VHAPM Part 813.106 Simplified Acquisition Procedures: Soliciting from a Single Source Attachment 1: Single Source Justification for SAP under the SAT

VHAPM Part 813.106 Page 1 of 2 Original Date: 08/30/17 Revision 01 Eff Date: 04/23/2018

DEPARTMENT OF VETERANS AFFAIRS

Justification for Single Source Awards IAW FAR 13.106-1 For

Over Micro-Purchase Threshold but Not Exceeding the SAT ($250K)

Acquisition Plan Action ID: 36C242-19-AP-1359

1. Contracting Activity and Agency:

Department of Veterans Affairs Department of Veterans Affairs Network Contracting Office 2 VA Medical Center 76 Veterans Avenue 400 Fort Hill Avenue Bath, NY 14810 Canandaigua, NY 14424

VISN 2

2237# 528-19-2-5085-0019

2. Brief Description of Supplies/ Services required and the intended use/Estimated Amount:

The VA Medical Center in Canandaigua, NY anticipates a firm fixed price contract to procure 67 new hospital beds with mattresses and accessories as part of a total renovation project of the Community Living Center (Nursing Home).

Estimated Amount: XXXXXXXXXXXX

3. Unique characteristics that limit availability to only one source, with the reason no other supplies or services can be used:

The Canandaigua (NY) VA Medical Center has a Community Living Center spanning over 3 buildings and 5 floors and serves approximately 120 residents. The facility began a bed upgrade in FY18 with the purchase of 40 Span America Encore beds which replaced an older version of the same brand and model. Included in this purchase were bed mattresses and accessories including bed assist handles, side rails, transport dollies, and trapeze bars. These accessories are specific to this model.

Based upon patient needs, bed configurations change on a routine basis. To keep their hospital bed “fleet” standardized in the Community Living Center, and because they maintain an inventory of Encore bed spare parts and accessories available to interchange when necessary (and with little notice), it is in the best interest of the facility to continue to use and purchase Encore Beds from Span America.

This facility is currently undergoing a CARES major construction project. Storage space is extremely limited and the facility simply does not have space to store more than one brand and model of beds, accessories and spare parts. In addition, to meet FDA guidelines, mattresses must be specific to the manufacturer. We must have spare mattresses to match each required configuration. Adding different manufactures only adds to the storage issue.

4. Description of market research conducted and results or statement why it was not conducted:

This purchase concludes the bed upgrade project and it is our conclusion the safest, most cost effective and storage friendly choice is the Span America Encore bed.

5. Contracting Officer's Certification: Purchase is approved in accordance with FAR13.106-1(b). I certify that the foregoing justification is accurate and complete to the best of my knowledge and belief.
