SCOPE OF WORK

For Louis A. Johnson VA Medical Center

Roof Replacement above Operating Suites and above ICU Project # 540-18-25S

-- Project Goals --

Project Goals: Acquisition construction services that will:

• Replace above referenced roofing systems, relocate and amend lightning protection systems and install metal coping in accordance with the attached drawings and specifications.

-- Project Scope --

GENERAL:

This project is for the acquisition of construction services (which shall include all labor, materials, equipment and sundries) to replace two roof systems totaling approximately 18,230 sf. The first roof is located above the Operating Room (OR) suites and the second is located above the Intensive Care Unit (ICU) at the LAJVAMC and as shown in the attached drawings and specifications.

• Remove the existing EPDM roof membrane to the concrete deck (note all EPDM curb flashings are to remain in place)

• Prime the concrete deck(s) as per the Manufacturer specification

• Install a tapered insulation system that promote positive drainage and impact resistance

(insulation requirements can be found in the specification)

• Insulation base layer will be adhered in a asphaltic urethane insulation adhesive, the taper insulation and recovery sub-straight will be adhered in Low-Rise Foam in accordance to the manufacturer requirement.

• Apply the Bio Adhesive to the recovery sub-straight at a rate of 2gal per square

• Embed the SBS Base Sheet in the BIO Adhesive with a minimum 1/8” bleed out using a 75#

Roller

• Apply the Bio Adhesive to the Base Ply at a rate of 2gal per square

• Embed the SBS Cap Sheet in the BIO Adhesive with a minimum 1/8” bleed out using a 75# Roller

• Using the Specified Liquid membrane, Tape and install at perimeter flashings, pipe penetrations, and curbs.

• Pressure wash the 3rd floor perimeter coping stone using a bio-degradable detergent at a maximum rate of 2500psi

• Using the Specified Liquid membrane, install according to the manufacturer requirements.

• Install Perimeter Metal Coping to meet the specifications on the 4th floor to a color determined by the VAMC

• In the event the above listed items of work contradict manufactures specifications or best practices the Owner shall be made aware of the situation immediately.

All removed items and construction waste shall be removed and disposed of in accordance with Local, State and Federal laws. Lightning protection will be replaced in accordance with governing standards. During construction it is required that the roofing contractor and related sub-contractors will coordinate their activities with other contractors working in or near the project site. At all times during the project the Contractor must prevent water entrusion into the facilities below.

SPECIAL NOTE: The southern portion of the ICU roof, adjacent to the air intake, must be installed during off peak hours as to not impact the Operating Room Suites. Off peak means after 5 pm during weekdays.

SPECIFICS:

Key Milestones:

• Perform thorough site survey prior to bid verifying quantities; submit necessary RFI’s as necessary prior to bid day.

• Contract Award and Notice to Proceed.

• Deliver submittals for review and approval. This includes but is not limited to Material Safety

Data Sheets.

• Perform demolition and construction based on project schedule or accelerated if time permits.

No additional monies will be paid for accelerated work.

• Conduct training and submit O&M’s, completed as-builts and final deliverables including warranty documentation. The warranty shall be for 20 years and require biennial inspection from the warranty provider. No additional costs shall be incurred by the owner for roofing material failures within the warranty period unless damages were caused by VA personnel or their representatives. Contractor shall respond to issues/leaks related to the roofing systems within one hour during normal business hours (M-F 0800-1700) and within two hours during all other times.

-- Site Investigation --

REQUIREMENTS: Conduct a site investigation of the facility.

Investigate and review the project requirements:

• Perspective contractors shall perform a thorough site investigation prior to bid. Actual dimensions of the work shall be determined by the Contractor and any information provided by the VA is for information only. This will be accomplished by scheduling an appointment with the CO/COR, to conduct the site investigation at least two weeks prior to the bid day.

All contractor questions shall be submitted in writing to the CO/COR so that addenda can be issued.

• Contractors shall submit a preliminary project timeline schedule highlighting major phases with their bid documents.

• Within five days after NTP, the contractor shall perform a secondary site walkthrough with participating Clarksburg Maintenance Staff, Infection Control, Fire Safety Officer, Safety Manager, Contracting officer, and COR to review project planning, ICRA, and project safety.

The contractor shall finalize the schedule for approval.

-- Program Development --

EXPECTATION: Develop the project program and maintain a schedule.

Develop the project program and Maintain a schedule: Project Calendar Days = 120

The contractor shall submit a schedule not to exceed the total number of performance days identified in the contract. Particular attention should be the preparation and submission of a cost loaded project schedule and project submittals. All submittals should be submitted within 5 days of NTP.

Execution: Execution of work shall not begin until submittals have been approved.

• Drawings and Specifications:

1. Performance is based on the attached drawings and specifications.

2. All local, state and federal codes apply.

• Project Schedule:

The goal of the project schedule is to monitor performance. The contractor shall submit a preliminary schedule highlighting major milestones. A more detailed draft schedule shall be submitted at the NTP meeting. The goal is to have a finalized schedule within 5 days of NTP. Contractors are expected to conduct a thorough field basement as well as submission of any necessary RFI’s related to design documents and/or field conditions prior to bid.

• Safety:

Special emphasis shall be placed on the contractor’s submission of a safety and ICRA plan that includes a fire prevention program for hot work, infection control measures and lift plan for any crane activities. Crane activities must be planned and approved (pick plan) well in advance.

Contractor shall abide by all OSHA and safety starndard for the duration of the project.

-- Execution of Work --

Expectations: Execute the work.

Provide services:

• The vendor will begin execution of work upon approval of submittals.

• The contractors work shall be expected to follow a project schedule as noted above. All progress payments shall be based on inspections of completed work. Payments requested for work that is not complete or does not meet design requirements will be rejected.

• The Contractor shall ensure no water intrusion into the spaces below over the course of the project.

• Additional deliverables:

1. The roofing supervisor or superintendent shall have completed the OSHA 30 hour course and the onsite contractor’s staff shall have completed the OSHA 10 hour course.

2. The contractor shall complete Coordination drawings in AutoCAD format and PDF showing all proposed layout prior to performing work. The VA shall have 7 days to review coordination drawings.

3. All components used in this project shall meet specifications and code. No substitutions will be permitted without Owner approval.

4. The contractor shall provide completed as-built drawings in AutoCAD format and PDF as well as two full size printed sets. All prints must be developed by a qualified draftsperson and final as-builts shall be submitted to COR prior to final invoice.

5. The contractor shall provide O&M manuals that include warranties for all roofing components or systems.

6. The contractor shall establish a training plan for VA staff to properly maintain the new roofing systems.

7. A waste materials report in terms of lbs or tons recycled or lbs or tons waste shall be submitted to the COR with each application for payment. Failure to supply the waste report will result in rejected application for payment.
