADD files are available for RFP provided (existing conditions). Can the CADD files be provided for Solicitors to generate the required “schematic Layout” as part of Factor 1?

ANSWER: See revised Evaluation Criteria

Q45: At site visit, it was mentioned that the design packages for 3rd floor can be completed first, reviewed and approved to have an early release to construction, since the 3rd floor will be constructed first in sequence. Then, the 4th floor design packages can be submitted while the 3rd floor construction is underway. Please confirm that this approach will be accepted since it has an effect on how each Solicitor responds to Factor 1, B. CPM Schedule submittal.

ANSWER: Yes, that is the plan to phase the design and construction.

Q46: Is this for Mental Health and Behavioral Patient Care Units, or a Mental Health Clinic, or both? Outpatient and in-patient care?

ANSWER: Outpatient only. No In-Patient!

Q47: Are there units here that will be more secure than others, with different construction and access features? Are levels of treatment employed, such as “acute,” “sub-acute”, etc.

ANSWER: No

Q48: Is this for men and women? Is treatment and housing for men and women separated?

ANSWER: This is an outpatient clinic for men and women. No housing required (inpatient clinic).

Q49: Are there particular needs for in-patients in terms of their environment? Is there to be some efforts at normalization? Direct supervision?

ANSWER: No In-patient! Outpatient only!

Q50: Is there to be an assessment area related to those with criminal charges?

ANSWER: This is an Outpatient Clinic for the Veterans of our Armed Forces, not a prison!

Q51: Reference: Phase 1 Evaluation Factors and Proposal Requirements Factor 1: TECHNICAL APPROACH (Tab B) – (Narrative 5 pages max) Question: Do the schedule and schematic count towards the 5 pages?

ANSWER: Requirement removed. See revised Evaluation Criteria.

Q52: Reference: Phase 2 Proposal Preparation and Submission: TAB C: Phase 2 TECHNICAL PROPOSAL Question: Does the Quality Control Manager (QCM) have to be a separate individual from the site superintendent or can the QCM & superintendent have collateral duties?

ANSWER: There’s no restriction as long as the individual is qualified to perform the task.

Q53: Reference: RFP Scope of Work Question: Please verify if a Certified Independent Third-Party Safety Professional (CITP) is required for this project?

ANSWER: Yes!

Q54: Reference: RFP Scope of Work – C. Project Overview:

Question: The verbiage in C 3. (a) states “The contractor will construct offices, restrooms, conference rooms, group rooms and all other spaces required by the Mental Health IPT.” Has a complete requirements list been developed by the ITP and if so, will this be provided for Phase 1? Examples: total number of VA personnel that will staff each floor; anticipated number of Veterans using the spaces so a complete understanding of room sizes, seating etc. can be planned.

ANSWER: see attached “LB Space Worksheet”

Q55: Reference: RFP Scope of Work – C. Project Overview:

Q56: Reference: RFP Scope of Work – C. Project Overview:

Question: The verbiage in 3. e states the use of “Orlando VA Medical Center (Environmental)Facilities Management Services Interior Designer established standards.” Are these standards available, and if so, will this be provided for Phase 1?

ANSWER: It may be provided in Phase 2

Q57: Reference: RFP Scope of Work – C. Project Overview:

Question: The verbiage in C 3. (f) states “Mechanical work shall provide all renovated spaces with new mechanical equipment, tied into existing building systems as appropriate.” Is it the VA’s intent for the Mechanical SOW to tie into the existing mechanical system(s)? ANSWER: Yes, the Mechanical SOW will tie the new mechanical system to the existing system via all new R/A ducts and S/A ducts as needed. The equipment for these two floors resides on our Mechanical deck which is below the two floors.

Examples: AHU’s, controls, main ducts. And if so, would the DB Contractor be required for Phase 1 to determine if the existing systems would meet the intent of the redesigned space and propose based solely on the concept of tying into the existing systems. Clarification is needed on if complete new systems for these two floors is the intent? ANSWER: NO! The existing HVAC units for these two floors are in place. Only the returns and supply’s will need to be modified to fit the proposed design.

