DESCRIPTION

This is a Combined Synopsis Solicitation (CSS) for commercial services prepared in accordance with the format in FAR Subpart 12.6, “Streamlined Procedures for Evaluation and Solicitation for Commercial Products and Commercial Services.,” using Simplified Acquisition Procedures found at FAR Part 13 and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation shall not be issued.

This Solicitation number 36C24425Q0627 is being issued as a Request for Quote (RFQ). The Government anticipates to enter a Purchase Order resulting from this solicitation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2025-04 (Effective Date 06/11/2025) and can be found at: FAR | Acquisition.GOV .

The associated North American Industrial Classification System (NAICS) code for this procurement is 722310 with a small business size standard of $47.0 million and the Product/Service Code is: S203.

This solicitation is a service-disabled veteran owned small business (SDVOSB) set-aside. Any SDVOSB responding to this solicitation must complete, sign, and return the full VAAR clause at 852.219-75 (included within).

This solicitation requires registration with the System for Award Management (SAM) prior to award, pursuant to applicable regulations and guidelines. Registration information is at: http://www.sam.gov . Confirmation of SAM registration will be validated prior to awarding a contract.

The Veterans Integrated Service Network 04 (VISN4), has a new requirement for a Food Service at Corporal Michael J. Crescenz VAMC as listed in the Statement of Work (SOW), Attachment A.

The Contractor shall provide all necessary licenses, certifications, training, and requirements cited in the SOW. All work is to be performed in accordance with applicable federal, state, and local regulations or ordinances.

All interested offerors shall provide a quotation for the following: Pricing Schedule, Attachment B.

Place of Performance: Corporal Michael J. Crescenz VAMC.

VISN-04 intends to enter into a Purchase Order anticipated to begin: 09/01/2025-08/31/26, with the inclusion of four 1-year option periods.

852.219-75 VA Notice of Limitations on Subcontracting - Certificate of Compliance for Services and Construction. (NOV 2022)

(a) Pursuant to 38 U.S.C. 8127(k)(2), the offeror certifies that -

(1) If awarded a contract (see FAR 2.101 definition), it will comply with the limitations on subcontracting requirement as provided in the solicitation and the resultant contract, as follows: [Contracting Officer check the appropriate box below based on the predominant NAICS code assigned to the instant acquisition as set forth in FAR 19.102.]

(i) ☒Services. In the case of a contract for services (except construction), the contractor will not pay more than 50% of the amount paid by the government to it to firms that are not VIP-listed SDVOSBs as set forth in 852.219-73 or VOSBs as set forth in 852.219-74. Any work that a similarly situated VIP-listed subcontractor further subcontracts will count towards the 50% subcontract amount that cannot be exceeded. Other direct costs may be excluded to the extent they are not the principal purpose of the acquisition and small business concerns do not provide the service as set forth in 13 CFR 125.6.

(ii) □ General construction. In the case of a contract for general construction, the contractor will not pay more than 85% of the amount paid by the government to it to firms that are not VIP-listed SDVOSBs as set forth in 852.219-73or VOSBs as set forth in 852.219-74. Any work that a similarly situated VIP-listed subcontractor further subcontracts will count towards the 85% subcontract amount that cannot be exceeded. Cost of materials are excluded and not considered to be subcontracted.

(iii) □ Special trade construction contractors. In the case of a contract for special trade contractors, the contractor will not pay more than 75% of the amount paid by the government to it to firms that are not VIP-listed SDVOSBs as set forth in 852.219-73 or VOSBs as set forth in 852.219-74. Any work that a similarly situated subcontractor further subcontracts will count towards the 75% subcontract amount that cannot be exceeded. Cost of materials are excluded and not considered to be subcontracted.

(2) The offeror acknowledges that this certification concerns a matter within the jurisdiction of an Agency of the United States. The offeror further acknowledges that this certification is subject to Title 18, United States Code, Section 1001, and, as such, a false, fictitious, or fraudulent certification may render the offeror subject to criminal, civil, or administrative penalties, including prosecution.

(3) If VA determines that an SDVOSB/VOSB awarded a contract pursuant to 38 U.S.C. 8127 did not act in good faith, such SDVOSB/VOSB shall be subject to any or all of the following:

(i) Referral to the VA Suspension and Debarment Committee;

(ii) A fine under section 16(g)(1) of the Small Business Act (15 U.S.C. 645(g)(1)); and

(iii) Prosecution for violating section 1001 of title 18.

