DEFINITIONS

Accessible Material – any material to which access can be gained by any means other than significant destruction of building components, or, for the purposes of describing building occupant activities, a material subject to disturbance by routine use or maintenance activities.

ACM By Virtue of Contact – any material that is tested and found via laboratory analysis to be non-asbestos containing, but it is in contact with or applied using another material that is determined to contain asbestos. Since these materials cannot be separated during removal, the non-asbestos-containing material is now contaminated and will need to be disposed of as ACM. Some examples include: floor tile and associated mastic, linoleum sheeting and associated mastic, adhesive upon mastic application, etc.

AHERA – Asbestos Hazard Emergency Response Act

Asbestos – a generic name given to a number of naturally occurring hydrated mineral silicates that possess a unique crystalline structure, are incombustible in air, and are separable into fibers. Asbestos includes the asbestiform varieties of Chrysotile (Serpentine), Crocidolite (Riebeckite), Amosite (Cummingtonite-Grunerite), Anthophyllite, Tremolite, and Actinolite.

Asbestos-Containing Material (ACM) – any material or product which contains more than one percent asbestos.

Asbestos-Containing Building Material (ACBM) – means surfacing ACM, thermal system insulation ACM, or miscellaneous ACM that is found in or on interior structural members or other parts of a building.

Bulk Samples – samples of bulk material; in the case of asbestos, suspect material.

Chain-of-Custody – formal procedures for tracking samples and insuring their integrity.

DOT – United States Department of Transportation

Friable – the material, when dry, may be crumbled, pulverized, or reduced to powder by hand pressure, and includes previously non-friable material after such previously non-friable material becomes damaged to the extent that when dry it may be crumbled, pulverized, or reduced to powder by hand pressure.

Good Condition – as used in the context of material condition, integrity of the material is generally complete, with no visible damage or deterioration, or showing only very limited damage or deterioration. Possible small areas of delamination or indications of limited contact or damage. The mechanism to retain the insulation in its original position (e.g., cloth wrapping over pipe insulation) is still present.

Homogeneous Application – an application of surfacing material, thermal system insulation material, or miscellaneous material that is uniform in color and texture, and vintage of application.

ii

NESHAP – National Emission Standard for Hazardous Air Pollutants

NIST – NVLAP – National Institute of Standards and Technology, National Voluntary Laboratory Accreditation Program

Operations and Maintenance (O&M) Program – a program of work practices to maintain friable ACBM in good condition, ensure cleanup of asbestos fibers previously released, and prevent further release by minimizing and controlling friable ACBM disturbance or damage. An O&M Program should be implemented at all buildings with ACM.

Optical Microscope – a microscope which uses the transmission of light through lenses to magnify a specimen for examination. Capable of resolution of fibers or other materials down to approximately

0.25 micrometers in diameter.

OSHA – United States Occupational Safety and Health Administration.

Polarized Light Microscopy (PLM) – an optical microscopic method for the identification of asbestos in bulk samples in which the sample is illuminated with polarized light (light which vibrates in only one plane) to distinguish between different types of asbestos fibers by their shape and unique optical properties.

1.0 BACKGROUND

PARS Environmental, Inc. (PARS), under contract to the Veterans Affairs Medical Center – Wilkes-Barre (VAMC Wilkes-Barre), completed comprehensive asbestos surveys at the VAMC Wilkes-Barre complex in May 2014. The nine (9) buildings that comprise the VAMC Wilkes- Barre complex were included as part of the survey. The buildings were identified to PARS as the following:

• Building #1: Medical Center

• Building #3: Office Building

• Building #5: Office Building

• Building #8: CWT Residents

• Building #11: Boiler Room and Mezzanine

• Building #12: Engineering

• Building #17: Warehouse

• Building #27: Community Living Center

• Building #28: Emergency Generator

The comprehensive asbestos surveys were completed to locate and identify Homogenous Areas (HAs), identify the asbestos content of HAs, quantify asbestos HAs, assess the condition of HAs, assess the friability and risk of occupant exposure to existing asbestos materials, and provide cost estimates and priorities for damaged or compromised asbestos discovered during the surveys. The survey was completed following local, state, and federal laws, rules, and codes regarding asbestos including the United States Environmental Protection Agency (USEPA) Asbestos Hazard Emergency Response Act (AHERA, 40 CFR 763, Subpart E), USEPA Asbestos School Hazard Abatement Reauthorization Act (ASHARA, 1992), and the Occupational Safety and Health Administration (OSHA) asbestos regulations for general and construction industries (29 CFR 1910.1001 and 1926.1101, respectively).

