This announcement constitutes the only solicitation; quotes are being requested, and a written solicitation document will not be issued.

This solicitation is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2021-02. Updated through FAC 2021-02 effective 11/23/20; and Acquisition Policy Flash 21-06 effective 11/16/20.

This solicitation is set-aside for Service-Disabled Veteran-Owned Small Business.

STATEMENT OF WORK

BED RENTAL- VA NEW JERSEY HEALTH CARE SYSTEM

1. SCOPE OF WORK: The Vendor shall provide bed rental equipment to the VA New Jersey Health Care System (VANJHCS), East Orange Campus, 385 Tremont Avenue, East Orange, NJ 07018, and Lyons Campus, 151 Knollcroft Road, Lyons, NJ 07939.

2. PERFORMANCE PERIOD: This is a firm-fixed contract. This contract will be for one base year plus four (4) option years.

3. RENTAL EQUIPMENT SPECIFICATIONS: The following items will be delivered as needed:

a) Pulsation LAL Mattress System CLC USE - Two Modes of Therapy Static LAL & Pulsation LAL, Adjustable Pulsation Cycle Times with Feedback Sensor for Optimal Pressure Settings, Lock Out Controls, Power Failure/Low Pressure Audible and Visual Alarms, Built-in 2"Foam Base for Comfort and Security, Fowler Feature for Seat Inflation when Head is Elevated, Waterproof, Vapor Permeable, Low Friction and Shear Top Cover, Static Mode Diffuses 100 Liters of Air Per Minute, 600 lb. minimum Weight Capacity or equivalent functioning model.

b) Bariatric Bed System - Fully Electric Bed, One Step Lock Brake System & Steer, Cardiac Chair Position, Trendelenburg and Reverse Trendelenburg Functions, Cardiac Chair Position, Integrated Rest Secure System (Bed Exit Alarm/Nurse Call Interface and Scale), Removable 3 Position Side Rails - Up, 45° and Down, Pressure Redistribution Mattress, Head Angle Indicator (V.A.P), Deck Low 15”: Deck High 30", Sleep Deck Width Retractable from 48" to 39", Sleep Lengths: 80", 86” or equivalent functioning model.

c) Bariatric Patient Repositioning System - Repositioning Pulsate Mattress System, Expandable Deck Width 39” – 42”- 48”, 80” & 86” Deck Length, 15” Deck Low 30”, In Bed Scale with 30 Day Weight History, 3 Stage Bed Exit Alarm System, Protocol Timer, Trendelenburg and Reverse Trendelenburg, Anti-Microbial/Anti-Bacterial Head/Foot boards and Side Rails, Multi-Language Display with Voice Recordable Alerts, Cardiac Chair Position, Full Frame Trapeze, Mattress Reposition System 39” Width Expandable to 48”, Push Button Drive Unit allows for 8-10 repositioning events per sheet, Safe Working Load 750 lbs., Positioning Sheets, Two modes of therapy Immersion LAL & Pulsation LAL, Adjustable Pulsation Cycle Times with Feedback Sensor, Lock Out Controls, Power Failure/Low Pressure Audible and Visual Alarms, Built-in 2” Foam Safety Base, CPR Emergency Release, Waterproof, Vapor Permeable, Low Friction and Shear Top Cover or equivalent functioning model.

d) Power Drive Option – Bariatric Bed Power Drive unit with joystick control, forward, reverse and turn controls, with speed control settings and audible alert notification, 1,000 lbs. minimum capacity or equivalent functioning model.

e) Full Frame Trapeze – Bariatric A-Frame Trapeze w/repositioning bar, 850 lbs. minimum capacity or equivalent functioning model.

f) Ultra-Low Bed - Fully Electric Bed, One Step Lock Brake System & Steer, Trendelenburg & Reverse Trendelenburg Functions, Cardiac Chair Position, Electronic Bed Control with Combined Synopsis-Solicitation for Commercial Items:

v) Bariatric Bedside Recliner – Mobile Recliner with Push Bar, High Density Foam Seat and Back, Quick Release, Break-Away Footrest, 30” Seat Width, 750 lb. minimum Weight Capacity.

4. Work in conjunction with Bed Flow Coordinator(s) (a staff member with clinical background) to coordinate rental equipment with inpatient admissions and bed assignments. Assisting with equipment needs and processes for prioritization and decision-making to ensure safe patient care during periods of high demand for rental inpatient beds. Coordinating rental product removal with the discharge of hospital patients.

a) All rental items delivered would be clean, sanitized, and ready for immediate patient use utilizing specified and traceable infection control and quality control guidelines as set forth by JACHO standards for both pre‐delivery inspection (PDI) and pre‐ maintenance inspections (PMI).

5. DELIVERY LOCATIONS:

VA New Jersey Healthcare System, 151 Knollcroft Road, Lyons, NJ 07939 or VA New Jersey Healthcare System, 385 Tremont Avenue, East Orange, NJ 07018.

6. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES:

All Bariatric and Standard equipment must be inspected by Biomed personnel prior to placing in service and must meet FDA compliance and the Department of Veterans Affairs Safe Patient Handling requirement. All bed frames greater than 80” in length must have equivalent size mattress. Mattress covers for low air loss mattresses will be replaced as needed. Vendor shall remove trash and all debris from installation and packing materials. Operator Manuals to be provided to users and Service maintenance as needed on rental equipment. Multiple surfaces to meet various patient needs (Pulse, Alternate, Immersion & Rotation).

a) SCHEDULE FOR DELIVERIES: Beds shall be delivered by the Vendor to all locations within four (4) to twenty-four (24) hours. Vendor will deliver standard and Bariatric beds, Mattresses, Advanced Patient Care products and accessories as requested in the styles and quantities needed. DDDDD

b) SCHEDULE FOR RETURNS: Equipment to be returned will be picked-up in 24 hours unless mutual agreement to hold in event unusual circumstances and delivery will be within same day if request is made by 12 noon. If the request is made after 12 noon, delivery is expected by 12 noon of the following day. Broken equipment must be picked-up as soon as possible.