Please provide this clarification and any Mechanical as-builts for the entire facility that reflect the existing systems for Phase 1.

ANSWER: As-Builts for the entire Facility are not needed. Mechanical As-Builts for 3rd and 4th floor may be provided during Phase 2 if needed.

Q58: Reference: RFP Scope of Work – C. Project Overview:

Question: The verbiage in C 3. (g) states “Electrical work shall provide all renovated spaces with new electrical equipment, tied into existing building systems as appropriate.” Is the intent for the Electrical SOW to tie into the existing electrical & special system(s)? Examples: Existing EL panels serving both floors are adequate and have capacity, Life Safety panels exist and are adequate, and any lighting controls and/or special systems have capacity for the redesigned spaces? And if so would the DB Contractor be required, for Phase 1, in determining if the existing systems would meet the intent of the redesigned space and propose based solely on the concept of tying into the existing systems. Clarification is needed on if complete new systems for these two floors is the intent? Please provide this clarification and any Electrical, Comm., Security, Nurse Call and other existing special system as-builts for the entire facility that reflect these existing systems for Phase 1.

ANSWER: Yes, It Is the intent for the Electrical SOW to tie into the existing electrical & special system(s). Understanding that the system existing now may not totally support all new electrical system’s to be proposed and so it would be expected that the Electrical design would take that into consideration in their design. As-built drawings may be provided during Phase 2 if needed.

Q59: Reference: Solicitation VIII- DESIGN AND DOCUMENT REQUIREMENTS: B. 2. Design Review Meetings:

Question: The verbiage in B. 2 states “The DB team members will each allow for (1) full day for each discipline/package design review meeting.” Is the intent to meet individually on separate days to review the discipline specific package or is the intent to have the entire design team for a day for any given design package? Please clarify.

ANSWER: Design review is conducted per design submittal (50%, 75%, 100%, ….) which covers all discipline and usually takes 2 to 3 hours. The entire design team shall be present.

Q60: Will the project include system furniture? Example: modular office furniture, cubicles & desks. And if so has type, size, number of units been decided/determined and who will provide? Please provide this clarification for Phase 1.

ANSWER: Possibly, this will be determined by the Interior Designer for the VA.

Q61: Reference: Solicitation VIII- DESIGN AND DOCUMENT REQUIREMENTS: B. Supplemental Requirements: and D. Electronic Media Question: Various locations in the solicitation have “AutoCad version 2010 and backwards compatible” and other areas has AutoCad version 2013. Please clarify which is correct?

Q62: Reference: Division 01 General Requirements: 1.6 OPERATIONS AND STORAGE AREAS - I. Construction Fence:

Question: This section states “Contractor shall provide a chain link construction fence, 2.1m (seven feet) minimum height, around the construction area indicated on the drawings.” Please verify if “(seven feet) minimum height” is correct?

ANSWER: Yes, Minimum 7’-0”

Q63: Reference: Division 01 General Requirements: 1.13 PROFESSIONAL SURVEYING SERVICES:

Question: This section mentions that the GC shall have a registered professional land surveyor or registered civil engineer… Is this correct?

ANSWER: I’m sure this is not needed as we are only working inside the building.

Q64: Reference: Division 01 General Requirements: 1.31 PHOTOGRAPHIC DOCUMENTATION and 1.32 FINAL ELEVATION DIGITAL IMAGES Question: Are these sections correct and are required for this project?

ANSWER: NO

Q65: Does the VA plan to store waste “Soiled Holding” in the new areas or completely remove waste to a central waste/soiled holding location elsewhere?

ANSWER: Soiled Holding Area/room is required.

Q66: Is there currently a Pneumatic Tube system that is in use on floors 3 and/or 4? If not, does the VA require one for this new renovation project for each of the floors? If there is an existing system, but is not required as part of the SOW is the contractor to remove any such system back to a certain point?

