Project #: 659-19-117
STATEMENT OF WORK
SCOPE OF WORK:
The Salisbury VAMC campus has multiple buildings connected by a ground floor tunnel/hallway system in varying stages of degradation. Sections are mismatched due to damage, past repairs and upgrades that have left uneven and worn floors and transitions, unsightly paint and marking schemes, and a general nondescript condition that cannot be corrected by routine maintenance. The intent of this comprehensive project is to replace the flooring and upgrade all finishes for a uniform, level, aesthetically pleasing and maintenance-friendly tunnel system for our patients and staff. The selected general contractor (GC) will provide all administration, supervision, labor, materials, tools and equipment, and perform all operations necessary to remove and replace the designated floors, transitions, handrails, wall guards, and cove base, and paint the hallways. Work will be performed according to project specifications and in accordance with current Salisbury VAMC construction policies, procedures and best practices.
1.0 ADMINISTRATIVE and COORDINATION:
Period of Performance. The contractor will have 240 days from the Notice to Proceed which includes the time required for all submittals, material procurement, planning and mobilization, construction activities, and inspections. Disapproved submittals are not reason for additional time or delay cost for the contract.
Smoke-free Campus. As of October 1, 2019, the VA will be implementing a new policy regarding smoking by our patients, visitors, volunteers, contractors, and vendors at our health care facilities. The policy applies to cigarettes, cigars, pipes, any other combustion of tobacco, including but not limited to electronic or e-cigarettes, vape pens or e-cigars. Therefore, W. G. Hefner (VAMC), Salisbury, NC will be a smoke-free campus and no smoking will be permitted by any contractor employees working on this contract on VA grounds.
POCs. The Contracting Officer’s Representative (COR) will be the primary Point of Contact for communication and coordination on site throughout the period of performance. The Contracting Officer (CO) retains authority for matters pertaining to the contract, as well as all financial authority on behalf of the Government.
Supervision by Contractor. The GC will provide a Site Superintendent as “Competent Person” who will be on-site whenever work is being performed. The Site Superintendent will be separate from the prime sub-contractor and employed by the general contractor. Responsibilities of the Site Superintendent include but are not limited to: a) Oversight of the project, b) Preliminary approval of all material submittals and change order requests, and c) Supervision of any subcontractors.
Contractor Badging. Badges must be worn by all contractors and subcontractors while on campus. The Salisbury VAMC requirements for access badges will be provided. Generally, the documentation for badges include two forms of ID, proof of OSHA construction safety certification and proof of negative TB testing and Flu vaccination. All workers must be legally authorized to work in the US.
Site Survey. The GC and CO/COR will conduct a Pre-Construction meeting and walk-through before mobilizing, to be coordinated with the COR.
Schedule. Daily schedule TBD, however it is anticipated that demolition, installation and painting will take place after-hours. Halls must be trafficable during regular hours. Weekend and holiday work must be coordinated through the COR.
Trafficability and Closures. All halls and tunnels must be fully trafficable for the day following the construction shift, to include operation of the TUG pharmaceutical distribution machine. The contractor must also incorporate an acceptable means of passage during construction shift that allows for movement of hospital beds and stretcher beds (with or without patients), food/meal carts, pallet jacks and any other flatbed rollers, and the like. Pedestrian access must be ADA compliant. Closures may be possible but will be strictly limited and require prior coordination.
Phased Construction Plan. The contractor is required to provide the Contracting Officer a phased construction plan to perform this contract. The GC will update this plan on a regular basis and submit to the Contracting Officer and COR.
Inspections, Reports and Progress Meetings. The COR will inspect the worksite daily. A schedule of progress meetings will be determined, however once weekly is anticipated. The GC will report construction progress daily to the COR in writing. The format will be discussed in the Pre-Construction meeting. The presence or absence of a Government inspector does not relieve the contractor from any contract requirements, nor is the inspector authorized to change any term or condition of the contract without written authorization from the Contracting Officer.
Removal of Unacceptable and Unauthorized Work. All work which does not conform to the requirements of the contract will be considered unacceptable. Unacceptable work, whether the result of poor workmanship, use of defective materials, damage through carelessness or any other cause, found to exist prior to the final acceptance of the work, will be remedied or removed immediately and replaced in an acceptable manner unless in the public interest the Government consents to accept the work with an appropriate adjustment in contract price.
