All Offerors shall be registered and current in the System for Award Management (SAM) database at time of offer submission and maintain active registration until contract award, completion of performance, and final payment.
System updates may lag policy updates. The System for Award Management (SAM) may continue to require entities to complete representations based on provisions that are not included in agency solicitations. Examples include 52.222-25, Affirmative Action Compliance, and paragraph (d) of 52.212-3, Offeror Representations and Certifications—Commercial Products and Commercial Services. Contracting officers will not consider these representations when making award decisions or enforce requirements. Entities are not required to, nor are they able to, update their entity registration to remove these representations in SAM.
Section D – Past Performance The Offeror shall provide three references: federal and private sector (if applicable). Identify all federal, state, and local government contracts as well as any private/commercial contracts of similar scope, size, complexity that are ongoing or have been completed within the last three years. List the following information for each contract:
· Company Name and Address
· Description of Services Performed
· Name, Telephone Number and Email Address of Responsible individuals who have first-hand knowledge of performance relative to similar type of services
· Dates of Contract Performance
· Contract Type (e.g. fixed-price, cost reimbursable) and total contract value
Failure to submit a complete proposal may result in a technically unacceptable determination.
2. Amendments:
Amendments to this solicitation will be posted on SAM.gov. Paper copies of the amendments will NOT be individually mailed. Offerors will be notified by SAM.gov of any new amendments that have been issued. No other notification of amendments will be provided. Potential Offerors are advised that they are responsible for obtaining and acknowledging any amendments to the solicitation. Failure to acknowledge an amendment may result in your quote being considered unacceptable.
3. AWARD BASIS
The Government intends to award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation, offers the lowest price, whose offered price does not exceed the amount of funding available for the procurement, and whose offered price is found to be reasonable. Past performance and Price are the evaluation factors.
Factor 1: Past Performance
PAST PERFORMANCE RATINGS
| Rating |
| Definition |
Acceptable Based on the offeror's performance record, the Government has reasonable expectation that the offeror will successfully perform the required effort.
Neutral The offeror did not submit relevant past performance or the information on past performance is not available or so sparse that no meaningful rating can be assigned.
Unacceptable Based on the offeror’s performance record, the Government does not have a reasonable expectation that the offeror will be able to successfully perform the required effort.
Past Performance will be evaluated to the extent of acceptable, neutral, or unacceptable for the Government’s requirements as outlined in the statement of work of the solicitation and based on the information requested in the instructions to the offeror’s section of the solicitation.
Factor 2: Price
Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement.
