About this file

This document is an Amendment to Solicitation 36C24527Q0788 issued by the Department of Veterans Affairs, Perry Point VA Medical Center Contracting Office, dated September 8, 2026, with Amendment Number 0001, extending the response deadline to September 21, 2026 at 10:00 AM EDT. The amendment includes consolidated vendor questions and official VA responses clarifying contract requirements and performance standards.

The solicitation concerns durable medical equipment (DME) delivery, setup, installation, repair, and storage services for the Washington DC VA Medical Center. Key clarifications include: facility location must be within 30 miles of the DC VAMC (county requirement removed); acceptable facility documentation includes signed letters of commitment, leases, floor plans, and specifications demonstrating 7,000 square feet of climate-controlled space with a 3,000-pound hydraulic dock or lift gate; setup/delivery is a fixed-price trip CLIN including routine assembly and installation, while separate hourly INSTALL CLINs apply only to out-of-scope installation work specifically authorized on purchase orders; cancellation CLINs are billable only when Government or Veteran cancels after dispatch but before contractor arrival; repair response requires 24-hour initial action with completion deadlines suspended pending VA approval or parts supply; contractor furnishes incidental repair parts under $50 while VA supplies major parts; loaner DME requests require PSAS notification and direction when clinically appropriate VA inventory is unavailable; Transition CLIN 00X0 remains unpriced and excluded from initial award evaluation; during the 60-day transition, incumbent contractors remain responsible until successor completes setup and VA confirms transition; quarterly reports are due by the 15th of the month following each fiscal quarter (January 15, April 15, July 15, October 15); weekly inventory reports satisfy monthly requirements when submitted with monthly packages; patient refusals during home visits require telephone notification before departing; incident reporting requires immediate notification followed by written reports within 24 hours; personnel documentation and pre-performance acknowledgment must be submitted and approved before any employee provides services; monthly invoices are due by the 10th working day following service completion with required supporting documentation; and The Joint Commission accreditation for home medical equipment/DME services is mandatory and acceptable accreditations from other organizations are not substitutes. The facility must be equipped with a qualifying hydraulic dock or lift gate, not satisfied by an onsite forklift, and third-party logistics providers may be used only if the offeror maintains exclusive operational control and performs all services with its own employees. Government will not provide detailed historical utilization data beyond the estimated quantities in the Price Schedule.

Solicitation 36C24527Q0788 – Consolidated Vendor Questions and VA Responses

1. Facility location and storefront requirement - PWS, Storage and Inventory Management, p.26; Instructions to Offerors, pp.75-76 The PWS states that the storage and repair facility shall be within the county of the individual VAMC or a border county and shall be a “storefront facility,” while the Instructions require a facility within 30 miles of the Washington VAMC. For this Washington DC requirement, please confirm the governing geographic standard and clarify what VA means by “storefront facility.”

VA Response (Question 1): Facility should be located within 30 miles of the DC VAMC.

Language regarding the county is hereby removed.

2. Facility proof and physical-specification evidence - PWS, Storage and Inventory Management, pp.25-27; Instructions to Offerors, pp.75-76 Please confirm what documentation VA expects with the quotation to demonstrate that the proposed facility meets the PWS physical requirements, including the minimum 7,000 square feet, climate control, hydraulic dock or lift gate rated at least 3,000 lb, wheelchair accessibility, and required operational areas. If a signed letter of commitment identifies the facility and these features, will that satisfy the quotation-stage documentation requirement, provided possession/control is established before contract start?

VA Response (Question 2):

A signed letter of commitment is an acceptable form of documentation to demonstrate the Offeror’s possession and/or control of a proposed storage facility before contract start, provided it identifies the specific facility and is signed by both parties, and demonstrates that the offeror will have possession or control before contract start lists as applicable.

However, a letter of commitment alone that only confirms facility availability or future possession/control does not demonstrate compliance with the PWS physical requirements. The Offeror must also provide sufficient documentation establishing that the proposed facility meets the applicable requirements, including the minimum 7,000 square feet of climate-controlled storage, hydraulic dock or lift gate rated at least 3,000 pounds, wheelchair accessibility, required functional areas, and other PWS storage and inventory-management requirements.

Supporting evidence may include a floor plan or layout, lease or property records showing square footage, building/HVAC specifications, dock or lift-gate manufacturer specifications or inspection records, and other non-photographic documentation. The Government will evaluate the documentation submitted with the quotation. Listed below are additional examples of acceptable documentation of the physical requirements of the proposed facility.

