About this file
This is Amendment 1 to a Request for Quotation (RFQ) issued by the Department of Veterans Affairs, Network Contracting Office 4, for Durable Medical Equipment (DME) services in VISN 4 West region. The RFQ number is 36C24427Q0004, issued August 7, 2026, with no extension to the original submission deadline of September 8, 2026 at 8:00 AM EST. This is a recompete of the existing contract 36C24421D0016 with VISN 4 Western Medical Centers, and the government expects to award a single contract.
The amendment provides clarifications on five key areas: (1) updated RFQ document with corrected RFQ numbers and emergency definitions aligned with the Performance Work Statement; (2) corrected Attachment 1 reflecting McKean County, Pennsylvania instead of "Kane"; (3) 29 vendor questions and answers addressing contract scope, pricing methodology, subcontracting, facility requirements, emergency service standards, and transition procedures; (4) updated CLINs excel file with accurate emergency definitions and period of performance dates; and (5) updated PWS sections addressing vendor pricing direction to originating facilities, Medicare and state licensing standards, emergency timeline measurement, laminated card provision, separate parts pricing, storage facility ownership/lease options, vendor training schedules, and QASP monitoring utilization. Key requirements include 24/7 emergency delivery and repair for life-sustaining equipment with four-hour completion standards, $500 preauthorization limit for emergency repairs (all-inclusive), 98% Acceptable Quality Level performance standards, and compliance with FAR 52.219-14 subcontracting limitations for small-business prime contractors.

Vendor questions and answers:
1. Please confirm that all references to Solicitation 36C24427Q0003 on pages 33-62 are typographical and that Solicitation 36C24427Q0004 governs. Please also confirm that the questions deadline is 4:00 p.m. Eastern Time on August 17, 2026 and the quotation deadline is 8:00 a.m. Eastern Time on September 8, 2026. The RFQ #36C24427Q0003 was a typo and has been updated to reflect correct RFQ # of #36C24427Q0004.
Question and submission deadlines are accurate and remain unchanged.
2. Is this a new or recompete contract? This is a recompete contract with VISN 4 Western Medical Centers (36C24421D0016).
3. For a Veteran residing in a county appearing in more than one major-site catchment, please confirm which facility SLIN governs pricing and invoicing, for example, the ordering/prescribing VA facility or another assignment rule. Please also clarify the intended geographic area identified as “Kane” under Pennsylvania counties in Attachment 1. The facility where OO submits the order is where the pricing and invoices would be directed. Attachment 1 has been updated to utilize McKean County where “Kane, PA” is located. Any reflective or updated wage determinations will be provided at contract award and in the meantime can be found on sam.gov.
4. Tier pricing for repairs, you have listed in the PWS the four tier that you want to designate for this contract, but the price contract schedule only has one line item? Were do we put the pricing for all four tiers? This is for labor only NOT parts too correct?
Tiers are listed in the PWS to define the amount of quantities to be invoiced per specific job. The total amount expected to be needed for all tiers, is included in the price schedule line. Vendors are to submit total pricing for the one line that will cover all tiers defined in PWS. These are labor only CLINs but not hourly billable.
5. Please confirm the intended repair pricing and invoicing method. Is the quoted facility-specific JOB unit price one billable job, with Tier 1 billed as 1 JOB, Tier 2 as 2 JOBs, Tier 3 as 3 JOBs, and Tier 4 as 4 JOBs after the Ordering Officer authorizes the tier?
Please also confirm that the pricing workbook’s reference to an "hourly rate" does not create a separate hourly billing basis. Yes, the job tiers define how are billable and after OO authorizes. This is not an hourly billable CLIN, Jobs are defined as tasks expected to be completed in that amount of time.
6. Will this solicitation be a win all bid or by site? Meaning if you only want to bid one site can you do that or must you bid all sites? The solicitations are posted for all medical center sites listed within the geographic regions in the attachments. It is expected this will be a single award contract.
