Project No. SL-24-001 Replace Surgical Suite Flooring

Ques�on 1: For the Surgical Suite Flooring at Manhatan VA (36C24224B0007), Drawing AF100 refers to ‘Laminate Flooring’ however there is not a Basis of Design or specifica�on for such. Please issue.

Response 1: Furnish and Install Epoxy Floor with integrate base to match existing elevation. Remove existing vinyl tiles 1x1 and pre floor for new flooring.

Specification 09 67 23.40 Resilient Flooring

Ques�on 2: For the Surgical Suite Flooring at Manhatan VA (36C24224B0007), there is an exis�ng 6” cove to be removed.

• Confirm all bidders are to include removing and replacing the botom 12” of wall with high impact gypsum board, and related pain�ng, etc.

• If so, issue paint specifica�on.

Response 2: Contractor will repair up to 100LF of damaged walls. Replace bottom 12” of the wall with 5/8” High Impact board up to 2 layer with metal stud/track 14 ga. up. Paint to match Semi-gloss

Benjamin Moore or equal approve: Linen White - OC151 and White 912.

Ques�on 3: Which rooms (or room #’s) have exis�ng 1x1 floor �le. Not all the rooms were accessible in the site visit.

Response 3: Room named DARK RM 4594. This room was accessible during the site visit.

Ques�on 4: For rooms with 4x4 wall �le, can the resinous cove base be installed on top of it, or is the scope to demo the wall �le, repair the wall/ gypsum board, and install the resinous cove base in it’s place? Provide detailed informa�on.

Response 4:

Contractor to remove/ demo tiles and repair wall for new cove base per above detail.

Ques�on 5: It appears drawings AF-100 and AF-101 are not to the 1/8” scale indicated on the drawings.

• Issue drawings to scale so takeoffs can be completed.

• The rooms have numbers below them. Advise if these numbers are the accurate Square

Footage of each area.

Response 5: Revised scale for AF-100, AF-101

Drawing scale is 3/32” = 1’-0”

Ques�on 6: Will the Contractor handle installa�on of door transi�on strips?

• If so, provide a profile as well as the base cap/trim throughout.

Response 6: Contractor will terminate approved mobilization phase with aluminum strip.

Ques�on 7: To obtain a Targeted 1,000 Sq� per week we will need addi�onal �me/days is this an op�on?

Response 7: Estimate 10 mobilization.

Ques�on 8: Will the VA be keeping the current saddles to match eleva�ons? Some are cracked.

Response 8: New flooring transition consist only in aluminum strip, no saddle transition to match or repair.

Ques�on 9: We suggest addi�onal (Phases) Weekends, 800 sq� is a realis�c Target with 16 (Phases) Weekends approximately is this an op�on?

Response 9: Estimate 10 mobilization.

Ques�on 10: The Cove cap cannot be installed without the tongue under another wall type like �le, or sheet. The cap would need to be installed then we cove Acelera up to the Cap.

• Clarify

Response 10: Please follow manufacture detail for cove strip AF-9, Aluminum strip. Install grout between bottom of the ceramic tiles and aluminum strip.

Ques�on 11: If the VA chooses to use op�onal 2nd topcoat to provide a smoother finish over the vinyl chip, a recommended addi�onal 24 hour recoat window if VA decides to go with 2nd topcoat.

• Clarify inten�on

Response 11: Yes, second coat required. Additional coat of Accelera EXT will provide additional floor protection.

Ques�on 12: It is professional opinion that the �me frame alloted of 7PM on Friday to Sunday morning 5:00am to lay 1,000 Sq� of flooring if the op�onal 2nd topcoat is used.

• Is this the VA’s intent

Response 12: Contractor to provide phasing plan schedule for VAMC approval to meet 10 or less mobilization.

Ques�on 13: Bid documenta�on indicates 1 layer of Chip Broadcast. Cer�fied installer usually installs 2 layers of chip broadcast;

• is 1 or 2 layers of Chip Broadcas�ng is necessary?

Response 13: Apply 1 Broadcast layer.

Ques�on 14: Bid documenta�on states 210 calendar days to complete the project, however it also states that the contractor should complete the job in 10 mobiliza�ons from Fri-Sun;

• Will all 6 phases need to be completed in 10 weekends total?

Response 14: Contractor must submit schedule /phasing for approval. Estimate 10 mobilization within 210 calendar days.

Ques�on 15: Are all 6 phases shown on the bid phasing plan final, or will there poten�ally be more phases?

• If so, will there be addi�onal mobiliza�ons alloted to complete the project?

Response 15: Contractor must submit schedule /phasing for approval within 10 mobilization

And within 210 calendar days.

