About this file

This is a Limitations on Subcontracting Certification form required for solicitation 36C24226Q0769, a Total Small Business set-aside RFQ issued by the Department of Veterans Affairs Veterans Health Administration Veterans Integrated Service Network 2. The certification applies to the procurement of Armored Courier Service for two New York VA facilities: Margaret Cochran Corbin VA Campus (pickup weekly on Wednesday mornings) and St. Albans VA Medical Center (pickup weekly on Thursday afternoons), with a 12-month period of performance from award through July 31, 2027, plus four optional years. The NAICS code is 561613 with a $43.0 million small business size standard.

Offerors must certify compliance with 13 C.F.R. 125.6 and applicable FAR/VAAR clauses by selecting one of two options: either no subcontractors are anticipated and the prime receives 100% of contract value, or the prime and similarly situated subcontractors collectively receive at least 50% of contract value with subcontractors meeting the size standard and VIP certification. Contractors must document self-performed work and subcontracted work by percentage cost, with each subcontractor identified by business name, DUNS number, and work description. Violations of subcontracting limitations carry severe penalties, including fines of $500,000 or the amount spent in excess of permitted levels, and potential debarment. Contracting Officers may require documentation of compliance at any time during contract performance.
