About this file
This document provides questions and answers regarding solicitation number 36C24422Q0838 for staffing anesthesiology physician and certified registered nurse anesthetist services. The solicitation is for a new requirement at the Wilkes-Barre VA Medical Center involving an on-site 8-hour tour of duty for physicians and CRNAs Monday through Friday, as well as on-call coverage evenings and weekends. The opportunity is set aside for service-disabled veteran-owned small businesses, veteran-owned small businesses, small businesses, and large businesses through a tiered evaluation process. Incumbent information and pricing details are not applicable as this is a new requirement. Training hours are billable, and a Drug Enforcement Administration certificate is not required for CRNAs. The medical center requires replacement coverage within 2 hours and has three operating rooms along with cardiac catheterization, gastrointestinal, and other outpatient procedure suites.

N/A
4. If contract renewal, are the required hours on the Price/Cost schedule filled by only 1 Anesthesiologist and 1 CRNA? If not, how many Anesthesiologists and how many CRNAs are used to cover the required working and on-call hours?
N/A
5. If the positions are already filled, are the incumbent Anesthesiologist(s) and CRNA(s) fully COVID vaccinated?
N/A
6. If there are incumbent health care providers (HCP), please provide their names, phone number, and email address.

7. On the Price/Cost schedule, items 0002, 0005, 1002, 1005, 2002, 2005, 3002, 3005 lists
0.00 under the hour quantity. Can the government confirm that this is correct?
Yes
8. Page 17, item 3.1.3 states the HCPs must be available on-call during all hours when the Wilkes-Barre VAMC clinic is closed, including evenings, weekends and holidays. Yet the Price/Cost schedule has 2080 hrs of work time and 3200hrs of on-call time per FTE. That equates to 5280 hrs per FTE for the year, which is not enough hours to cover all evenings, weekends, and holidays (5280 hrs / 365 days = 14.47 hrs / day for normal work hrs and oncall hrs). Can the government clarify exactly what the expectation regarding call hours are?If call is expected to be for all hours when the VAMC is closed, request the on-call CLIN hrs be increased.
On site 8 hours tour of duty from 7:30 am to 4 pm Monday to Friday.
On call hours from 4pm though 7am next day during weekdays Weekend call starts at 4pm Friday and ends at 730am Monday CRNA is expected to report to the facility when called within 30 minutes.
CRNA will be called for any after hour surgery being done in the facility or for airway management.
9. Page 12, item 2.1.6 – Training. Will the VA provide the HCPs the training listed in the table? Are these hrs billable?
Yes, these training are provided by VA and are billable hrs
10. Page 14, item 2.1.9 – DEA. It is customary for Physician Anesthesiologists to have a DEA certificate, but CRNAs do not typically carry one. Can the government confirm if it is mandatory that the CRNA(s) have a DEA certificate as well? If so, will the VA/Government reimburse for the cost of obtaining such certificate (typically $700-800 every 3 years).
CRNA should not hold a DEA certificate to be employed (confirmed by Credentialing &Privileging Service)
11. Page 16, item 2.9.3 – Coverage. The solicitation states “… the contractor shall provide replacement physician coverage within 2 hours…”. Does the government only require this rapid replacement coverage for the Physician Anesthesiologist(s), or for the CRNA(s) as well? With the 2 hour replacement requirement, does the Government require more than one Anesthesiologist and more than 1 CRNA to be credentialed and trained in order to fulfill this requirement?
Yes
12. Page 18, item 4.1.1 – What method is currently used for the HCPs signing in and out when in attendance?
Anesthesiologist will do sign out
13. How many Operating Rooms are in the hospital?
Three Operating Rooms
14. How many and what type of out-of-OR suites are covered by the anesthesia department (for example: endoscopy, MRI, Gastroenterology, Cardiac Cath, etc)?
Cardiac Cath, GI Suites, Pulmonary, ER, In patient unit medical floor and ICU
15. Page 20, item 4.5.2.1.1 states that services include patients undergoing cardiac surgery, orthopedic joint replacement, and major vascular, thoracic, neurosurgical, plastic, ENT, urologic and ophthalmologic operations. Can the Government confirm that the hospital actually does these types of cases?
No, we do not perform heart surgery, plastics, nor ENT procedures
16. Page 20, item 4.5.2.1.2 states that services include patients undergoing electroconvulsive therapy and diagnostic and therapeutic procedures in the cardiac catheterization laboratory, electrophysiology, gastrointestinal and radiological suites. Can the Government confirm that the hospital actually does these types of cases?
We do not perform ECT, electrophysiology
17. Page 29, VHA Supplemental Contract Requirements for Combatting COVID-19, mentions testing requirements for HCPs who are unvaccinated. Does the VA Hospital or the Anesthesia Department provide testing kits for those HCPs who must test?
No
18. Page 69, E.7, the paragraph and verbiage seem to have been cut off, as it states: “The following factors shall be used to evaluate offers: Technical and past performance, when combined, are .” Can the government clarify the entirety of that paragraph?
This section should read:
E.3 52.212-2 EVALUATION—COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES (NOV 2021)
(a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. Comparative/Tiered Evaluation will be used to evaluate offers:
What Comparative Evaluation is:
1. It is the direct comparison of one quotation/offer with another in a uniform and fair manner to determine which quote/offer provides the Government what it needs, where and when as identified in the Request for Quote (RFQ).
2. It is an assessment of which response is the “best” as a whole.
3. Once one quotation/offer is found acceptable, it is compared side by side to the remaining ones, and the “best” one is chosen.
What Comparative Evaluation is not:
1. It does not individually evaluate quote/offers against specified evaluation factors.
2. It does not assign a rating using a color, numerical, or other rating methodology.
3. It does not require additional evaluation to determine if minimal advantage is worth a minimal or normal price difference, but not prohibited.
4. It is not a low price technically acceptable (LPTA) or trade-off process.
This procurement is set aside based on an order of priority as established in 38 U.S.C. 8127.
TIERED EVALUATIONS INCLUDING LARGE BUSINESS CONCERNS: This solicitation is being issued as tiered evaluation with the following tiers: (1) Service-Disabled Veteran Owned Small Business concerns, (2) Veteran Owned Small Business concerns, (3) Small Business concerns, and (4) Large Business concerns. If award cannot be made, the solicitation will be cancelled, and the requirement resolicited.
(b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).
