B.4 PERFORMANCE OF WORK (PWS)

SALISBURY VAMC, LINEN SERVICE,

1.1 Description of Services

The Department of Veterans Affairs (VA) intends to establish a contract for offsite laundry and linen services. The contractor shall furnish all labor, personnel, supervision, management, equipment, transportation, materials, supplies, certification, training, and laundry facilities to provide medical laundry services to W. G. (Bill) Hefner VA Medical Center (VAMC) located at 1601 Brenner Avenue, Salisbury, North Carolina. The contractor shall collect soiled linen and laundry, provide laundry services, and deliver clean items in accordance with the scope of this Performance Work Statement (PWS).

1.2 Background

Performance Work Statement, Laundry Services for Salisbury VAMC

The W. G. (Bill) Hefner VA Medical Center (VAMC) is located at 1601 Brenner Avenue, Salisbury, North

Carolina. Since 1953, the W. G. (Bill) Hefner VA Medical Center has been improving the health of the men and women who have so proudly served our nation. They provide primary and secondary inpatient health care to more than 287,000 Veterans living in a 24-county area of the Central Piedmont Region of

North Carolina to include the Charlotte and Winston-Salem areas. They currently have an estimated

683,556 annual pounds of soiled linen and laundry. The W.G. (Bill) Hefner VA Medical Center will be referred to as the Salisbury VA Medical Center (VAMC) throughout this performance work statement.

1.3 Compliance

1.3.1 All workmanship shall be in accordance with practices and guidelines established by the

Association for Linen Management (ALM) and accepted industry standards. It is intended that services shall include all processes necessary for the laundering and finishing of the articles even though every step involved is not specifically mentioned.

1.3.2 All work shall be performed under sanitary conditions as specified by the Joint Commission on

Accreditation of Healthcare Organizations (JCAHO).

1.3.3 All workmanship shall be in accordance with practices and guidelines established by the Center for

Disease Control (CDC) and the Occupational Safety and Health Administration (OSHA).

1.3.4 Services shall include all processes necessary for the laundering and finishing of articles within medical laundry industry standards. All linen and laundry must be processed in a manner consistent with its intended use, free of wrinkles, damage, lint, stains, objectionable odors, etc.; ensuring that quality will meet or exceed expectations of this performance work statement.

1.3.5 The contractor’s facilities in which the work is done shall be open to inspection of Government representatives. These inspections may be scheduled or unscheduled.

1.3.6 Contractor shall have an established program to oversee the quality assurance of this requirement and provide a copy to the Government for concurrence within 10 days of contract award.

1.3.7 Physical separation, through the presence of a barrier wall and the use of pass-through equipment, is required to maintain positive air pressure in the clean section relative to a negative air pressure in the soiled section. In no case shall clean and soiled linen share the same physical space. The laundry plant layout/design should incorporate a “design for asepsis” whereby clean linen neither comes into contact with soiled linens nor shares the same physical space, thus avoiding cross-contamination and/or reintroduction of bacteria once processed. After processing, laundered articles shall be in serviceable and sanitary condition, meeting established quality and performance requirement standards as outlined in this PWS and in accordance with the CDC and OSHA standards.

1.4 Delivery and Pickup

1.4.1 Contractor shall provide pickup and delivery, five (5) days per week, Monday through Friday between the hours of 7:00 am and 9:00 am. The Salisbury VAMC has a designed Soiled Dock and Clean

Dock for pickup and deliveries.

1.4.2 The contractor shall pick up soiled linen/laundry for laundering and deliver clean laundry within 48 hours.

1.4.3 Clean, processed linen and laundry (from the previous days pick up) is to be unloaded by the contractor personnel at the designated clean linen and laundry dock.

1.4.4 The Laundry Manager or designee shall verify the deliveries daily and discuss any discrepancies with the Contracting Officer Representative and the Contractor’s Project Manager (CPM).

1.4.5 The contractor shall document the daily pickups. This may be in the form of a delivery ticket to be used for receiving, marking, and classifying. The contractor shall complete and provide to the Laundry

Manger or Designee a soiled laundry report before departure.

1.4.6 Upon pickup, soiled laundry will be accounted for by Laundry Manager or Government designee;

contractor personnel will record the counts and or weight on a laundry ticket supplied by the contractor.

Documentation will be accomplished prior to pick up and removal of soiled laundry by the contractor.

The contractor shall verify the counts and countersign the ticket to indicate agreement. A copy of each completed soiled laundry ticket shall be furnished to the Government.

1.4.7 Upon delivery, laundered items will be counted and or weighed by Laundry Manger or

Government designee; contractor personnel will record amounts at time of delivery. The contractor and

Government shall verify counts and jointly sign the ticket to indicate agreement with those weights. A copy of each completed clean laundry ticket shall be furnished to the Government and the contractor.

1.4.8 In the event of medical facility emergency disaster drills, the contractor shall be notified fourteen

(14) days in advance. The deliver/pick-up times may vary for a short period of performance, not to exceed seven (7) days.

1.5 Linen Control

1.5.1 Any articles invoiced, but not received by the Government, shall be supplied by the contractor in the next regularly scheduled delivery at no additional cost to the Government. Government-owned items lost or badly damaged, (except normal wear and tear) while under the control of the contractor must be reimbursed within 30 days. The COR will provide the cost of each replacement items to the contractor.

Such items shall be identified, for contractor recognition by the Laundry Manger when they are returned on the next scheduled pickup after an unsatisfactory condition is discovered. These items are to be kept separate from the normal laundry items for separate processing. Re-cleaned items will be identified separately and not charged to the Government.