(b) The offeror represents and understands that by submission of its offer and award of a contract it may be required to provide copies of documents or records to VA that VA may review to determine whether the offeror complied with the limitations on subcontracting requirement specified in the contract. Contracting officers may, at their discretion, require the contractor to demonstrate its compliance with the limitations on subcontracting at any time during performance and upon completion of a contract if the information regarding such compliance is not already available to the contracting officer. Evidence of compliance includes, but is not limited to, invoices, copies of subcontracts, or a list of the value of tasks performed.

(c) The offeror further agrees to cooperate fully and make available any documents or records as may be required to enable VA to determine compliance with the limitations on subcontracting requirement. The offeror understands that failure to provide documents as requested by VA may result in remedial action as the Government deems appropriate.

(d) Offeror completed certification/fill-in required. The formal certification must be completed, signed and returned with the offeror's bid, quotation, or proposal. The Government will not consider offers for award from offerors that do not provide the certification, and all such responses will be deemed ineligible for evaluation and award.

Any questions or concerns regarding this solicitation should be forwarded in writing via e-mail to the Point of Contact listed below.

Quotes in response to this RFQ are due on 10:00 AM (EST), Tuesday, July 22, 2025. Quotes shall be submitted in writing and sent via email to Contracting Specialist Britni Greenleaf: Britni.Greenleaf@va.gov. Please insert “Solicitation Number 36C24425Q0627 “CMJC VA Food Services” in the subject line of your email. All submissions shall be 5MB or less. If the email is more than 5MB, submit multiple emails labeling them 1 of XX, 2 of XX, etc. Late submissions shall be treated in accordance with the solicitation provision at FAR 52.212-1(f).

Any questions or concerns regarding this solicitation shall be forwarded in writing via email to the Contracting Specialist by 10:00 AM EST, Tuesday, July 15. Questions will not be accepted after Tuesday, July 15. Responses to the Offeror’s question(s) will be posted via an amendment on SAM.gov / Contract Opportunities via: http://www.sam.gov

Failure to follow all instructions may result in Offeror(s)s being determined as non-responsive and be removed from this competition. All proposals shall be valid 60 days from the quote response date.

Offeror(s)s are cautioned to be responsive to all requirements in the SOW and to provide sufficient information to allow for evaluation of the quotes.

Offeror’s are required to submit the following:

1. Attachment B: Price Schedule: Please complete price information in the last two columns of Attachment B and return with your quote.

2. To facilitate the award process, all quotes shall include a statement regarding the terms and conditions herein as follows:

4. Offeror(s)s shall include a completed copy of the provision 52.212-3 with their quote if the Offeror(s) has not completed it electronically in the System for Award Management (SAM), accessed via: http://www.sam.gov

5. Offeror(s)s shall submit a Technical Capability which shall address the following at a minimum:

a. Training/Certifications. Describe how food handlers are trained and provide examples of certifications required for food safety and sanitation practices.

b. Infrastructure. Provide information about the staffing structure that will be utilized by the Contractor to ensure timely preparation and serving of meals during mealtimes.

c. Communication. Describe in detail the communication methods intended to be utilized by the Contractor during the performance of the awarded contract.

d. Contingency Plan. Provide a contingency plan for unforeseen disruptions to meal services such as staff call outs and/or food shortages.

6. Review and complete the full VAAR Clause at 852.219-75 - VA Notice of Limitations on Subcontracting

The following are factors used in the comparative evaluation of quotes:

The Government will perform an evaluation using a comparative evaluation of each quote. The Government will compare quotes to one another in order to select the quote that best benefits the Government. Comparative evaluation will be conducted in accordance with FAR 13.106-2(b)(3). The following factors shall be used to evaluate Offeror(s)s:

1. Price
2. Technical Capability
3. Past Performance

(b) A written notice of award or acceptance of a quote, mailed or otherwise furnished to the successful Offeror(s) within the time for acceptance specified in the quote, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept a quote, whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.

The Contracting Officer will award one, firm fixed price contract resulting from this RFQ to the responsible Offeror(s) whose quote conforming to the RFQ is most advantageous to the Government.

i. Price— The government reserves the right to award to an Offeror with other than the lowest price. The award will be made to the Offeror whose quote represents the best overall value to the government.

ii. Technical— The Offeror clearly demonstrates in its response a clear understanding of all the Government’s requirements and effective approach for the completion of service in accordance with the SOW.

iii. Past Performance— Offerors must have a record of satisfactory or neutral past performance. If no record of past performance is found, the Offeror will receive a rating of neutral. The Government will utilize but is not limited to internal documentation including Federal Awardee Performance and Information System (FAPIIS) for determining the past performance of Offerors.