7. Please clarify the anticipated award structure. It is expected this will be a single award contract.
8. 4.10.1 training report do you want training records included in the bid? Or just after award? If vendor has training records to be submitted for acceptance, these are to be provided to POC after contract award. Vendors are not required to have training records to provide with quote submission.
9. 4.20 currently does the VA still use PKI for outside vendors? There are times when VA will use PKI for security purposes, these capabilities are required for vendors.
10. May a small-business prime contractor utilize qualified small-business subcontractors or teaming partners to perform portions of the DME storage, delivery, installation, repair, cleaning/processing, and technical services, provided the arrangement complies with
FAR 52.219-14 and all other solicitation requirements? Yes, in accordance with
52.219-14 limitations on subcontractors
11. The solicitation requests two examples of similarly sized and scoped past performance.
Can relevant experience performed by a proposed subcontractor or teaming partner be considered in the Government's evaluation, or must both examples have been performed directly by the prime offeror? In accordance with the evaluation factors, previous work examples shall be work reflected of the contractor submitting the quote.
12. May an offeror use leased facilities and/or qualified subcontractor facilities as its proposed DME storage locations, or must the storage facilities be owned and directly operated by the prime contractor? The solicitation requires offerors to provide the addresses of proposed storage locations? As long as addresses can be provided at the time of quote and locations meet or exceed requirements outlined in the PWS, the storage location can be owned or leased.
13. May the prime contractor satisfy the 24-hour/7-day emergency delivery and repair requirements through qualified subcontractors or teaming partners, provided the prime remains responsible for contract performance? The solicitation requires 24/7 emergency delivery and emergency repair for life-sustaining equipment. Yes, in accordance with
52.219-14 limitations on subcontractors
14. Please confirm the required quotation package and file structure for this services RFQ, including whether the technical response is one PDF not exceeding 30 pages and whether the separate PDF described as containing the "technical specifications of the offered item" is required in addition to that technical response. These are the same thing. Instructions refer to the technical pdf described in technical section and then the completed CLIN excel file.
15. For purposes of the Veterans Involvement evaluation factor, please confirm the verification source and documentation VA will use to validate an offeror’s
SDVOSB/VOSB status at quotation submission and award, given the solicitation’s references to the VIP/VetBiz database. The government utilizes the SBA designated source to define status.
16. Please clarify which emergency definition governs classification, the four-hour performance standard, and use of the after-hours/emergency CLINs. The PWS defines an emergency as an event posing risk of serious property damage or health and safety hazards, while the pricing workbook describes emergency as "life sustaining." The statement in pricing excel workbook and CLINs in RFQ have been removed. Refer to PWS for emergency definition.
17. 4.2.8.5 laminated cards to be affixed to bed end for process on using emergency crank will the VA be providing these laminated cards? No, must be provided by the contractor
18. If VA adds a clinic, catchment area, or service location during performance, please clarify how the applicable facility SLIN, pricing, and Service Contract Labor Standards wage determination will be identified and incorporated when the added geography was not included in the original quoted service area. Wage Determinations are predetermined by county and wages set by the Department of Labor. If a clinic or catchment area are added during contract period of performance, it would require a bi-lateral contract modification. The changes and rates would be discussed and agreed upon prior to contract modification.
19. Please define the start point for the four-hour emergency completion clock and explain how timeliness is measured when required VA-supplied equipment is not yet available or when the Veteran/caregiver cannot accept service within four hours. Four-hour clock starts when the call is received. Documentation should be provided to the facility
POC when the delay is due to Veteran/caregiver request or if equipment is unavailable. (VA should not be requesting emergency delivery of items not routinely available in VA stock)
20. Please clarify whether the $500 emergency repair preauthorization limit is inclusive of the after-hours/emergency service charge, repair JOB units, parts/materials, and freight, or whether it applies to a specific component of the emergency repair. VA can only authorize a total of $500 without prior authorization. It would be an all-inclusive charge. Anything over $500 would require VA authorization the next business day.