3. Setup/delivery versus hourly installation CLINs - Price Schedule, pp.6-19; PWS, Specific Tasks p.22 and Delivery/Removal p.29 The PWS states that the setup/delivery line item includes assembly, setup, installation, and adjustment of DME, while the Price Schedule contains separate hourly INSTALL CLINs for each zone. Please define the work that is separately billable under the hourly INSTALL CLIN and confirm whether a setup/delivery trip and hourly installation may both be billed for the same service event when the ordered work requires both.

VA Response (Question 3):

Below clarifies the difference between installation in regard to routine work vs out of scope installation:

CLIN Definitions—Setup/Delivery and Installation

1. Setup and Delivery (TRP): The setup/delivery CLIN is a fixed price per trip to one Veteran address. It includes all labor, supervision, transportation, mileage, scheduling, delivery or pickup, assembly, routine setup, routine installation, adjustment, removal of packaging, completion of delivery documentation, and required Veteran/caregiver education associated with the DME specified on the purchase order.

2. Installation (HR): The INSTALL CLIN is an hourly labor CLIN for separately authorized installation work that is outside the routine setup, routine installation, and adjustment included in the setup/delivery CLIN. The purchase order must specifically identify and authorize the distinct installation work to be performed and authorize the applicable INSTALL CLIN. The contractor shall bill only actual authorized installation labor, in hourly rate increments that supports the actual effort required, and shall maintain supporting service records.

3. No duplicate billing: The contractor shall not bill the INSTALL CLIN for any assembly, setup, installation, adjustment, delivery, pickup, travel, Veteran education, or documentation already included in the setup/delivery CLIN. A setup/delivery CLIN and an INSTALL CLIN may be billed for the same service event only where both CLINs are expressly authorized on the purchase order and the contractor documents that the hourly installation services were separate and non-duplicative.

4. Cancellation CLIN billing trigger - Price Schedule, pp.7-19; PWS, Delivery/Removal and Patient Refusal, pp.29 and 33 Please define the event(s) that are billable under the CANCELLATIONS CLINs and distinguish those events from a Veteran refusal after dispatch/arrival, an unsuccessful/no-show delivery attempt, a cancellation before dispatch, and a re-delivery requiring reauthorization.

VA Response (Question 4):

Cancellation CLIN—Definition and Billing

1. The CANCELLATIONS CLIN applies only when the Government, Veteran, or authorized caregiver cancels a scheduled delivery, pickup, or service after the contractor has dispatched personnel and/or a vehicle to perform the authorized service, but before the contractor arrives at the Veteran’s address, and the cancellation is through no fault of the contractor.

2. The contractor may bill one Cancellation CLIN only when the cancellation is documented in the service record and includes: the purchase order number;

Veteran identifier; scheduled service; scheduled date/time; time of dispatch;

time and source of cancellation; and the name of the contractor employee documenting the event.

3. A documented Veteran refusal occurring after the contractor has made the trip or arrived at the Veteran’s address shall not be billed under the CANCELLATIONS CLIN. It shall be compensated on the same basis as a completed setup/delivery trip, consistent with the PWS, provided the contractor meets all required refusal notification and documentation requirements.

4. A cancellation occurring before dispatch of personnel or a vehicle is not billable.

5. An unsuccessful delivery attempt or no-show by the Veteran is not billable under the CANCELLATIONS CLIN. The contractor shall comply with the PWS scheduling, reminder-call, address-verification, and COR-notification requirements. VA assumes no liability for additional trips required because the Veteran is not home when delivery is attempted.

6. A re-delivery following a refusal, cancellation, or unsuccessful attempt may occur only after Government reauthorization. The re-delivery shall be billed only under the specifically authorized CLIN(s) on the reissued purchase order.

The contractor shall not bill both a Cancellation CLIN and a setup/delivery trip CLIN for the same cancellation event.

7. The Cancellation CLIN includes all contractor costs associated with the cancelled dispatch, including labor, vehicle use, mileage, and administrative costs. No additional trip, travel, installation, setup, or other charge may be billed for the same cancellation event.

5. Repair response, authorization, and completion clocks - PWS, Storage/Repair Area p.26; Equipment Safety and Repairs pp.35-36 Please reconcile the repair timeframes and authorization steps. The PWS requires action within 24 hours of PSAS notification, states contractor should make every effort to return repaired DME within 48 hours, requires repair estimates in other circumstances, and states equipment should be ready for reissue within five calendar days unless parts require a mutually acceptable schedule. Please confirm which timeframe governs each repair scenario and whether the repair-completion clock is suspended while CONTRACTOR is awaiting VA-provided parts, a separate purchase order, or other required VA authorization.