ANSWER: None existing and none required!

Q67: Will Duress buttons be required for any of the consult or exam rooms?

ANSWER: Yes, refer to design guides

Q68: Are there any FFE furniture being provided by the VA? If so can this be clarified?

Q69: Are there any TV monitors being provided and installed by the VA for this project? If so can this be clarified by types and locations? Examples: Waiting areas, lobby, team work zone etc.

ANSWER: Yes, there will be TV’s in waiting rooms and possibly group and work rooms provided by the VA.

Q70: It was mentioned at the site visit that the project will need to be phased. Can the VA provide the specifics for how the phasing is planned?

ANSWER: Currently the Fourth floor is occupied by our Mental Health Outpatient clinic. It will be planned that design and construction be phased so that the design for the third floor be completed first so the Mental Health team could relocate to the completed third floor space while the fourth-floor space is designed and constructed. At the time of the completion of the fourth floor they would relocate to specific programmed space.

Q71: It was mentioned at the site visit that even though code or otherwise may not require each floor to be zoned the VA still wanted to maintain 2 zones per floor. Can the VA be more specific on the intent for zones on these floors?

ANSWER: Each floor is divided into two distinct Fire Zones. 301, 302. 401, 402. Please see attachment “Fire Zones”

4. Phase 2 Evaluation and Award Determination:

(a) This is a competitive Phase 2, Request for Proposal (RFP) that represents the best value to the Government conducted under FAR Part 15 101-1, Tradeoff Process.

(b) In accordance with FAR 15.101-1, all evaluation factors, other than cost or price, when combined, are significantly more important than cost or price.

(c) Thus, award will be made to the firm whose Phase 1 proposal was initially ranked in the top 5 and who now offers a Phase 2 proposal, which based on both its technical and pricing proposals, offers the overall best value to the Government. Should your company be awarded a contract based upon a successful rating received from all evaluation factors to include any teaming arrangements, it is imperative that this team remain intact from initial award as the evaluation was based upon this teaming arrangement. If there is any change after award, you must notify the Contacting Officer and COR with specific reasons.

(1) Each pricing element indicated in TAB B will be evaluated to determine its reasonableness. The Government may use various price analysis techniques and procedures to make a price reasonableness determination. Offerors should assure that their ability to meet or exceed the minimum needs of the Government is adequately described in the offer. The Government intends to evaluate proposals and award a contract without discussions with offerors (except clarifications as described in FAR 15.306(a)). Therefore, the offeror’s initial proposal should contain the offeror’s best terms from a cost or price and technical standpoint. The Government reserves the right to conduct discussions if the Contracting Officer later determines them to be necessary. The Government considers it to be in its best interest to allow consideration of award to other than the lowest priced offeror or other than the highest technically rated offeror.

(2) The Government will evaluate price based on the total price. Total price consists of the basic requirement which may include alternate bid items such as, deductive, or options. Analysis will be performed by one or more of the following techniques to ensure a fair and reasonable price:

i. Comparison of proposed prices received in response to the RFP.

ii. Comparison of proposed prices with the IGE.

iii. Comparison of proposed prices with available historical information.

iv. Comparison of market survey results.

(d) Evaluation Overview: The Government intends to award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The proposals must conform to the RFP's requirements and judged to represent the Best Value to the Government. The Best Value is the most advantageous offer, price and other factors considered, and consistent with the Government's stated importance of evaluation criteria. This may result in award being made to a high-rated, higher-priced offeror where the Contracting Officer determines that the Technical capability of the higher-priced offeror outweighs the price.

(e) SDVOSB Verification: The Government will review VetBiz (https://www.vip.vetbiz.gov/) to confirm Service Disabled Veteran-owned Small Business (SDVOSB) status verification for the submitted proposals. Evaluations will be performed on proposals submitted by verified SDVOSBs only. Subsequent confirmation of SDVOSB verification status will be performed for the apparent award.