21. 4.11 will VA now be picking up DME equipment if a veteran DC or expires? VA no longer picks up equipment.
22. To support a common transition and pricing basis, please provide, to the extent available, the current approximate Government-furnished property inventory and active beneficiary count by major site, including major equipment categories and primary makes/models, together with the complete incumbent storage pickup address(es). The
PWS currently lists only Malvern, Harrisburg, Bloomsburg, and Malvern by city and states that complete locations will be provided after award. Incumbent locations are listed in PWS section 4.7.1.11. The list of inventory and beneficiary count is not available at this time. This changes daily based on delivery requests and incoming product.
23. Are there specific DME supplier accreditations, Medicare/DMEPOS enrollment requirements, state licenses, or other certifications that the prime contractor must possess at the time quotations are submitted or prior to contract performance? Vendor must meet both federal Medicare standards and state licensure requirements
24. For services requiring a Certified Technician, including complex power wheelchair assembly/setup, are manufacturer-certified technicians acceptable, and may these technicians be employees of an approved subcontractor? The price schedule specifically identifies a Certified Technician for complex power chairs. ONLY if there is a sub-contractor on this contract, in accordance with 52.219-14 limitations on subcontracting. Technicians can be certified by the manufacturer however it should be noted certified technicians are only required under Tier 4 repairs.
25. Please clarify the relationship among the 15-day incumbent-inventory pickup requirement, the duplicate-cost provision after 30 calendar days, and the 90-calendar-day transition period, including what constitutes completion of "phase-in." Please also confirm that the successor Contractor will not be responsible for duplicate incumbent costs to the extent delay is caused by the incumbent, VA, unavailable prescriptions or
Government-furnished equipment, Veteran availability, or other circumstances outside the successor Contractor’s control. 15 day incumbent -inventory pick up means that the awardee has 15 calendar days to pick up and relocate all Government
Furnished Property stored at the incumbent’s storage facility and move it to the awardee’s storage facility. The 90 calendar day transition period is the ultimate drop-dead timeline to complete the entirety of the “phase-in”. Completion of
“phase-in” refers to the fully completed transition of inventory and patient records from the incumbent Contractor to awardee. 100% fully operational.
26. Please clarify whether any incumbent Contractor-owned DME is currently deployed to
Veterans and must be replaced by the successor Contractor, or whether the successor’s transition obligation is limited to transfer, storage, service, and management of VA-owned/Government-furnished equipment. If incumbent-owned equipment must be replaced, please provide the approximate quantities and equipment categories by major site. Current contracts do not include any contractor owned equipment. ALL equipment is purchased by the VA and delivered/serviced by the contractor
27. Please clarify how the 98% AQL will be calculated for each performance objective, including the measurement period and what constitutes an individual case or occurrence for the numerator and denominator. Please also clarify treatment of cases where timeliness depends on VA-supplied equipment, VA action, or Veteran availability. See
PRS in section 6 of the RFQ and incorporated into the corresponding QASP.
28. Please confirm the required patient-facing document/signature package for (a) delivery/set-up/installation, (b) repair-only, and (c) pickup-only encounters, including whether Attachment 5 Patient Satisfaction Survey is required for pickup-only service.
Attachment 2 includes a pickup service option, while PWS §4.2.9 lists the document package for delivery, installation, set-up, and repair orders. Attachments 2, 4 and 5 are required patient-facing documents. Attachment 5 is required for pick up only service.
29. Please clarify whether approved repair parts/materials and applicable freight or delivery charges are reimbursed separately from the repair JOB unit price and, if so, the required pricing basis and invoicing method. Please also clarify how the JOB charge is handled when VA elects to purchase required parts and have them delivered to the Contractor.
Parts are separately priced from CLINs with job tiers. Section 4.5.1.2 in PWS has been updated to reflect.
