About this file

This document is a Quality Assurance Surveillance Plan (QASP) for Snow Removal and Deicing Services at the Wilmington VA Medical Center in Wilmington, Delaware. The QASP establishes the framework for monitoring contractor performance in accordance with the Performance Work Statement (PWS) and defines performance objectives, acceptable quality levels (AQLs), surveillance methods, documentation requirements, and corrective action procedures.

The QASP specifies five core performance areas with corresponding metrics and compliance standards. Timeliness of Response requires 1-hour mobilization and 15-minute emergency callbacks with zero emergency deviations and maximum one non-emergency deviation per month, monitored through observation and call logs. Snow and Ice Removal Quality requires safe conditions maintained with clearing by 6:00 AM, targeting 95% overall compliance and 100% in critical areas through daily inspections. Critical Area Maintenance prioritizes ADA-accessible routes, emergency access, and fire hydrant clearing with 100% compliance verified through real-time inspections. Garage Compliance requires use of approved chemicals and prohibits metal blades, with 100% compliance verified through equipment and garage inspections. Documentation Accuracy demands error-free Snow Control Sheets and National Weather Service (NWS) data for Mount Holly, New Jersey, with 100% compliance verified during invoice review. Government oversight is shared among the Contracting Officer (CO), Technical Point of Contact (TPOC), and VA Police/Engineering Service. The contractor must maintain an internal Quality Control Program including self-inspections, timely corrections, and operational readiness. Surveillance methods include direct observation, structured inspections, random spot checks, document reviews, and VA staff feedback. The TPOC must validate all Snow Control Sheets and NWS accumulation data before invoice approval. Deficiencies documented by the Contracting Officer Representative (COR) require immediate corrective action, with repeated or significant issues resulting in contract remedies or negative evaluations.
