rachel.beason@va.gov rachel.beason@va.gov Air Monitoring – Renovate Building 6, 1st Floor (6A), Canandaigua Project #528A5-17-536 The purpose of this amendment is to provide answers to requests for information received by potential offerors. Q.1. The request was to provide an updated submittal requirement for air monitoring for the following items. The clarification is provided below:

SCOPE OF WORK - Page 3 Submittals:

· Proof of competent person training – VA will consider NYS licensure documentation to meet this required.

· Current fit test – VA maintains this is a requirement

· Certificate of Worker’s Acknowledgement for each worker – VA maintains this is a requirement

· Affidavit of Medical Surveillance and Respiratory Protection for each worker - VA has removed this requirement from the SOW.

· Asbestos Hazard Abatement Plan – VA clarifies – this is the requirement to provide a review of the construction contractor’s abatement plan after it is provided to the Air Monitor by the VA.

· Daily logs (provide example as pre-construction submittal) - VA maintains this is a requirement. The contractor is to submit a clank daily log for government review/approval before abatement start date.

· Documentation of inspection of the regulated area (to be submitted during construction) - VA maintains this is a requirement

· Completion of submittals (02 82 11 section 2.4.3 – to be submitted at construction or phase completion) - VA has removed this requirement from the SOW.

Combined Synopsis/Solicitation - Page 4 Technical Capability:

· Factor 1 – Quality (a) and (b) typically for abatement contractor – VA maintains the components of Factor 1 remain unchanged. Failure of a firm to properly support their technical capability may result in them being considered not technically acceptable or eligible for award.