7. MAINTENANCE: Vendor will provide manufacturer's cleaning instructions and ensure all rental property is cleaned and decontaminated prior to bringing to VANJHCS and must be tagged with name of Vendor for ownership/identification purposes.

8. TRAINING/IN-SERVICE: Two (2) Operator trainings per product category of covered equipment conducted for staff upon request 24/7 five (5) days per week Monday through Friday excluding holidays.

9. HOURS OF WORK: Beds will be delivered on an as needed basis for patients per request of the Wound Care Team or Clinical Coordinators. National holidays observed by the Federal Government are as follows:

If a holiday falls on Sunday, the following Monday will be observed as the National Holiday. When a holiday falls on a Saturday, the preceding Friday is observed as a National Holiday by the U.S. Government agencies. Also included would be any day specifically declared by the President of the United States of America as a National Holiday.

10. IDENTIFICATION/PARKING: The Vendor's employee(s) shall wear visible identification at all times while on the premises of the VANJHCS. It is the responsibility of the Vendor to park in the appropriate designated parking areas. Information on parking is available from the Police and Security Section.

11. PERMITS AND RESPONSIBILITIES: The Vendor shall obtain and maintain all permits, licenses, and appointments required for the execution of work under this contract at no additional cost to the Government. The Vendor shall ensure that all employees receive the mandatory training, as required by OSHA, JACHO, etc., and shall comply with all standards and/or requirements.

12. CONTRACTING OFFICER REPRESENTATIVE (COR): The Contracting Officer reserves the right to designate representatives to act for him/her in furnishing technical guidance and advice or generally monitor the work to be performed under this contract. Such designation will be in writing and will define the scope and limitation of the designee's authority. A copy of the designation shall be furnished to the Vendor.

13. CONDUCT OF PERSONNEL: The Vendor shall be responsible for the performance and conduct of its employees. Personnel employed by the Vendor in the performance of this SOW or any representative of the Vendor entering the VA premises shall be subject to inspections by the Government as deemed necessary.

14. CHANGES TO CONTRACT: The Contracting Officer is the only person authorized to modify the terms and conditions of the requirement once the contract has been awarded.

The contract period of performance is 02/01/2021 – 01/31/2022 with the provision of four (4) option years.

FAR 52.212-1, “Instructions to Offerors–Commercial Items” (JUN 2020), applies to this acquisition. All offers must reference:

4) Technical description of services to be performed in accordance with the Statement of Work. This Technical description should address the requirement specifically. Capability statements that are merely an overview of what the contractor offers will be determined incomplete

5) Terms of any express warranty;

6) Price should be for all services detailed on the pricing schedule of this document. The pricing must be broken down in a manner that allows for price reasonableness to be determined. Failure to provide complete pricing will result in contractors package being determined to be incomplete;

7) “Remit to” address, if different than mailing address;

8) A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3(b) for those representations and certifications that the quoter shall complete electronically);

9) Acknowledgment of Solicitation Amendments;

10) Past performance information, and

11) A statement specifying the extent of agreement with all terms, conditions, and provisions included in the solicitation. Offers that fail to furnish required representations or information or reject the terms and conditions of the solicitation may be excluded from consideration.

(End of Addendum to 52.212-1)

1. The provision at FAR 52.212-2, Evaluation—Commercial Items (OCTOBER 2014), applied to this acquisition.

(a) Award shall be made to the contractor whose quotation offers the best value to the government considering: 1) Technical Capability; (2) Price; and (3) Past Performance.

(1) Technical capability - Contractor shall provide a maximum 10-page capability statement demonstrating their ability to meet all the requirements of the SOW including specifications (make and model) of equipment with brochure.

a. Technical Proficiency: Contractor shall submit a proposal that demonstrates its understanding of the requirement in accordance with the Statement of Work and the extent to which potential risks are identified and mitigated.

b. Technical Experience: Contractor shall supply documentation on 3 – 5 contracts in the past 5 years where they performed work of similar size and complexity.

c. Management Capability: Must include a Point of Contact (both Prime and Sub-Prime) for scheduling purposes, urgent requests and issues, should they arise.

(2) Price: The Government will evaluate the price by adding the total of all line item prices, including all options. The Total Evaluated Price will be that sum.

a. Options: The Government will evaluate quotations for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s).

(3) Past Performance: The offeror shall provide 3-5 instances of past experience of similar scope within the past 5 years. The contractor shall be a firm regularly engaged in the rental of specialty bed frames and surface/mattress equipment for use in healthcare. Individuals with no past performance shall receive a neutral rating for this evaluation factor.

All quoters shall submit the following: one copy of technical capability, pricing, and past performance with an additional redacted copy.

This is an open market Total Small Business Set-Aside for Wastewater Plant Lab analysis services as defined herein. The government intends to award a contract as a result of this combined synopsis/solicitation that will include the terms and conditions set forth herein. To facilitate the award process, all quotes must include a statement regarding the terms and conditions herein as follows:

"The terms and conditions in the solicitation are acceptable to be included in the award document without modification, deletion, or addition."

OR

"The terms and conditions in the solicitation are acceptable to be included in the award document with the exception, deletion, or addition of the following:"

The award will be made to the response most advantageous to the Government.

Responses should contain your best terms, conditions.

Quoters shall list exception(s) and rationale for the exception(s), if any.
