About this file
This is a Statement of Work (SOW) for medical transcription services required by the Veterans Affairs Medical Center in Washington, DC, specifically for anatomic pathology specimen transcription. The contractor must provide traditional and speech recognition transcription and correction editing services using VA-provided dictation equipment and existing telephone lines to access VHA's digital voice files. All transcription and verification services must be performed in the United States of America, including US territories, with no exceptions permitted. The contractor is prohibited from subcontracting the work or any portion thereof without prior written approval from the Contracting Officer.
Service requirements include a turnaround time of 8 to 12 hours from receipt of dictations, with all dictations completed by no later than 7:00 AM Monday through Friday, and corrected transcriptions provided within four hours of completion. The contractor must maintain a current Business Associate Agreement (BAA) compliant with HIPAA and HITECH regulations, and must comply with FISMA, the Privacy Act of 1974, and all VA/VHA security and privacy directives. Quality assurance standards mandate 98% accuracy based on error-to-lines ratio, with random monthly sampling of reports. Failure to meet the 98% accuracy rate for two consecutive months results in a 10% payment reduction until compliance is restored. Header errors must not exceed five per month, and uploading reports to wrong VHA facilities is considered a reportable HIPAA privacy/security violation. The contractor must secure and maintain necessary clearances for transcriptionists to access the VISTA system, establish One-VA VPN access, and return completed transcriptions either through direct entry into VISTA or upload to the system at the facility's discretion.
