This amendment provides for an extension of the bid due date. The bid due date has been extended to Septebmer 4, 2019 at 3:00 p.m. to allow for review of RFI responses. The public bid opening will occur at the Contracting Offices at 513 Rosebud Plaza, Clarksburg, WV 26301
All other terms and conditons remain unchanged.

36C24518B0008 Technical Questions Project Title: MODERNIZE SPECIALTY CLINICS
QUESTIONS AND CLARIFICATIONS:
1. QUESTION: How are we to remove trash and debris from the office spaces during construction; freight or passenger elevator.
VA RESPONSE: Elevator use is disallowed during normal hospital hours. See spec section 01 00 00 part 1.19.A. Except that South freight elevator can be used for debris removal and material delivery prior to 0730 and after 1700 daily, provided material is in a clean sealed container with an attached lid meant for this purpose. Debris removal and new material delivery via elevators shall not interfere with normal hospital operations. It is also possible to install a debris chute for phase one demolition.
2. QUESTION: Where can the contractor put a dumpster?
VA RESPONSE: See spec section 01 74 19 part 1.3.E. VA-COR will assign Contractor a debris staging area at vicinity of loading dock.
3. QUESTION: Where will the contractor staging/lay down area be located.
VA RESPONSE: See spec section 01 00 00 part 1.6. There is no space available on Clarksburg campus outside the construction site for a contractor laydown area.
4. QUESTION: Which wage scale should be used for this project?
VA RESPONSE: The wage scale for this project is included in the text of solicitation at the end of the document.
5. QUESTION: 01 00 00 1.34 Will use of submittal exchange be required? This is costly.
VA RESPONSE: The A&E offers Newforma platform at not cost and will provide if needed.
6. QUESTION: Can the SSHO and the superintendent be the same person?
VA RESPONSE: Yes. Specification section 01 35 26 part 1.7.D indicates, "The SSHO or an equally-qualified Designated Representative/alternate will maintain a presence on the site during construction operations in accordance with FAR Clause 52.236-6: Superintendence by the Contractor." Since FAR 52-236-6 describes the Contractor's "competent superintendent," it appears the Government allows the SSHO to serve collateral duty as superintendent, if approved by Contracting Officer.
7. QUESTION: The drawings show that new exterior metal furring receives +/- 8” of closed cell foam insulation. We want to verify that the intended thickness is wanted to be that thick as well as the void in the concrete decking above this assembly.
VA RESPONSE: Intended thickness is 8" +/- (fill wall cavity) and to fill void in concrete decking above.
8. QUESTION: Basement RCP, AE110, states by note 02.31 that existing ceilings in those areas are to be removed for new work above and then the existing ceilings are to be replaced. We need to verify that these ceilings are to be reused. If they are to be replaced, please provide a basis of design for what is currently installed. Also, the same note states that this work is to be completed after hours. Please verify that the remainder of the project can be performed during normal working hours.
VA RESPONSE: Ceiling removed for new work above is to be replaced with a new ceiling grid and tile to match existing hospital standards. Reinstall existing lights and ceiling fixtures in same locations.
9. QUESTION: The finish legend shows (2) types of Armstrong Woodworks ceilings, which we are assuming the WD-01 would represent what the 1st floor RCP shows as WDGRL-01 below AT-01 in Check-In 1148. Please confirm that this is correct and the WD-02 is not used. Also, we have not found a specification on this product in the specification book. Can you provide a specification, part #, or any additional information what the owner desires for this product?
VA RESPONSE: WD-01 is Armstrong Woodworks Grille #7095, 4 slat, Backer only, Color shall be Walnut.
10. QUESTION: The finish schedule calls for Reception 1149 to receive the wood grille below AT-01. However, the 1st floor RCP does not show this. Please verify the acoustical ceiling finish in this room.
VA RESPONSE: Reception 1149 will not receive wood grille below AT-01.
11. QUESTION: The drawings show a "posture mirror" in the exam rooms (Item A1080). What are the exact dimensions on this and the basis of design?
VA RESPONSE: Bailey Manuf. Model 702, Adult Posture Mirror, 28" x 72"
12. QUESTION: Drawing EY101, Sheet Keynote 27.02, identifies six strand multimode fiber and Sheet Keynote 27.03 identifies 25 pair category 6 copper cabling. Drawing EP503 IT Riser Diagram identifies twelve strand multimode fiber and 50 pair copper. Please clarify.
VA RESPONSE: IT Riser Diagram A1/EP503 shows the correct requirements and supersedes the key notes and other references.
13. QUESTION: The corner guard description does not match the detail. Please advise.
VA RESPONSE: FSN-20N, 933 Mission White, Suede Texture
14. QUESTION: The crash rail does not match the detail. Please advise.
VA RESPONSE: SCR-48N, 933 Mission White, Suede Texture
15. QUESTION: There is no model for the handrail. Please advise.