1.6 Carts

The contractor shall disinfect carts after removing soiled linen from them and before placing clean linen in them, with a Tuberculocidal Germicide, which has been approved by the COR. Carts containing clean linen being returned shall be covered with clean covers to protect them against contamination while in transit between the laundry plant and the medical center. If reusable laundry bags or liners are used to transport soiled laundry, they must be laundered after use. If reusable bags are not available, the VA

Medical Center will utilize plastic bags for soiled linens, which are to be destroyed.

1.7 Rag Out Program

Salisbury VAMC reserves the right to add any additional items as needed based on the need for patient care. Any change of function by the Government, which may affect this contract as contemplated by this paragraph, and which requires permanent adjustments in frequency or type of performance, will be coordinated with the contractor by COR and CO prior to initiation of such a change to assure adequate contractual coverage by bilateral modification to the contract.

1.10 Estimated Quantities

The contractor shall provide all services that may be ordered during the contract term, exact quantities are not determined. The volume will vary depending on patient occupancy. 2.6 Contractor Performance Assessment Reporting System (CPARS)

(a) FAR 42.1502 direct all Federal agencies to collect past performance information on contracts. The

Department of Veterans Affairs has implemented use of the Contractor Performance Assessment

Reporting System (CPARS) to comply with this regulation. One or more past performance evaluations will be conducted in order to record your contract performance as required by FAR 42.15

(b) The past performance evaluation process is a totally paperless process using CPARS. CPARS is a web-based system that allows for electronic processing of the performance evaluation report. Once the report is processed, it is available in the Past Performance Information Retrieval System (PPIRS) for

Government use in evaluating past performance as part of a source selection action.

(c) Please furnish the Contracting Officer with the name, position title, phone number, and email address for each person designated to have access to your firm’s past performance evaluation(s) for the contract no later than 30 days after award. Each person granted access will have the ability to provide comments in the Contractor portion of the report and state whether or not the Contractor agrees with the evaluation, before returning the report to the Assessing Official. The report information must be protected as source selection sensitive information not releasable to the public.

(d) When your Contractor Representative(s) (Past Performance Points of Contact) are registered in

CPARS, they will receive an automatically-generated email with detailed login instructions. Further details, system requirements, and training information for CPARS are available at www.cpars.gov. The

CPARS User Manual, registration for online training for Contractor Representatives may be found at this site.

(e) Within 60 days after the end of a performance period, the Contracting Officer will complete an interim or final past performance evaluation and the report will be accessible at www.cpars.gov

Contractor Representatives may then provide comments in response to the evaluation, or return the evaluation without comment. Comments are limited to the space provided in Block 22. Your comments should focus on objective facts in the Assessing Official’s narrative and should provide your views on the causes and ramifications of the assessed performance. In addition to the ratings and supporting narratives, blocks 1-17 should be reviewed for accuracy, as these include key fields that will be used by the Government to identify your firm in future source selection evaluations. If you elect not to provide comments, please acknowledge receipt of the evaluation by indicating, “No Comment” in Block 22, and then signing and dating Block 23 of the form. Without a statement in Block 22, you will be unable to sign and submit the evaluation back to the Government. If you do not sign and submit the CPAR within

30 days, it will automatically be returned to the Government and will be annotated: “The report was delivered/received by the contractor on (date). The contractor neither signed nor offered comment in response to this assessment.” Your response is due within 30 calendar days after receipt of the CPAR.

(f) The following guidelines apply concerning your use of the past performance evaluation:

(1) Protect the evaluation as “source selection information.” After review, transit the evaluation by completing and submitting the form through CPARS. If for some reason you are unable to view and/or submit the form through CPARS, contact the Contracting Officer for instructions.

(2) Strictly control access to the evaluation within your organization. Ensure the evaluation is never released to persons or entities outside of your control.

(3) Prohibit the use of or reference to evaluation data for advertising, promotional material, pre-award surveys, responsibility determination, production readiness reviews, or other similar purposes.

If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers.

(b) General Liability: $500,000.00 per occurrences.

(c) Automobile liability: $200,000.00 per person; $500,000.00 per occurrence and $20,000.00 property damage.

C.3 52.216-21 REQUIREMENTS (OCT 1995)

(a) This is a requirements contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies or services specified in the Schedule are estimates only and are not purchased by this contract. Except as this contract may otherwise provide, if the Government's requirements do not result in orders in the quantities described as "estimated" or "maximum" in the Schedule, that fact shall not constitute the basis for an equitable price adjustment.

(b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. Subject to any limitations in the Order Limitations clause or elsewhere in this contract, the Contractor shall furnish to the Government all supplies or services specified in the Schedule and called for by orders issued in accordance with the Ordering clause. The

Government may issue orders requiring delivery to multiple destinations or performance at multiple locations.

(c) Except as this contract otherwise provides, the Government shall order from the Contractor all the supplies or services specified in the Schedule that are required to be purchased by the

Government activity or activities specified in the Schedule.

(d) The Government is not required to purchase from the Contractor requirements in excess of any limit on total orders under this contract.

(e) If the Government urgently requires delivery of any quantity of an item before the earliest date that delivery may be specified under this contract, and if the Contractor will not accept an order providing for the accelerated delivery, the Government may acquire the urgently required goods or services from another source.

(f) Any order issued during the ordering period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's ordering period;

provided, that the Contractor shall not be required to make any deliveries under this contract after 09/30/2031.

(End of Clause)

These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 10 days of the end of the current period of performance.

(End of Clause)

C.5 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR

2000)

(a) The Government may extend the term of this contract by written notice to the Contractor within 10 days of the end of the current period of performance; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 10 days before the contract expires. The preliminary notice does not commit the Government to an extension.

(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.

(End of Clause)