Ques�on 16: Will it be necessary to work over long weekends and holidays?

• If so, this will increase the labor cost. Advise how many holiday weekends where work is expected to take place.

Response 16: Yes, Contractor is responsible to include premium labor rates for work in their bids for work hours iden�fied within the contract documents. Bid shall include a statement with acknowledgement that the fixed fee includes premium pay for work for this project.

Ques�on 17: What will the ICRA classifica�on be for each phase?

• Provide a detailed ICRA drawing

Response 17: Contractor must provide ICRA Level IV at all times.

Ques�on 18: Will all the ICRA measures for that classifica�on be necessary to implement?

Response 18: Class IV must be implemented measures.

Ques�on 19: Will the contractor be required to construct anterooms for each phase?

Response 19: Follow ICRA Level IV requirements.

Ques�on 20: Bid document 3.0 SOW Item 4 says to prep wall to allow for new cove base for up to 50LF 12” AFF and to use high impact GP up to double layer 5/8 with studs and track. Where is this work taking place?

Response 20: After demo existing cove base Contractor and CO will inspect to quantify of damaged walls.

*Contractor to estimate up to 100LF of repairs.

Ques�on 21: Bid document 3.0 SOW Item 11 says to furnish and install 100LF of 6” cove base at control sta�on and holding room. During the walk-through we no�ced that the exis�ng cove base is 4” high with plas�c splash guard protec�on and corner guards just above the 4” cove base.

• Will the contractor be required to remove, shorten, and reinstall the splash guards behind the beds, and patch and paint the walls where they are installed?

• Will the contractor be required to remove and reinstall the corner guards above the new 6” cove base?

Response 21: Contractor to remove existing base and furnish and install integral base to match

Ques�on 22: Will the room with ceramic �le (4594C) be required to have 6” integral cove base?

• If so, will all the wall ceramic �le need to be removed and replaced?

Response 22: Demo existing ceramic to allow 6” cove base with aluminum strip.

This demo shall include any wall repair required.

Ques�on 23: What is the thickness of exis�ng ceramic �le flooring system above the exis�ng concrete slab?

Response 23: Not available, during the demolition contractor to patch and level floor.

Ques�on 24: When is each phase scheduled to happen?

Response 24: Starting day Friday off hour. Contract must submit schedule for approval and

Ques�on 25: Are there any underlayment requirements?

• If yes, what is the thickness required? Manufacturer is not recommending any thickness

Response 25: Follow manufacture sub coating and pre work.

Ques�on 26: Room near to #4618 Neuro/ Ortho O.R. 1 is shown as ge�ng epoxy flooring as per AF100 Floor Layout, but this room is not given under any of the 6 Phases.

• Which phase does this room belong to?

Response 26: Contractor response to submit phase schedule. Critical Surgical Room, Anesthesia

Room, OR#1 Neurosurgery, OR#6 Vascular and Orthopedics and OR#7 Cardiothoracic.

Ques�on 27: As per 3.0 Scope of Work Item 6 - the total resinous flooring qty is 10,500 SF, whereas we are only coming up with 5,645 SF as per our take off.

Seems the drawing scale of 1/8” is NOT correct.

• Clarify and issue a corrected scale drawing so that we can scale the perimeter of each room to calculate quan�ty of flooring and 6” cove base as well or

• Provide SF quan��es of flooring and LF quan�ty of cove base to bid on.

Response 27: As per Scope of Work Item#6 revised to 11,600 SF of Resinous Flooring (Epoxy) This includes control station and holding area. Contractor is to remove vinyl tiles in control station, holding room and gas storage room and prep floor for new epoxy flooring. Attached drawing scale to 3/32”

Ques�on 28: Is the bid to be submited electronically or hand delivered with original copy?

Please clarify.

Response 28: As per Instructions, Conditions and Other Statements to Bidders/Offerors – 36C24224B0007 page 8; No hand-carried or mailed bids will be accepted. All bids shall be emailed to Julie Provenzano, Contract Specialist, at Julie.Provenzano@va.gov by 10:45 am prior to bid opening time at 11:00 am.

Ques�on 29: Should we consider that some type of self-leveling product was used to install the exis�ng flooring?

• If so, the self-leveling product will need to be demolished or removed to expose the concrete surface. This may add an addi�onal day to the demoli�on scope of work for each phase.

Response 29: Concrete surface profile of 3-4. Installation of new floor must follow manufacture guidelines.

Question 30: In order to complete all 6 phases in 10 or less mobiliza�ons, each phase would need to be completed in less than 2 weekends. This does not seem possible due to the manufacturer's process for installa�on and the �me required for demoli�on.

• Will it be possible to u�lize 2 full weekends per phase?
