B.4 STATEMENT OF WORK (SOW)
1.0 Scope & Objectives
1.1 Scope:
The work of chaplains intersects with mental health issues on multiple levels, yet chaplains are often not optimally integrated with the mental health care treatment system in VA. The nationwide VA Mental Health and Chaplaincy Office, which is supported out of the VA Office of Mental Health Services and Suicide Prevention (OMHSP) and administratively housed in the VISN 6 MIRECC, aims to more effectively integrate the work of chaplains with mental health care services in the VA via research, education, and clinical trainings. In collaboration with the VISN 6 MIRECC and the OMHSP, administrative services are required to support VA Mental Health and Chaplaincy in its efforts to provide chaplains, mental health care providers, and other affiliated care providers with trainings and educational support that are rooted in research-informed practices.
The integration of mental health services with other patient care services is a public health priority. Integrated mental health services provide patients with better access to a full spectrum of care, improve patient health outcomes, decrease the stigma attached to receiving mental health treatment, and embody scientific understandings of persons as biopsychosocial beings. In recent years, a foremost leader in implementing models of integrated mental health care has been the Veterans Administration (VA).
As a federal entity, VA is able to implement and sustain system level improvements to patient care without encountering many of the barriers that community-based health care systems face in adopting integrated care models. For instance, in the span of a few years the VA Primary Care-Mental Health Integration Initiative successfully enrolled over 500 integrated care programs from across the country. VA Mental Health and Chaplaincy has had similar success in reaching across the system – having within a few years extensively trained approximately 120 VA and DoD chaplains from across the country as part of a year-long sub-specialty Mental Health Integration for Chaplain Services (MHICS) training initiative. These integration initiatives are prime examples of VA leadership in the innovative provision of mental health services. Still, many Veterans with mental health needs do not seek or receive services. In addition, there is a growing recognition that families of Service members are at heightened risk for mental health problems and are in need of mental health attention. To effectively reach these Veterans and their families, a continued commitment to innovative efforts is needed.
The current project will build on multiple ongoing efforts to better integrate mental health care services with chaplaincy in the VA. This statement of work applies to educational endeavors that will be carried out in service of creating a more optimally integrated system of mental health and chaplain care.
1.2 Objectives:
This contract is requested to support the VA Mental Health and Chaplaincy Office. Specifically, the contractor shall perform administrative, logistical, and coordination services for the planning, development, and production of high quality educational offerings in service to the Mental Health and Chaplaincy Office. The support objectives are to:
1) Arrange for a highly qualified filming studio with relevant prior experience to film, edit, and produce professional quality videos to complement the existing MHICS video offerings, making use of existing content (e.g., introductory logos/music; discussion question format) so as to seamlessly integrate new videos into the overall MHICS training program.
2) Arrange for consultants to provide filmed educational content as well as input and feedback around issues relating to the integration of chaplains with mental health services in both VA and the broader communities in which Veterans, Service members, and their families receive care.
3) Coordinate travel arrangements for consultants (and film crew if necessary) as appropriate, and manage invoice processing and payment for consultants and related personnel issues.
4) Manage, plan, and support a full range of educational and quality improvement offerings in service to the objectives of the Mental Health and Chaplaincy Office.
1.3 Target Audience
The MHICS training seeks to improve integration and function of chaplains in VA and related domains (e.g., chaplains in DOD), as related to care provision in coordination with providers in mental health care, primary care, and faith communities. Ultimately, the initiative seeks to promote care that is consistent with the values and preferences of Veterans. While the present contract focuses heavily on chaplain education, the below schematic outlines our major target audiences and the aims that we have for each audience.
1.4 Features & Functionality
The contractor must have:
1) the select level of expertise and qualifications sufficient to facilitate mental health and chaplaincy educational content development, as well as to coordinate other quality improvement and training offerings;
2) demonstrated readiness, through past collaborations, to immediately execute the contract with minimal modifications;
3) demonstrated experience and expertise interacting with necessary subcontractors (e.g., filming studio, subject matter experts) in an efficient and expedient fashion to develop a coherent and coordinated final product for delivery to and use in VA;
4) demonstrated experience and expertise supporting chaplain collaborations with mental health specialists in the provision of mental health care;
5) demonstrated experience and expertise organizing and executing VA educational offerings, including virtual (e.g., online education, in conjunction with relevant partners) and onsite (e.g., conferences, workshops, meetings and travel);
6) quality control at all stages of contract execution, ensuring continuity and reliable quality of support;
2.0 Background
A promising new approach for addressing the mental health needs of Veterans and their families is to better integrate mental health and spiritual care services. The need for such integrative efforts is suggested by multiple strands of data, including findings that many Veterans suffer from profound guilt, loss of life meaning, and general moral injury because of their military service. As a result, there are many Veterans who prefer to seek help from clergy. It is our view that mental health and spirituality are interrelated aspects of overall health and are optimally attended to within a coordinated system of care. Because VA already employs thousands of mental health providers and chaplains, a coordinated system of mental health and spiritual care is achievable.
Although the inclusion of chaplain services in the VA can be traced back to 1945, chaplains have a history of ministry to military personnel that well predates the existence of VA. Throughout this long history, chaplains have provided a wide array of services, both in combat contexts and in modern health care environments like the VA. One service frequently offered by chaplains in health care environments is to care for the needs of patients with mental health problems. As new understandings about the relationship between spiritual and mental health emerge, the chaplain’s role in attending to mental health needs is being viewed as increasingly important, particularly in Veteran populations.
It is with an appreciation of chaplains’ roles as care providers for patients with mental health needs that we have developed the Mental Health Integration for Chaplain Services (MHICS) training program. The MHICS program is a one-year training that aims to better equip chaplains in the provision of care to Veterans and Service members with mental health problems. The MHICS training is comprised of three 12-week courses. Each course is comprised of five distance modules and one face-to-face training. Distance modules typically include three 50-60-minute video didactics, readings, and a regularly scheduled 90-minute small-group consultation call using video conferencing. Each distance module can be completed at any time during a two-week span, with modules taking an estimated time of 8-12 hours each to complete. Each course also includes completing a written assignment, which at the end of the three courses in the MHICS training will culminate in a unified final paper. While the majority of the MHICS training is accomplished through distance learning modalities, each of the three courses also includes a 2.5-day face-to-face training intensive.
The majority of the MHICS training is completed via distance education, with a number of assignments and activities integrated throughout to enhance learning and interactivity. Distance modules include elements that stress application of learning in both clinical and operational contexts. For example, modules include interactive video engagement questions, surveys, threaded discussions, and consultation calls to make the learning as experiential as possible. All participants present actual cases during their small-group consultation calls as part of learning how to incorporate different evidence-based modalities into their chaplain care – in particular, Acceptance and Commitment Training (ACT), Problem-Solving Training (PST), and Motivational Interviewing (MI).
MHICS presenters include top experts from the Department of Veterans Affairs, the Department of Defense, and academic settings. Presenters are featured in over 50 hours of existing video content that is currently used within the MHICS training. Additional content is needed to sufficiently enhance the MHICS training to be able to offer advanced training credentials and to update select content.