2.0 INTRODUCTION

PARS Environmental, Inc. (PARS), under contract to the VAMC Wilkes-Barre, has prepared this Asbestos Survey Report for VAMC Wilkes-Barre Building #1 (“Site”).

The asbestos survey was performed by Mr. Julian Fernandez-Obregon and Mr. Cody Postlethwait of PARS. Mr. Fernandez-Obregon is certified as an Asbestos Building Inspector by the Commonwealth of Pennsylvania Department of Labor and Industry (Certification No.

051514) and was responsible for the technical aspects of the survey. Mr. Postlethwait is certified as a USEPA/AHERA Asbestos Building Inspector and assisted Mr. Fernandez- Obregon with non-technical aspects of the survey. Copies of their certifications are included in Appendix A.

The objective of the survey was to identify suspect asbestos-containing material (ACM) for potential removal prior to renovation/demolition activities impacting these materials. During the course of the work, PARS was only allowed to inspect accessible areas. Therefore, only

PARS Project No. 1040-01 limited access was obtained for destructive testing to identify materials which may be hidden or obstructed at the Site. Examples of concealed or obstructed areas include, but are not limited to, wall cavities, pipe chases, spaces above fixed ceilings, and equipment interiors.

The USEPA defines an ACM as a material that contains greater than one percent (>1%) asbestos. A list of applicable regulations of the local, state, and federal regulations governing asbestos are provided in Appendix B.

3.0 SITE DESCRIPTION

The Site encompasses approximately 498,533 square feet (SF) and is a thirteen-story facility referred to as the Main Hospital. The building is constructed with a steel-frame with a brick exterior. There are multiple roofs on this building, on several different levels, some having been replaced recently. Roofing materials vary from a newer rubber membrane roof, to an older, felt and stone covered roof. The interior finishes vary floor to floor, with varying amounts of concrete, carpet, and vinyl floors, ceiling tile of different types, and mostly plaster walls, but some with drywall and joint compound. The structure is primarily heated by a large multi-boiler plant on the basement level, identified as a separate building from the Main Hospital, with heat provided by radiators on most floors.

4.0 METHODOLOGY

PARS performed an asbestos survey at the VAMC Wilkes-Barre from May 5 through May 15, 2014. During that time, PARS completed its survey of the Site. The survey approach included a visual inspection of the suspect ACM. The survey included limited intrusive testing. PARS did not use destructive testing techniques in concealed or obstructed areas during the survey.

All accessible suspect materials were sampled regardless of previous results or age of material.

Suspect materials were divided into homogeneous areas, which are building materials evaluated by the inspectors to be the same based on their color, texture, and suspected age.

Representative samples were collected of each homogeneous material. Suspect materials identified and sampled in Building #1 are listed in Table 1. A photographic log of confirmed ACM is included in Appendix C.

Bulk samples of suspect materials were collected in 4-mil plastic bags and sealed for transport to the laboratory under chain-of-custody protocols. Each sample was assigned a unique homogeneous application number (e.g., M-1, M-2, S-1) and sample identification number (e.g., MH-13-1). If the laboratory identified layered materials in the sample submitted for analysis, for reporting purposes it was assigned the same sample number as the base sample; however, the corresponding homogenous application number and an “A” added (e.g., M-1A, S-1A).

The following are the homogeneous material symbols used for identification in this report:

T = Thermal System Insulation (TSI) M = Miscellaneous Materials S = Surfacing Materials (troweled or spray-applied)

4.1 ANALYTICAL METHODS

Laboratory analysis was conducted by International Asbestos Testing Laboratories, Inc. (IATL) located in Mount Laurel, New Jersey. IATL is certified by the National Institute of Standards and Technology and National Voluntary Laboratory Accreditation Program (NIST-NVLAP #100188) for asbestos analysis. Material identification was performed via Polarized Light Microscopy (PLM) using USEPA Method 600/R-93/116. Samples containing 10% or less of asbestos were also subject to the USEPA 400-point count method. Certifications for IATL are included in Appendix D.