VA Response (Question 5):

Contractor performance is not measured against a completion deadline while VA is withholding necessary approval or has not supplied required parts

While the VA retains firm requirements for initial 24-hour action, timely repair estimates, ongoing status communication, and provision of loaner equipment, a contractor can rely on pending approval from the VA to delay action that it could and should have taken independently.

6. Responsibility for repair parts - PWS, Storage/Repair Area p.26 and Repairs pp.35-36 Please clarify responsibility for repair parts. The PWS states that contractor is responsible for incidental parts but not major parts, states PSAS will supply parts after troubleshooting, and separately defines incidental repairs as those costing less than $50 with major parts requiring a separate purchase order. Please confirm which parts contractor is expected to furnish within its hourly rate and which parts VA will furnish or separately authorize.

Vendor Response (Question 6):

The contractor furnishes incidental repair parts costing less than $50 as part of its hourly repair rate, while VA/PSAS supplies or separately authorizes major parts and the contractor may not proceed with those repairs without separate Government authorization.

7. Loaner DME when a clinically appropriate VA loaner is unavailable - PWS, Repairs p.35 When DME must be repaired at contractor’s facility, the PWS requires CONTRACTOR to provide a loaner from DC VAMC inventory stored at contractor’s facility. If the VA inventory does not contain a clinically appropriate loaner for that Veteran, please confirm the required escalation and interim process. Should contractor notify PSAS and await direction, or may VA authorize an alternate rental, substitute, or other temporary solution?

VA Response (Question 7):

The contractor shall notify PSAS and await direction.

8. Initial quotation treatment of Transition CLIN 00X0 - Price Schedule p.20; Ordering Procedures p.38; Instructions to Offerors p.76 The Price Schedule includes Transition CLIN 00X0, while the solicitation states that transition will be separately assessed and proposed after a Government task-order RFP and NTE is established. Please confirm how offerors should complete CLIN 00X0 in the initial quotation and whether the transition CLIN is excluded from the evaluated total price until a transition task order is competed/negotiated after award.

VA Response (Question 8):

The solicitation indicates that Transition CLIN 00X0 is not priced at the initial quotation stage. Rather, if the VA determines transition services are required, the Government will issue a post-award task-order RFP, establish a not-to-exceed amount, and negotiate/award a firm-fixed-price task order. Therefore, CLIN 00X0 should be left unpriced as directed by VA, and it should be excluded from the evaluated total price for initial award.

9. Responsibility during the 60-day transition - PWS, Transition Period p.25 During the 60-day phase-in/phase-out, please confirm whether the incumbent remains responsible for routine service to each Veteran until the successor has completed setup and the Veteran appears on the successor’s daily transition list, after which the successor assumes responsibility and billing for that Veteran.

VA Response (Question 9):

The 60-day phase-in/phase-out period is intended to ensure continuity of DME services while Veterans are transferred from the incumbent contractor to the successor contractor.

The incumbent contractor shall remain responsible for routine services and support for each Veteran until the successor contractor has completed the Veteran’s required DME setup and the transition has been confirmed by VA. The successor contractor shall provide the COR with its daily list of Veterans for whom setup has been completed and shall notify the incumbent contractor of those Veterans, consistent with the PWS.

Upon VA confirmation of completed setup and transition, the successor contractor assumes responsibility for services for that Veteran and may bill in accordance with the authorized order and applicable CLINs. The successor contractor shall not receive payment for a Veteran until completed setup is confirmed. The initial month of service will be prorated, as applicable.

The incumbent and successor contractors shall coordinate throughout the transition period to prevent interruptions in service, duplicate services, or duplicate billing.

10. Quarterly reporting calendar - PWS, Quality Improvement pp.34-35; Attachment A, Report Summary pp.58-60 Please confirm the governing quarterly reporting due dates. The PWS lists certain quarterly report dates as February 10, May 10, and November 10 and separately lists QI metrics/sentinel-event logs as due 15 days post-quarter, while Attachment A states quarterly reports are due by the 15th of the month following the reporting quarter. Please provide the single reporting calendar contractor should follow, including the April-June quarter.

VA Response (Question 10):

VA confirms that quarterly reports shall be submitted to the COR no later than the 15th calendar day of the month following the applicable fiscal-year quarter. The governing quarterly reporting calendar is:

• October 1–December 31: due January 15

• January 1–March 31: due April 15

• April 1–June 30: due July 15

• July 1–September 30: due October 15

This schedule applies to all quarterly deliverables required by the PWS and Attachment A. The inconsistent dates in the PWS Quality Improvement narrative are superseded by this clarification

11. Weekly versus monthly inventory reports - PWS, Storage and Inventory Management p.27; Reporting Deliverables p.35; Attachment A p.59 The PWS requires an electronic inventory report every Friday, while Attachment A also identifies a monthly Inventory Report of Government Furnished Equipment. Please confirm whether the monthly inventory report is a separate deliverable or whether a compliant weekly inventory report may satisfy the monthly requirement when submitted with the applicable monthly package.