STATEMENT OF WORK

NUTRITION & FOOD SERVICE

Snyder House MHRRTP Corporal Michael J. Crescenz Va Medical Center 3900 Woodland Avenue, Philadelphia, Pa 19104

DESCRIPTION OF SERVICES

SCOPE OF WORK: The contractor shall provide comprehensive meal preparation and serving services for the Corporal Michael J. Crescenz VA Medical Center’s Residential Rehabilitation Treatment Program (RRTP). Services by the contractor are to include the preparation, cooking, and daily serving of breakfast, lunch, dinner, and snacks, including on weekends and holidays. The contractor shall be prepared to serve up to forty meals per service to ensure all residents are adequately fed. All meals provided shall meet the dietary and nutritional requirements as stated within this Statement of Work (SOW). Adherence to Doctor’s orders and dietician consultation recommendations for restricted and/or special diets is also required.

PLACE OF PERFORMANCE:

SERVICE REQUIREMENTS: The awarded contractor shall furnish all personnel, food, and beverages necessary to prepare meals as described below for the residents of "Snyder House" Behavioral Health VA Medical Center, Residential Rehabilitation Treatment Program (RRTP). All equipment necessary for food preparation, cooking, heating, and refrigeration equipment, and tools will be provided to the Contractor as Government Furnished Material. Additionally, all pots, pans, serving utensils, cups, dishes, forks, spoons, knives, trays, trash containers, will be provided by the Government.

PERFORMANCE REQUIREMENTS: The Contractor shall be responsible for providing the following meals:

Breakfast: Breakfast will consist of a continental breakfast and full hot breakfast and shall be served every day, including weekends and all holidays. Breakfast shall be prepared and placed in buffet tables by 6:45 am each day. Break down and cleanup of breakfast will start at 8:00 am.

Note: All requested boxed (to-go) meals for the day shall be prepared with breakfast. The request will be submitted to the contractor 24 hours in advance.

a. Continental Breakfast: A continental breakfast shall consist of a combination of the following: fresh fruit, assorted cold cereals with some of them being sugared, some non-sugared, and some low sodium; assorted varieties of Danish; assorted varieties of Bagels; bread for toasting; butter; assorted jelly flavors; milk, and yogurt.

b. Full Hot Breakfast: A full hot breakfast shall consist of hot, nutritious entrée’, starch, cereal, fruit or juice, hot/cold beverage and appropriate condiments.

c. Boxed Breakfast: A requested boxed breakfast shall consist of the same items that are delivered for that breakfast’s hot meal/or continental meal. However, they must be delivered in a sanitized container that can be refrigerated and then reheated in a microwave oven. The contractor shall also provide the microwave cooking time needed for proper reheating. A packaged drink must accompany the meal.

Lunch: Lunch will be served every day, including all weekends and holidays. A full lunch shall be prepared for the residents eating at the RRTP and a boxed lunch shall be provided for those residents who will be off-site during lunch time. Lunch shall be prepared and placed on the buffet tables by 11:15 am each day. Break down and cleanup of lunch will start at 12:30 pm.

a. Full Lunch: A full lunch shall consist of nutritious, hot food or cold platter to include entrée', starch, vegetable, salad, fruit, dessert, hot/cold beverages, and appropriate condiments.

b. Boxed Lunch: A requested boxed lunch shall consist of the same items that are delivered for that lunch's regular hot meal/cold platter; however, they must be delivered in a sanitized container that can be refrigerated and then reheated in a microwave oven. The contractor shall also provide the microwave cooking time needed for proper reheating. A packaged drink must accompany each meal.

Dinner: A full dinner will be served every day, including all weekends and holidays, and boxed dinners shall also be prepared for residents unable to return in time for dinner. Dinner shall be prepared and placed in buffet tables by 4:15 pm each day. Break down and cleanup of dinner will start at 6:00 pm.

a. Themed dinner meals will be prepared on the following Holidays: The Fourth of July, Veterans Day, Thanksgiving, and Christmas.

