EVALUATION CRITERIA

Polymerase Chain Reaction (PCR) Equipment/System to Perform Molecular Diagnostic Patient Testing (MRSA, c.Diff, Influenza, and CT/NG) Through a Cost-Per-Test Contract/Task Order

**ALL factors must be rated ACCEPTABLE to be considered for award.

Non-Cost/Price Factors

1. Technical Approach - Technical approach to complete the requirement for PCR Equipment to perform diagnostic patient testing in accordance with the Statement of Work (SOW). Offeror’s shall address how they intend to meet the requirements of the contract IAW SOW. Capability statement shall include the following:

1. Demonstrate the ability to meet the entire SOW for this particular contract.

1. Demonstrate ability to simultaneously run all required tests on a single PCR testing platform.

1. Demonstrate ability to provide walk-away capability and ease of use, including minimum set up time, automated sample barcoding and minimal time to perform daily, weekly, and monthly maintenance.

1. Demonstrate ability to meet Reagents & Supply Requirements to include Inventory Mgmt, Emergency Orders, and all other areas as stated in the SOW.

1. Demonstrate ability to meet Maintenance and Service Support requirement as per SOW.

2. Management/Quality Control Approach - Quote shall address your proposed system’s Quality Control and Management approach to perform diagnostic patient testing requirement in accordance with the SOW.

3. Relevant Experience/Past Performance: - The Relevant Experience/Past Performance evaluation will assess the relative risks associated with offeror’s likelihood of success in performing the requirement as indicated by that offeror’s record of Relevant Experience/Past Performance. Offeror must demonstrate that at least three delivery orders of similar size were performed successfully within the last three years.

In case of an offeror without a record of relevant past performance or for whom information on past performance is not available, the offeror may not be evaluated favorably or unfavorably on past performance.

Each factor will be scored as Acceptable or Unacceptable as defined below. Each quote will be evaluated against the SOW and the stated evaluation criteria. Each quote is evaluated using the same standards.

Acceptable ALL of the minimum acceptable criteria are clearly met by the quote. The offeror's quote meets the performance and technical capability requirements defined in the SOW. NOTE: Once the quotes have been determined to be "technically acceptable," award will based on cost/price only.

Unacceptable Not all of the minimum acceptable criteria are met by the quote. An unacceptable quote contains one or more deficiencies. Quote fails to meet specified minimum performance and technical capability requirements defined in the SOW