VA Response (Question 11):

A weekly inventory report may satisfy the monthly requirement when submitted with the applicable monthly package

12. Patient refusal notification timeframe and method - PWS, Contractor Rights and Responsibilities p.33; Attachment A pp.58-59 Please confirm the governing patient-refusal notification requirement. The PWS states that refusal specifics are reported to the COR/designee by telephone within four hours followed by the refusal form, while Attachment A states telephone notification within one hour (next available business day if after 5 PM) and notification during the home visit when applicable. Please confirm the required timeframe and approved follow-up method.

VA Response (Question 12):

VA confirms that the following requirement governs:

If a Veteran refuses DME or requests removal of DME during a home visit, the Contractor shall notify the COR or designee by telephone during the home visit, before departing the residence when practicable. For all other patient-refusal events, the Contractor shall notify the COR or designee by telephone within one hour of becoming aware of the refusal. If the refusal occurs after 5:00 PM and no after-hours Government contact is available, notification shall be made within one hour of the next available business day.

The Contractor shall submit the completed refusal form and written report to the COR or designee by a VA-approved secure electronic method no later than the next business day. The submission shall document the Veteran’s name or approved identifier, date and time, equipment/service involved, reason for refusal if provided, actions taken, and signature of the Veteran/caregiver when obtained. If the Veteran/caregiver refuses to sign, the Contractor shall document that fact.

This clarification supersedes conflicting refusal-notification language elsewhere in the solicitation.

13. Incident reporting: immediate notice and written report timing - PWS, Equipment Safety p.35 and Incident Reporting p.38; Attachment A p.58 Please confirm whether the intended incident-reporting sequence is immediate verbal/electronic notification to the COR (and manufacturer when applicable), followed by a written report within 24 hours. The solicitation uses both “immediately” and “within twenty-four hours.”

VA Response (Question 13):

The solicitation outlines is a two-step process: immediate notification of the incident to the COR/designee and manufacturer when applicable, followed by a written incident report within 24 hours. The current language uses “immediately” for notifying VA/manufacturer and “within 24 hours” for investigation and written findings.

14. Personnel-document submission and Government acknowledgment - PWS, Response Times / Personnel Qualifications p.23 The PWS states that personnel documentation is to be provided within five business days of a staff member providing services, but also states that no contractor personnel may provide services without Government acknowledgment of receipt of those documents.

Please confirm that the qualification/training/competency package must be submitted and acknowledged before the employee’s first contract service, and identify the Government point of contact/process for obtaining that acknowledgment.

VA Response (Question 14):

Personnel documentation submission has been reviewed and updated to reflect the following process:

Contractor Personnel Documentation and Pre-Performance Acknowledgment

1. Before any Contractor employee performs services under this contract, the Contractor shall submit a personnel qualification package to the COR and copying the Contracting Officer.

2. The employee shall not perform any contract service unless and until the Government acknowledges receipt of the package in writing. The Government may acknowledge receipt by secure email or other written method designated by the COR or Contracting Officer.

3. Government acknowledgment of receipt is administrative confirmation only and does not constitute Government approval, certification, supervision, or assumption of responsibility for the employee’s qualifications, conduct, or performance. COR or CO will provide approval or disapproval after review of the information received.

4. The Contractor shall provide, within 10 calendar days after award, a complete list of all employees proposed to perform contract work and copies of applicable training certificates. The Contractor shall submit the required personnel package for each new, reassigned, or replacement employee before that individual begins any contract service.

5. The Contractor shall promptly update the COR and Contracting Officer if an employee’s qualification, license, certification, training status, or eligibility changes or expires.

15. Monthly invoice and monthly report due dates - Contract Administration Data p.4;

PWS, Invoicing p.36; Attachment A pp.58-59 Please confirm the monthly billing/reporting timeline. Contract Administration Data states invoices are submitted 30 days in arrears after completion of services; the PWS requires monthly invoices by the 10th working day of the following month; and Attachment A states monthly reports are due by the 5th workday. Please confirm the invoice due date and which supporting reports must accompany the invoice versus be submitted separately by the 5th workday.

VA Response (Question 15):

VA confirms that invoices shall be submitted monthly, in arrears, no later than the 10th working day of the month following the month in which services were performed.

“In arrears” means that the Contractor may invoice only for completed services; it does not establish a separate 30-day invoice-submission deadline.