16. QUESTION: It cannot be determined which locations are to receive solid standard sheet and which locations are to receive AbD. The finish plan uses wall protection types to annotate the floor plan but they do not specify which type sheet is to go where. Please advise
VA RESPONSE: All locations are to received solid standard sheet.
17. QUESTION: Corner guard description (Clear) does not match the detail showing flush mounted vinyl corner guards. Can you verify the corner guard product that you need with an Acrovyn model number?
VA RESPONSE: FSN-20N, 933 Mission White, Suede Texture
18. QUESTION: Acrovyn Crash Rail description 6” high wall guard does not match the detail. Can you verify the crash rail product that you need with an Acrovyn model number?
VA RESPONSE: SCR-48N, 933 Mission White, Suede Texture
19. QUESTION: There is no handrail model listed. Can you specify what Acrovyn Handrail model you need for this project?
VA RESPONSE: HRB-20N, 933 Mission White, Suede Texture
20. QUESTION: From the plans we cannot determine which locations are to receive solid standard sheet and which locations are to receive AbD . The finish plan uses wall protection. Can you clarify on the finish plan what type sheet is supposed to go in which location?
VA RESPONSE: All locations are to received solid standard sheet.
21. QUESTION: Is there an existing roof warranty that must be maintained when the AHU is flashed in? If so, who is the manufacturer?
VA RESPONSE: There are no existing roof warranties for this affected roof.
22. QUESTION: Can you please confirm the 10-hour OSHA is all that is required for non-competent persons?
VA RESPONSE: Confirmed. 10-hour OSHA card is required for general construction personnel.
23. QUESTION: SECTION 02 82 11 TRADITIONAL ASBESTOS ABATEMENT
1.2 VARIATIONS IN QUANTITY
A. The quantities and locations of ACM as indicated in the assessment report and the extent of work included in this section are estimated which are limited by the physical constraints imposed by occupancy of the buildings and accessibility to ACM. Accordingly, minor variations (+/- 5%) in quantities of ACM within the regulated area are considered as having no impact on contract price and time requirements of this contract. Where additional work is required beyond the above variation, the contractor shall provide unit prices for newly discovered ACM and those prices shall be used for additional work required under the contractor. Can you please confirm the contractor is to assume a 5% variance without cost impact?
VA RESPONSE: Per spec section 02 82 11, "...minor variations (+/- 5%) in quantities of ACM within the regulated area are considered as having no impact on contract price and time requirements of this contract." Therefore, to respond to the question, Bidders can assume a 5% variance without cost impact.
24. QUESTION: Can you please send out the drawings in a PDF format and not scanned?
VA RESPONSE: HDG, the architect of record has already provided the drawings in PDF format to the VA.
25. QUESTION: Can you please send out the site visit roster of those who attended it?
VA RESPONSE: The record of attendance for the first site visit was included in Amendment 1 and the second site visit was included in Amendment 3.
26. QUESTION: Will approved equal substitutes be accepted for the Nurse-call portion of this project?
VA RESPONSE: Approved equals will be considered with bids, provided equals are submitted in accordance with solicitation requirements. "Or equal" products proposed after bid opening will not be considered. Following FAR and VAAR requirements, Bidders are reminded that their "or equal" descriptive material must explain, in sufficient detail, how proposed equal products match salient characteristics of Brand Name products.
27. QUESTION: For section 117300, would a bariatric gantry system with a 1000 pound weight limit be accepted for this project?
VA RESPONSE: No, a 1000 pound weight limit is not acceptable.
28. QUESTION: Please advise what the existing electrical panels we have to provide circuit breakers for are? (Manufacturer, Model, etc.)
VA RESPONSE: This information should be obtained through the contractors site visit.
29. QUESTION: On drawing AE403 detail D4 phase 4 it shows a barn door and if you look at the elevation it refers to AE601/C5 it gives the detail of a barn door. Please confirm if there is any barn doors on this project and if so can we get some spec's.

30. QUESTION: Also, please confirm if there is any integrated door assembly on this project. I am not finding anything on the door schedule, but it is in the spec book.
VA RESPONSE: Yes, see door schedule.
31. QUESTION: What is the Required wage scale?
VA RESPONSE: The wage scale for this project is included in the text of solicitation at the end of the document.
32. QUESTION: Drawings are calling out blinds. Are spec’s going to be provided for blinds?
VA RESPONSE: The blinds will be inside of the windows. See spec section included with response.
33. QUESTION: The word solid at the end implies the use of a solid surface material rather than the laminate for the work surface. If this is true, the solid surface work surfaces come with a backsplash which would make the surface clamp monitor arm no longer compatible. Should I change the work surface to laminate and keep the clamp mount monitor arm or keep the solid surface work surface and change to a grommet mount monitor arm?
The typical created by Herman Miller for this project included a task light mounted below the flipper. It is not shown in the bid documents. Should this be added?
VA RESPONSE: Keep the work surface solid surface and change to a grommet mount monitor arm. No, do not add the task light below the flipper.
34. QUESTION: Should the L shape offices have task lights?
VA RESPONSE: No, the L shape offices will not have task lights.
35. QUESTION: EG001 calls for A02 lights to be 2x2; however, AE110 and EL100 show these lights to be 2x4. Please confirm the size of these lights.
VA RESPONSE: The light fixtures are to be 2x4.
