About this file
This is a Past Performance Questionnaire form used to evaluate contractor performance on solicitation 36C24426Q0703 for the Butler Veterans Affairs Medical Center (VAMC) in Pennsylvania.
The form requests evaluators to assess a contractor's past or present performance across three primary categories: Quality of Services/Care, Qualified Personnel, and Business Relations. For Quality of Services/Care, evaluators must address the contractor's ability to provide therapeutic and rehabilitative services in a safe environment to homeless eligible Veterans, including meals, transportation, laundry facilities, gender-specific accommodations, and ADA accommodations. The Qualified Personnel section requires assessment of the contractor's ability to provide experienced managers and supervisors. The Business Relations section evaluates the contractor's willingness to correct noncompliance issues, address problems timely, display initiative and flexibility, and maintain effective working relationships with government personnel. Each of the three primary assessment elements uses a five-point adjectival rating scale: Excellent, Good, Satisfactory, Marginal, or Unsatisfactory, with detailed performance descriptions provided for each rating level. The form also requests identification of overall contractor strengths and weaknesses, whether the evaluator would re-award to the contractor, and an overall Past/Present Performance rating. Completed forms must be submitted to enrico.tan1@va.gov with the solicitation number and contractor name referenced in the subject line.
