About this file

This is a Quality Assurance Surveillance Plan (QASP) for a Wellness Services: Online Fitness contract that establishes systematic Government evaluation methods for contractor performance delivery of virtual wellness services.

The QASP defines ten key performance objectives with specific standards and Acceptable Quality Levels (AQLs): Platform Availability must achieve 99% monthly uptime; Class Delivery Requirements mandate minimum five live classes Monday-Friday and two on weekends in Eastern time between 6am-9pm with class sizes not exceeding 25 participants; Instructor Qualifications require 100% certification compliance verified within 10 business days of award and annually within five business days of updates; Class Quality & Interaction requires 95% compliance for two-way real-time engagement and safety monitoring; Security Compliance demands 100% encryption of user data with no Protected Health Information sharing; Anonymized Data Reporting requires 100% on-time monthly delivery of usage, newsletters, attendance, and engagement data; Veteran-Specific Content mandates 95% monthly availability of Veteran-only and Women-Veteran-only classes; Specialized Wellness Modalities (yoga, tai chi, HIIT) require 95% availability; SOP Compliance requires 100% adherence to safety, operations, and quality assurance procedures; and Customer Service Responsiveness requires 95% response rate to Government inquiries within three business days. The PPOC monitors performance through direct observation, system reports, random sampling, user feedback, and contractor documentation review. Noncompliance triggers corrective action plans, increased oversight, termination actions for severe violations, and negative CPARS evaluations. All surveillance findings, compliance checklists, deficiency notices, and corrective actions must be documented and retained in the contract file.
