?.## PERFORMANCE WORK STATEMENT
1. Bi-Annual Preventive Equipment Maintenance Service
Provide preventive maintenance service on all centrifuges and other included instruments bi-annually on the government –owned equipment for the Geriatric Research, Education, and Clinical Center (GRECC), located at 508 Fulton Street, Durham, NC 27705.
2. Performance Objectives:
a. Preventive Maintenance will be performed on all centrifuges and other included instruments bi-annually in accordance with the manufactures recommended maintenance manual. This service will include cleaning instruments, calibrating them, and performing test runs.
b. The covered instruments will be repaired for any malfunctions on an as needed basis. This will include the cost of any parts and labor needed to complete the service. Contractor will be contacted by GRECC lab personnel when repaired is required.
c. The contractor shall provide all personnel, equipment, manuals, tools, schematics to perform contracted services.
d. The contractor shall provide preventive maintenance plan outlining all services for each type of equipment and the title of the individual(s) who shall perform services.
e. The contractor shall provide on-site records of all preventative maintenance conducted by the contractor and corrective action (if any) taken.
f. The contractor shall give one (1) calendar weeks’ notice prior to arriving on site to perform scheduled preventive maintenance. Upon arrival at the station to perform maintenance service or in answer to telephone request by authorized employee of the Department of Veteran Affairs, the contractor shall check in with engineering service prior to performing services during normal working hours, 8:00am to 4:30pm, Monday through Friday.
g. After completion service or maintenance work during the regular working hours, the contractor shall obtain the signature of the Contracting Officer’s Technical Representative (COR) or other authorized person in the using service acknowledging the receipt of service. A copy of the signed service or repair tickets shall be provided to the engineering service or the GRECC representative.
h. Contractor must have a Field Service Engineer (FSE) on site within 24 hours of receipt of call for emergency service if the service call is initiated during normal business hours of 8:00am to 4:30pm, Monday through Friday, excluding federal holidays.
i. If an emergency call is issued other than normal working hours, the contractor representative will be required to check in operations, located at the boiler plant, building 7 on site, and will contact the appropriate engineering service representative.
3. Constraints
a. All work is to be performed by competent personnel, experienced, and qualified to work on the specific equipment listed on the schedule. All work performed shall be in accordance with standard manufacturing practices.
b. Contractor shall return calls for emergency service within two hours.
c. The contractor shall submit emergency repair proposals that specially identify all cost and cost components involved for the specific repair. The proposal shall reflect in detail any discounts offered by the contractor.
d. Any emergency repair costs shall be reviewed by the COR and/or designated engineering service representative. The COR shall request approval from the Contracting Officer (CO) prior to proceeding repairs. The cost of any repair made without advanced approval shall be denied. At night, weekends, and holidays, the contractor shall deliver service tickets to the technologist in charge of the equipment.
e. Contractor is to provide all transportation, lodging, labor, tools and materials necessary to accomplish the work outlined herein. No additional charges, except for those specifically outlined in the contract, shall be charged to the VA at any time.
f. Contractor shall wear a uniform that adequately identifies the company and employee in a professional manner, to all personal, patients, and visitors of the Medical Center.
4. Performance Standards Metrics:
Performance standards define desired services. The Government performs surveillance to determine if the contractor exceeds, meets, or does not meet these standards.

Government Responsibilities The Government will assure that the contractor has access to and make readily available documents in support of all tasks, including the preventive equipment maintenance documentation.
BACKGROUND INVESTIGATIONS
A full background check is not required for this position, however all contract employees must bring photo identification and advance notice from the requiring service to obtain a VA Badge in order to work on a VA Facility. Upon award, the CO shall provide the contractor with detailed instructions on fulfilling security requirements.
5. Vaccine Mandate
Please see section C below, FAR Clause 52.223-99.
6. Tort Claims
The Contractor employees are not covered by the Federal Tort Claims Act. When a contractor’s employee has been identified as a provider in a tort claim, the Contractor employee is responsible for notifying the contractor’s legal counsel and/or insurance carrier. Any settlement or judgement arising from a contractor employee’s action or non-action is the responsibility of the contractor and/or insurance carrier.
7. CONTRACT ADMINISTRATION
Notwithstanding the Contractor’s responsibility for total management during the performance of this contract, the administration of the contract will require maximum coordination between the Government and the Contractor.
The Contracting Officer is the only person authorized to approve changes or modify any of the requirements of this contract. The Contractor shall communicate with the Contracting Officer on all matters pertaining to contract administration. Only the Contracting Officer is authorized to make commitments or issue changes that shall affect price, quantity or quality of performance of contract.
The COR shall be responsible for the overall technical administration of this contract as outlined in the COR Delegation of Authority.

C.3 52.217-8 OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.

C.4 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 30 days before the contract expires. The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.
(c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed five (5) years.
(End of Clause) C.5 VAAR 852.219-74 LIMITATIONS ON SUBCONTRACTING—MONITORING AND COMPLIANCE (JUL 2018)
(a) This solicitation includes VAAR 852.219-10 VA Notice of Total Service-Disabled Veteran-Owned Small Business Set-Aside.
(b) Accordingly, any contract resulting from this solicitation is subject to the limitation on subcontracting requirements in 13 CFR 125.6, or the limitations on subcontracting requirements in the FAR clause, as applicable. The Contractor is advised that in performing contract administration functions, the Contracting Officer may use the services of a support contractor(s) retained by VA to assist in assessing the Contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to Contractor's offices where the Contractor's business records or other proprietary data are retained and to review such business records regarding the Contractor's compliance with this requirement.

ENSURING ADEQUATE COVID-19 SAFETY PROTOCOLS FOR FEDERAL CONTRACTORS (OCT 2021) (DEVIATION)
(a) Definition. As used in this clause -
United States or its outlying areas means—
(1) The fifty States;
(2) The District of Columbia;
(3) The commonwealths of Puerto Rico and the Northern Mariana Islands;
(4) The territories of American Samoa, Guam, and the United States Virgin Islands; and

(a) Pursuant to 38 U.S.C. 8127(k)(2), the offeror certifies that—