For each monthly invoice, the Contractor shall submit the required invoice support identified in the PWS, including the completed and appropriately signed invoice;

required Veteran Rights and Responsibilities documentation; individual beneficiary equipment/service detail; applicable cleaning documentation; signed delivery safety checklist; and a service report for each Veteran visited during the billing period.

The monthly inventory report of Government-furnished equipment shall be submitted separately to the COR no later than the fifth working day of the following month.

The PM and Repair Report shall be submitted separately by the 10th calendar day of the following month. Weekly inventory reports remain due by close of business each Friday. The Contractor shall comply with all applicable daily, quarterly, annual, and event-driven reporting requirements separately.

16. CMS compliance documentation with quotation - Instructions to Offerors pp.74-75 The Instructions state that, prior to award, the contractor must maintain Joint Commission accreditation and meet/exceed Centers for Medicare & Medicaid Services standards, with written documentation supporting the requirement. The detailed pass/fail documentation language addresses Joint Commission accreditation. Please clarify what, if any, separate documentation VA expects offerors to submit with the quotation to demonstrate CMS compliance.

VA Response (Question 16):

VA confirms that the quotation must include documentary evidence that the Offeror itself holds current, active, unexpired, and unrestricted Joint Commission accreditation applicable to home medical equipment/durable medical equipment services, as required by the solicitation.

The solicitation does not require offerors to submit a separate CMS certification or other CMS-specific document with the quotation. The Government will evaluate Joint Commission accreditation on a pass/fail basis as stated in the solicitation.

Offerors should address their ability to meet or exceed applicable CMS standards in the Technical Capability Volume, including applicable policies, procedures, personnel qualifications, quality control, patient records, infection control, equipment management, and patient education practices. The Government may request additional information or verify compliance before award or during contract performance, consistent with the solicitation and applicable law.

The Contractor remains responsible for meeting all applicable CMS requirements throughout contract performance, regardless of whether a separate CMS document is submitted with the quotation.

17. Hydraulic Dock or Lift Gate Requirement Page 25

The solicitation states that the “facility must be equipped with hydraulic dock or lift gate rated at >3000lbs”. Would an onsite forklift rated at or above the 3,000 lbs capacity satisfy this requirement?

VA Response (Question 17):

No. An onsite forklift does not by itself satisfy the requirement for a hydraulic dock or lift gate rated at least 3,000 pounds. The PWS requires the storage facility itself to be equipped with a qualifying dock or lift gate; a forklift is different material-handling equipment and does not necessarily provide safe truck-to-facility loading/unloading capability.

18. The solicitation requires offerors to have current accreditation by The Joint Commission (JC) for home medical equipment or durable medical equipment services. Will the Government accept accreditation from another CMS-approved DMEPOS accreditation organization, such as the Accreditation Commission for Health Care/Board of Certification (ABC/BOC), provided the accreditation meets or exceeds applicable CMS DMEPOS Quality Standards?

VA Response (Question 18):

No. The solicitation requires the offeror itself to hold active, unexpired, unrestricted accreditation by The Joint Commission for home medical equipment/durable medical equipment services, and it makes that document a quotation-stage pass/fail requirement. Accreditation from ACHC, ABC, BOC, or another CMS-approved DMEPOS accrediting organization is not identified as an acceptable substitute.

19. The solicitation requires the Offeror to demonstrate possession and/or control of a storage facility located within 30 miles of the Washington DC VAMC and provide a signed lease or letter of commitment as evidence. Can the Offeror satisfy this requirement through a third-party logistics/storage provider under a signed letter of commitment, provided the Offeror has contractual control of the required space and operations.

VA Response (Question 19):

An Offeror may use a third-party property owner or landlord solely to provide leased premises, provided the Offeror—not the property owner, landlord, or another third party—has exclusive operational control of the space used for the contract and performs all required services using its own employees. The Offeror remains fully responsible for custody, security, segregation, inventory control, access, compliance, and performance at the facility.

Any arrangement under which a third-party provider performs or shares responsibility for contract services is prohibited.

20. To assist Offerors in developing realistic staffing and pricing, will the Government provide actual historical utilization for the most recent 12 months, including the number of deliveries/setups, installation hours, repair hours, emergency visits, cancellations, and pickups by zone?

VA Response (Question 20):

The estimated quantities stated in the Price Schedule were developed using the Government’s average historical utilization and are provided for Offerors’ pricing and proposal-preparation purposes.

The quantities are estimates only and do not represent a guarantee of future ordering volume, staffing levels, service frequency, or distribution of work by zone.

The Government will not provide additional detailed historical utilization data beyond the estimated quantities included in the solicitation.