4.2 FINDINGS

Based on the visual inspection performed by PARS, friable and non-friable suspect ACMs were identified at the Site. Below is a list of suspect ACMs that were identified and sampled. A total of 563 bulk samples were collected for analysis. Bulk asbestos sample results are summarized in Table 1 and the corresponding laboratory reports are included in Appendix E. PARS Project No. 1040-01

The identified ACMs were noted to be in good condition (i.e., no visible damage or deterioration or showing only very limited damage or deterioration) with the exception of pipe insulation and associated pipe fittings located within the basement level construction area in the west wing (Homogenous Areas S-117, T-118, T-119, T-120).

Destructive testing methods to access concealed and obstructed areas were not employed as part of the survey. Limitations and service constraints are discussed in Appendix G.

4.3 SURVEY OBSERVATIONS

Several homogenous areas were created that spanned several floors, with a minimum of one (1) sample collected per floor for the respective homogenous area. However, lab analysis created a subset of certain homogenous areas, as is the case for M-38 (12” x 12” White Floor Tiles with Beige Spots). The first samples collected on the tenth floor were associated with a yellow and black mastic combination, as determined by the laboratory split, with these samples analyzed showing no asbestos. The same floor tiles were observed on the basement and first floor with mastic that was analyzed positive by the laboratory. Although both share a homogenous area, as the tiles are identical, the underlying mastic is positive on the lower floors, but negative on the upper floors. This allows a split within a homogenous area, as the mastics analyzed by the laboratory showed different results. The homogenous area is not universally identified as ACM, only the area with the mastic confirmed as ACM by the laboratory. This case is observed in several instances, where the mastic is positive for one homogenous area of floor tiles, but in another area with the same tiles where different analytical results for the mastic are identified.

Regarding TSI, confirmed ACM pipe runs were found in the mechanical rooms on the east and west wings, and elevator shaft mechanical rooms of most floors. Suspect pipe insulation above the drop ceiling was only observed on the fourth and tenth floors. The overhead pipes on the other floors were observed to be fiberglass pipe insulation, and not suspect ACM. From the fifth floor down to the basement, in the north wing, suspect pipe insulation was observed and sampled, again remaining contained to a single mechanical shaft, and not branching out above the drop ceiling. Sub-wall pipe insulation was visible only in the basement west wing construction area, where most walls were open and interstitial spaces were exposed for renovation.

5.0 CONCLUSIONS AND RECOMMENDATIONS

Based on the findings of the survey, ACM was identified in Building #1. The identified ACMs, detailed in Section 4.2, were noted to be in good condition (i.e., no visible damage or deterioration or showing only very limited damage or deterioration) with the exception of referenced pipe insulation and associated pipe fittings located within the basement level construction area in the west wing.

PARS Project No. 1040-01

Damaged ACM located within the basement level construction area in the west wing should be removed by a Pennsylvania licensed asbestos contractor with appropriate air monitoring and oversight. Renovation in this area should not proceed until the damaged ACM has been removed and air clearance has been achieved. The damaged ACM included:

• S-117 Black Waterproofing/Vapor Barrier

• T-118 6” Pipe Insulation

• T-119 Pipe Fitting Insulation Associated w/6” Pipe

• T-120 3” Pipe Insulation

All ACM should be removed by a Pennsylvania licensed asbestos contractor prior to renovation or demolition activities impacting these materials. For ACM in good condition that will remain in-place, an operations and maintenance (O&M) program should be developed. The O&M program will define work practices, training requirements, and management procedures designed to maintain the ACM in good condition.

If suspect ACM not referenced in this report is identified in concealed or obstructed areas during renovation and/or demolition activities, it is recommended that the activities that will disturb these materials cease and the materials be sampled and analyzed to assess the potential presence of asbestos. Cost estimates for abatement of the damaged ACM is provided in Table 2.
