About this file
This document is a Quality Assurance Surveillance Plan (QASP) for the Augusta VA Healthcare System's Home Oxygen and Ventilator Services contract. The plan outlines performance standards and surveillance methods for a contractor providing home oxygen and ventilation services to veterans in VISN 7, specifically at the Augusta Veterans Medical Center. Key performance requirements include timely prescription confirmation, delivery notifications, patient education, emergency service response within 4 hours, equipment maintenance, and compliance with CDC COVID-19 procedures and Joint Commission standards.
The QASP details comprehensive monitoring methods including direct observation, periodic inspections, user surveys, customer complaint validation, and random sampling. Performance will be evaluated against specific metrics with acceptable quality levels ranging from 90-100% compliance across various service requirements. The contractor will be required to submit multiple types of reports on a daily, monthly, quarterly, and annual basis, with potential corrective action plans if performance standards are not met. The Contracting Officer's Technical Representative (COTR) will conduct quarterly performance assessment meetings to review the contractor's performance and address any discrepancies.