Site Clean-Up and Restoration. The contractor shall ensure the work site and areas around the work site are free of any debris and materials upon completion of work each day, as well as at the completion of the job. Contractor shall remove and properly dispose of all materials and equipment at the completion of the job.
2.0 TECHNICAL REQUIREMENTS:
Location. Refer to contractor drawings. See below for scope purposes (green lines).
Flooring Material. Mohawk Group, Hot and Heavy C0059 or Patcraft Surface Tone I6464V. A brand of like kind and quality will be considered, subject to VA Interior Design approval.
Leveling. Given the hospital environment and ADA requirements, certain areas may require leveling to achieve an acceptable effect. Some areas may not be evident until removal of old floor covering. A leveling compound such as DAP (10L516) should be used for patching and/or leveling.
Priority of Effort and Dimensions. The contractor shall be responsible for taking and applying accurate field measurements for the purposes of their proposals. Notify the COR of deviations greater or less than 10% of values specified below.

CONTRACTOR PERFORMANCE ASSESSMENT REPORTING SYSTEM:
(a) FAR 42.1502 direct all Federal agencies to collect past performance information on contracts. The Department of Veterans Affairs has implemented use of the Contractor Performance Assessment Reporting System (CPARS) to comply with this regulation. One or more past performance evaluations will be conducted in order to record your contract performance as required by FAR 42.15
(b) The past performance evaluation process is a totally paperless process using CPARS. CPARS is a web-based system that allows for electronic processing of the performance evaluation report. Once the report is processed, it is available in the Past Performance Information Retrieval System (PPIRS) for Government use in evaluating past performance as part of a source selection action.
(c) Please furnish the Contracting Officer with the name, position title, phone number, and email address for each person designated to have access to your firm’s past performance evaluation(s) for the contract no later than 30 calendar days after award. Each person granted access will have the ability to provide comments in the Contractor portion of the report and state whether or not the Contractor agrees with the evaluation, before returning the report to the Assessing Official. The report information must be protected as source selection sensitive information not releasable to the public.
(d) When your Contractor Representative(s) (Past Performance Points of Contact) are registered in CPARS, they will receive an automatically-generated email with detailed login instructions. Further details, system requirements, and training information for CPARS are available at www.cpars.gov. The CPARS User Manual, registration for online training for Contractor Representatives may be found at this site.
(e) Within 60 calendar days after the end of a performance period, the Contracting Officer will complete an interim or final past performance evaluation and the report will be accessible at www.cpars.gov Contractor Representatives may then provide comments in response to the evaluation, or return the evaluation without comment. Comments are limited to the space provided in Block 22. Your comments should focus on objective facts in the Assessing Official’s narrative and should provide your views on the causes and ramifications of the assessed performance. In addition to the ratings and supporting narratives, blocks 1-17 should be reviewed for accuracy, as these include key fields that will be used by the Government to identify your firm in future source selection evaluations. If you elect not to provide comments, please acknowledge receipt of the evaluation by indicating, “No Comment” in Block 22, and then signing and dating Block 23 of the form. Without a statement in Block 22, you will be unable to sign and submit the evaluation back to the Government. If you do not sign and submit the CPARS within 30 days, it will automatically be returned to the Government and will be annotated: “The report was delivered/received by the contractor on (date). The contractor neither signed nor offered comment in response to this assessment.” Your response is due within 30 calendar days after receipt of the CPARS.
(f) The following guidelines apply concerning your use of the past performance evaluation:
| (1) Protect the evaluation as “source selection information.” After review, transit the evaluation by completing and submitting the form through CPARS. If for some reason you are unable to view and/or submit the form through CPARS, contact the Contracting Officer for instructions. |
| (2) Strictly control access to the evaluation within your organization. Ensure the evaluation is never released to persons or entities outside of your control. |
(3) Prohibit the use of or reference to evaluation data for advertising, promotional material, pre-award surveys, responsibility determination, production readiness reviews, or other similar purposes.
(g) If you wish to discuss a past performance evaluation, you should request a meeting in writing to the Contracting Officer no later than seven calendar days following your receipt of the evaluation. The meeting will be held in person or via telephone or other means during your 30-day review period.
(h) A copy of the completed past performance evaluation will be available in CPARS for your viewing and for Government use supporting source selection actions after it has been completed.