Consultation with RRTP Government personnel will take place prior to these holidays in order to definitize the meal content on these occasions.

b. Full Dinner: A full dinner shall consist of nutritious, hot food or cold platter to include entrée, starch, vegetable, salad, fruit, dessert, hot/cold beverage, and appropriate condiments.

c. Boxed Dinner: A requested boxed dinner shall consist of the same items that are delivered for that dinner's hot meal/cold platter'; however, they must be delivered in a sanitized container that can be refrigerated and then reheated in a microwave oven. The contractor shall also provide the microwave cooking time needed for proper reheating. A packaged drink must accompany the meal.

DETAILED PERFORMANCE REQUIREMENTS

1. By no later than August 1, 2025, the awarded contractor shall provide the 30-day meal plan to the RRTP for acceptance.

2. Services for breakfast, lunch, and dinner shall start September 1, 2025.

3. Contractor prepared meals shall meet the Federal Recommended Dietary Allowance (RDA) requirements with a fat content of 30% or less.

4. Meals shall be prepared in large trays that will fit the equipment outlined in the "Government Furnished Materials." section of this Statement of Work.

5. All meals will accommodate the dietary requirements of RRTP program residents.

6. The Contractor shall be serving all of the meals "cafeteria style.

7. At each meal, the contractor is responsible for determining and providing sufficient quantities of appropriate condiments (such as mustard, ketchup, ranch, hot sauce, relish, salt, salt-substitute, pepper, sugar, sugar-substitute, etc.).

8. For residents with restricted or special diets - individual meals may be prepared for them in order to meet their special dietary and nutritional needs. RRTP staff will provide these dietary requirements to the contractor at least one-day prior, except for same day admissions or changes in Doctor's orders. In those cases, RRTP will provide this notification as soon as possible, but at least two (2) hours prior to mealtime.

9. The individual trays used by residents to carry their food and drinks shall be placed in a particular designated area for easy access for the residents.

10. Contractor will provide a copy of the menu cycle and tally sheets at least two weeks in advance of the actual project start-up.

11. The contractor shall furnish to the RRTP, 30 days in advance, a varied lunch and dinner menu, with at least a three (3) week meal cycle (meals must not be repeated during the cycle). The menu for the regular meals and the restrictive diet meals will be reviewed and approved in writing by a Philadelphia VA Medical Center Registered Dietician.

12. The contractor must designate a person who shall provide oversight to individuals working on this contract to ensure all requirements outlined within this SOW are met. The contractor’s assigned person will also be the primary point of contact for VA Snyder House staff in regard to this contract, resident headcount for meal purchases, and meal preparation.

13. The RRTP will provide the food trays which will be used by the residents to hold their plates, cups/glasses through the line, and to carry their meal to their tables. The contractor will be responsible for ensuring that all used dishes, trays, cooking supplies, and utensils are washed and sanitized.

14. The contractor shall maintain a clean and sanitary kitchen and dining area at all times.

15. The contractor shall ensure that all food handlers are properly trained and certified in food safety and sanitation practices.

16. The contractor shall provide sufficient staffing to ensure the timely preparation and serving of meals.

17. The contractor shall ensure that staff members are courteous and professional in their interactions with residents and VAMC staff.

18. All food shall be placed in large serving trays which must fit into the hot and cold buffet tables supplied by the RRTP. The contractor must place the trays into the buffet tables, and also must place miscellaneous items requiring refrigeration (such as beverages, etc.) into the chilled serving cabinet located in the dining area adjoining the serving area.

19. The contractor shall meet or exceed the nutritional care requirements set forth by the Joint Commission in the Comprehensive Accreditation Manual for Hospitals.

20. The contractor shall provide a menu cycle that will meet the following dietary requirements as needed when ordered by RRTP:

21. Contractor will furnish tally (production) sheets, menus, and summary of the nutrient analysis of the regular and modified menus. Contractor must provide tally sheets for both hot and cold items. These batched daily tally sheets for the month must be provided by the fifth (5th) working day of the following month.

22. The contractor agrees to prepare meals in a manner that meets all Federal, State and Local health standards, and assures that all health and sanitation requirements will be met at all times.

a. All food products, including spices must be labelled with a an opened/cooked date and an expiration date.

b. Spices must be labelled with an expiration date.

23. Contractor must post any and all state certifications at the preparation facility.

24. Canned items will be opened by contractor and placed in storage containers with lids. The containers will be dated.

25. RRTP staff will submit an end of day census count to the Contractor following a census change to ensure preparation for the following day’s meals.

26. When meals are prepared, contractor will provide a summary sheet of the contents of meals prepared, and number of persons intended to be fed.

