About this file

This is an Industrial Waste Discharge Permit issued by the Huntington Sanitary Board to the Veterans Administration Medical Center in Huntington, West Virginia. The permit (No. 986002-M) authorizes the facility to discharge domestic and non-domestic wastewater from hospital activities into the Huntington Sanitary Board's collection system via an existing sanitary manhole. The permit is effective November 30, 2024, and expires November 30, 2027.

The permittee is classified as a non-categorical significant industrial user with discharge flows exceeding 25,000 gallons per day, with an average daily water consumption limit not to exceed 150,000 gallons per day. Authorized discharges include hospital laundry wastewater, non-contact cooling water, boiler blowdown, non-hazardous laboratory wastewater, and sanitary sewage, along with pretreated wastewater from dental, food preparation, and imaging areas. The permit establishes specific wastewater discharge limitations detailed in Table I, including parameters for total suspended solids, dissolved solids, ammonia-nitrogen, BOD-5day, mercury (0.0006 pounds/day and 0.0005 mg/L), zinc (0.8 pounds/day), pH (5.0-10.0 SU), flash point (>140°F), oil and grease (160 mg/L), and total toxic organics (2.13 mg/L). Prohibited discharges include hazardous wastes, flammable materials, toxic pollutants, radioactive wastes, extreme pH levels, petroleum oils, storm water, and various solid materials. The permittee must develop and maintain a Total Toxic Organic Solvent Management Plan, Slug Discharge Control Plan, and Waste Minimization Plan. Semi-annual self-monitoring reports with compliance certifications are required, with submission deadlines of June 30th and December 31st annually. The facility must notify the Board immediately of any accidental spills, upsets, or noncompliant discharges. Violation of permit terms may result in permit revocation and fines up to $25,000 per day. All records must be retained for a minimum of three years and made available for Board inspection.
