About this file
This document is a Deaerator Tank Condition Report for a Cleaver-Brooks horizontal deaerator tank located at the VA Hospital in West Haven, Connecticut. The inspection, conducted by Industrial Steel & Boiler Services, Inc. and Mistras Group, Inc. on 4/4/25, revealed several critical maintenance requirements. Specifically, the report identified corrosion at the top vessel nozzle connections requiring replacement, cracking and pinholes at the safety valve connection and vacuum breaker, and excessive pitting on the manway surface.
The recommended repairs include cutting out and replacing the corroded top portion of the vessel (approximately 10"x10"x24"), fabricating a new section using original SA-36 grade material, field welding the new section, replacing leaking safety valves and vacuum breakers, eliminating overhead piping, running new lines to a floor drain, and replacing the manway ring and cover plate. Despite these needed repairs, the overall condition of the tank was assessed as good, with minimal pitting and corrosion. The tank, manufactured by Cleaver-Brooks in 1981, has a design temperature of 650°F and a design pressure of 50 MAWP, with serial number 75056 and National Board number 50379.

Summary
Enclosed 3rd Party report
Mistras Group, Inc. Report enclosed –
External/Internal Visual Inspection:
External inspection of the Condensate Tank was performed. The top of vessel (3) nozzle connections is corroded and must be replaced.
An internal inspection of the vessel included the cleaning of all accessible welds. Most of the interior shell was found in good condition. The only exception being the safety valve connection (2) and (1) vacuum breaker. The testing did uncover cracking and pinholes.
Minimal pitting of the balance of vessel was found.
The manway surface has excessive pitting and leaks. This also needs to be cut out and replaced with a new ring and manway cover plate.
Non-Destructive Testing:
UT Thickness testing and inspection of accessible welds did not reveal any cracks or deficiencies. Other than noted above All welds tested were found to meet acceptance standard per code specification ASME SECT V Acceptance Criteria ASME
SECT VIII.
Recommendations:
Cut out the top portion of the vessel with the (3) nozzle connections. 10”x 10” x 24”. (est. final measurements are needed) Shop fabricate and bend a new piece using the same original SA-36 grade material. This will include the needed nozzle connections. Field weld new section into place.
This will require a field visit from our ASME inspector and will also need to include additional testing. Testing can either be hydrotesting the unit or X-ray testing.
Replacing the leaking safety valves and vacuum breaker. Eliminating the overhead piping and running new lines to a floor drain Replace the manway ring and cover plate.
Based on the visual and MT inspection of the unit, the overall condition of the unit appears to be in good condition with minimal pitting and corrosion.