GENERAL INFORMATION

Quality Control: The contractor shall provide and maintain an inspection system acceptable to the Government covering the services under this contract. Complete records of all inspection work performed by the contractor shall be maintained and made available to the Government during contract performance and at least three (3) years after contract completion. The Government has the right to inspect and test all services called for by this contract, to the extent practicable, on all items and at all places of production during the term of the contract. The Government shall make every effort to perform inspections and tests in a manner that will not unduly delay the contractor's work performance.

Quality Assurance: When the Government performs food service inspections or tests on VA premises, the contractor shall furnish, without additional charge, all reasonable accommodations and assistance for the safe and convenient performance of these duties.

Formal Training: Each contract employee must complete the below VA specific TMS trainings prior to beginning work. Each contract employee will be sent TMS login instructions to complete the trainings and will be required to send a copy of each training certificate to the program POC or COR upon completion.

CONTRACTOR STAFF INFORMATION

Uniforms: All contractor employees shall wear contractor provided uniforms, (contractor code of dress), which include pants, shirts or blouses, and protective clothing which are clearly distinguishable from other VAMC uniforms currently in use. The uniform will be worn as designed by the manufacturer. Open toe shoes or sandals will not be worn. A basic leather shoe covering the entire foot is recommended. All personnel will display identification badges on their uniforms, which will show the employee’s last name and the contractor’s full name.

Personal Hygiene: Contractor’s personnel shall be clean, bathe daily (including hair), and wear a clean uniform daily. Fingernails will be clean and free of dirt. Hair will be neatly trimmed and combed. While in the kitchen area, all contractors will need to wear a hairnet or hat.

Security Requirements:

a. All contractor employees must complete the required onboarding documents and be fingerprinted in order to begin the onboarding process.

b. All contractor employees will be required to undergo a Background Investigation and be cleared by the personnel security specialist following a favorable background investigation outcome in order to be cleared to be on site and begin work.

c. All contractor employees will be required to obtain a Personal Identity Verification (PIV) card as part of the onboarding process.

d. The contractor’s employees shall wear visible identification at all times while on the premises. It is the responsibility of the contractor to park in the appropriate designated parking areas. The Department of Veterans Affairs will not invalidate or make reimbursement for parking violations of the contractor’s employees under any condition. Smoking is prohibited on VA grounds. Possession of weapons is prohibited. Violations of VA regulations may result in citation answerable in the United States (Federal) District Court, not a local district, state, or municipal court.

Damage: Extreme caution shall be exercised to prevent damage to the building and its contents. Any damage must be reported to the COR immediately for appropriate action. Any damage caused by the contractor’s staff will be repaired and or replaced to the satisfaction of the VA at the contractor’s expense. Damage to the building, its contents, or loss of government property, in excess of $100,000 will require a Report of Survey or Investigation Report by the VACC. Appropriate Government personnel will conduct the investigation. The responsible party will be required to replace and/or submit payment for the damage or loss.

Access (Keys): If the COR provides keys for access, it shall be the Contractors responsibility to safeguard and create a key control (record keeping) to control the keys provided. The Contractor shall be responsible for establishing a methods for making sure all keys issued to the Contractor by the Government are not lost or misplaced, and are not used by unauthorized persons. Any keys issued to the Contractor by the Government shall not be duplicated.

GOVERNMENT FURNISHED MATERIALS (GFM)

Department of Veterans Affairs regulations contain the basic procedures for the operation, maintenance and protection of property. The primary regulations and related procedures to be followed by the Contractor are listed below. Proposed changes to the basic procedures must be approved by the Contracting Officer or authorized representative and must be incorporated by modification into this contract.

1. Accountability for government furnished materials: All materials, supplies, equipment, and property furnished by the Government under this contract shall remain the property of the Government.

2. Use of govt. Property: Government property shall be used for official Government business only in the performance of this contract. Government property will not be used in any manner for personal advantage, business gain, or other personal endeavor by the Contractor or the Contractor's employees.

3. Safeguarding of government property: The contractor shall take all reasonable precautions in accordance with sound industry practices, as directed by the Government, or in the absence of such direction to safeguard and protect Government property. Under no circumstances shall the Contractor remove any GFM from the place of performance (Snyder House).

4. Malfunction of government property: The Contractor shall be responsible for reporting to the facility technical POC the malfunctioning of any Government equipment used by the Contractor or the Contractor's employees.
