The document confirms multiple solicitation updates, including: correction of Block 15 on the SF 1449 form to reflect the Charleston facility rather than Clarksburg; clarification that catchment area zip codes will reflect areas surrounding the Charleston CBOC instead of Kentucky; modification of the Patient Custody section (4.2.8) to allow "nurse or medical staff" instead of requiring a charge nurse; reduction of simultaneous transport requirements to four (4) Basic Life Support (BLS) requests and one (1) Advanced Life Support (ALS) request, with up to Tier III Critical Care Transport capability; confirmation of average daily transports of 12.5 per the Performance Work Statement; elimination of paper run sheet requirements in favor of electronic patient care records; revision of patient check-in procedures to direct presentation to VA clinic staff; addition of Charleston CBOC references throughout; adjustment of wait time CLINs to 15-minute increments; update of wage determination to Kanawha County, West Virginia prevailing wage rates; removal of duplicate wage determination pages and correction of contract number headers throughout the document; and removal of Federal Acquisition Regulation clause 52.212-5 in accordance with FAR Part 12 rewrite guidelines.

1. Will block 15 of the SF 1449 be changed to the Charleston facility instead of Clarksburg? Yes it will be updated.

1. Will the catchment area zip codes on pages 5 and 6 be changed from Kentucky to reflect catchment areas surrounding the Charleston CBOC? The Solicitation identifies all locations within the Huntington VAMC and its catchment areas. NOTE: For the Charleston location, the following areas are considered within the catchment area:. See below:

1. ”Will the 4.2.8 Patient Custody section be changed to reflect simply “nurse or medical staff” instead of “charge nurse” as the charge nurse may not be always available at the destination point? Please see the note below 4.2.8 that states, The charge nurse or designated responsible individual must oversee the change in custody.

1. Will the 4.4.2.10 be changed to accurately reflect the number of simultaneous transport requests for the Charleston CBOC clinic. They would require much less than stated.

The contractor shall have sufficient vehicles and personnel to provide services for up to four (4) simultaneous transportation requests for BLS transport and one (1) simultaneous transportation request for ALS. One of these five (5) simultaneous transports may require CCT capability up to Tier III. If HWW VAMC requires ambulance services exceeding four (4) BLS simultaneous transportation requests, or one (1) ALS simultaneous transportation requests the vendor will be offered to provide the services if they have sufficient vehicles/personnel available; but may decline these additional orders if sufficient vehicles/personnel are unavailable.

1. Will the 4.4.2.11 be changed to accurately reflect the typical range of transports out of the Charleston CBOC clinic which would be much less than stated. OUR DAILY AVG PER PWS INFORMATION IS 12.5

1. Will the 4.5.3 statement be changed to accurately reflect the use of modern electronic patient care records which can be requested from the VA at any time should a need arise. The crews will not have a paper copy of the run sheet at the time of transport. I have spoken with staff to confirm our current process for checking patients in for appointments. At this time, we do not receive any paper documentation from the vendor. The vendor brings the patient to the designated clinical area, has the Veteran check in directly with staff when possible, and when that is not feasible, the vendor staff relay the check‑in information to our staff. We can revise the statement accordingly to reflect this process.

Option:

When delivering a patient, ambulance personnel shall present patient to the appropriate VA clinic and healthcare staff for check in.
