B.2 STATEMENT OF WORK
Title of project Merit Review “Functional Outcomes of Cannabis Use (FOCUS) in Veterans with Posttraumatic Stress Disorder” Authority for the project and Purpose of the project Veterans Affairs has an ongoing merit review to study cannabis use in veterans. Posttraumatic stress disorder (PTSD) is a disabling psychiatric disorder that affects 20%-30% of U.S. Veterans. PTSD is strongly associated with increased risk for substance abuse comorbidity, including cannabis use disorder; however, multiple states now include PTSD as a condition for which patients can be legally prescribed medical marijuana, despite the fact that there has not been a single large-scale randomized clinical trial demonstrating the efficacy of cannabis to treat PTSD to date. The overall objective of the current merit review is to prospectively study the impact of reduced cannabis use on psychosocial functioning among Veterans with PTSD. We will first use ecological momentary assessment (EMA) methods to evaluate the relationship between cannabis use and daily functioning among Veterans with PTSD. Next, we will use mobile contingency management (CM) and EMA to assess the impact of reduced cannabis use on daily functioning among Veterans with PTSD who are heavy cannabis users. Our central hypothesis is that reductions in cannabis use will lead to positive changes in the functional outcomes of Veterans. The rationale for this research is that it will provide the first and only real-time data concerning the impact of reduced cannabis use on daily functioning among Veterans with PTSD. As a result, this innovative and timely project has the potential to significantly advance VHA healthcare and will directly inform the ongoing national debate concerning the impact of cannabis use on the long-term functional recovery of Veterans with PTSD Scope and methodology Lab testing to be performed at screening, mid-study and at follow-up. Changes across the study will be examined to understand fluctuations in bio-verified cannabis use in research study participants.
Tasks and Associated Deliverables Study participants have urine and saliva samples collected at three different time points during their study participation over an 8-week period. Urine and saliva samples will be collected throughout the year and picked up within 48 hours, but ideally same day as sample collection, at 3022 Croasdaile Drive Durham, NC 27705. Tests can be picked up between 8:00 AM-5:00 PM Monday-Friday. Tests to be run, and outcome delivered to study coordinator within one month of sample submission. Invoicing to be submitted monthly. Tests required are described below:
Participant urine needs to be tested for Cannabinoid: Creatinine Ratio. This test measures cannabinoid concentration normalized using creatinine to correct for diurnal fluctuations in fluid intake and urine output. This test generates three results: Cannabinoids Quantity (reported in ng/mL), Creatinine (reported in mg/mL), and the Cannabinoid/Creatinine ratio (reported in ng/mg). This test uses Immunoassay methodology.
Participant urine will also need to be tested should initial results be ambiguous. For example, if a study participant who reports never using cannabis, but who’s in-lab urine screening yields positive results for THC, it’s necessary to know if our in-lab urine screening provided a false positive. In these rare cases, a Cannabinoid (THC) Confirmation test is needed as a follow up measure. This test utilizes Mass spectrometry (MS) methodology to confirm the presence or absence of cannabinoids.
Participant saliva needs to be tested for Marijuana (THC), Oral Fluid w/Confirmation Reflex as an additional oral fluid test to complement other oral fluid testing our study is administering. Importantly, oral fluid samples are more sensitive than urine samples. Initial screening is performed by immunoassay (reported as positive or negative). To follow, an additional mass spectrometry analysis is provided on any samples presumed positive for THC (reported in ng/mL).
Reporting requirements Tests to be run, and outcome delivered to study coordinator within one month of sample submission. Invoicing to be submitted monthly. Results to be reviewed by study staff within 2 weeks.
