2. STATEMENT OF WORK-DENTAL LAB SERVICES

A. Background:

At the time of issue of this solicitation for commercial dental laboratory contract providers, the Dental Service at the VA Pittsburgh Healthcare System (VAPHS) utilizes both in house and contracted laboratories for various stages in the fabrication of oral prostheses. VAPHS seeks a contract to provide local commercial dental laboratory services listed in the schedule below in a high quality, efficient and cost effective manner. This requirement is for a local commercial dental laboratory to provide for the scheduled dental services. To be eligible, the commercial dental laboratory must be able to commute and provide pick-up/delivery services from the dental laboratory to the Dental Clinic at VAPHS Heinz Campus in approximately 1 hour allowing for such variables as traffic and weather. The local commercial dental laboratory must provide courier service for pick-up and delivery of case work from the Dental Clinic at the VAPHS Heinz Campus. No other methods of pick-up/delivery are acceptable. Having worked with both local and out of town labs in the past, VAPHS has experienced exponentially more breakage with shipping than with hand delivery leading to repeated procedures and increased treatment time as well as increased cost. The staff of the Dental Clinic at the VAPHS Heinz Campus requires frequent personal attention from a representative of a local commercial dental laboratory to visualize patient issues/problems to then discuss specific solutions and capabilities. This also includes product that needs visualized for minor corrections. It is common that work submitted by a lab requires a change, minor tweak, etc. At times their mistakes are even more extensive. The ability to phone a local lab on the spot and have that case back in their hand and the problem addressed the same day or the next morning is invaluable. This is a prime factor for this requirement. The Dental Clinic staff also requires personal attention on critical cases. At times, there are highly specialized prosthetics required for surgeries where there is collaboration between several surgical disciplines. Too much balances on that prosthetic being exactly correct. A local commercial dental laboratory allows a professional rapport to develop increasing the understanding of the needs of Dental Clinic staff increasing successful product from the local dental laboratory.

B. Standards of Performance:

1. The Contractor shall fabricate all prostheses and perform all laboratory steps to the highest standard of quality, with special attention given to neatness and laboratory prescription detail. Materials used shall be consistent with those listed in the schedule and should meet all Food and Drug Administration (FDA) requirements and approval.

2. The Contractor's laboratory facility must be certified by the American Dental Association (ADA), National Association of Dental Laboratories (NADL), or equivalent. The Contractor shall provide a copy of all relevant licenses, permits, registrations and certifications inclusive of any sanctions current or pending throughout the United States of America along with proposal.

3. The VAPHS reserves the right to require “remake” at no additional cost if casework does not conform to the highest standard of quality and the deficiency is clearly caused by the Contractor.

4. All work shall be completed only by the specific lab under contract or an affiliated laboratory directly owned by the specified laboratory under contract. The Contactor, under no circumstances will sub-contract the laboratory work to another laboratory. All laboratory work must be performed within the United States of America. The lab under contract and/or its’ affiliated laboratory owned by the specified laboratory under contract are to maintain the same standards of materials specified in this Statement of Work (SOW).

C. Pick-Up & Delivery of Casework:

1. The commercial dental laboratory must be able to commute and provide pick-up/delivery services from the dental laboratory to the Dental Clinic at VAPHS Heinz Campus in approximately 1 hour allowing for such variables as traffic and weather. The local commercial dental laboratory must provide courier service for pick-up and delivery of case work from the Dental Clinic at the VAPHS Heinz Campus. The Dental Clinic is located at VAPHS, H. J. Heinz Division, 1010 Delafield Road, Pittsburgh, PA 15215. The proximity allows for same day turnaround time on repair cases. The proximity also allows for personal attention on specific cases.

2. VAPHS anticipates that daily inside pick-ups and/or inside deliveries will be required. As noted above, it is common that work submitted by the lab requires a change, minor tweak etc. At times their mistakes are even more extensive. The ability to phone a local lab on the spot and have that case back in their hand and the problem addressed the same day or the next morning is invaluable. The inside point of pick-up/delivery will be specified upon award. The particular times for scheduled pick-ups will be determined by mutual agreement between the VAPHS Dental Service and the Contractor. Transportation of casework shall be provided at no additional cost to VAPHS; therefore, unit pricing of the line items contained in the schedule should appropriately reflect transportation costs by courier.

3. The Contractor will provide all the necessary requisitions/prescription forms and packaging materials for the transportation of the casework by courier at no additional cost to the VAPHS.

4. The Contractor will meet the Turnaround Time (TAT) for all line items as defined below. TAT is the time required from pickup of the order by the courier utilized by the contracted local commercial dental laboratory from the Dental Clinic at the VA Heinz Campus until the time of delivery of finished case by courier utilized by the contracted local commercial dental laboratory to the Dental Clinic at the VA Heinz Campus including transit time.
