Tri-annual Electrical Inspections & Testing Services at the Togus VA Maine Medical Center located at 1 VA Ctr, Augusta, ME 04330
Hello VA, AMP personnel, Ryan Echols, Rusty Bennett, Lex Wright, Caleb Hipp and Matt Prombo participated in this project and the onsite work occurred from 1/6/23 to 1/27/23. AMP performed visual mechanical inspections, labeling, photographing, IR scanning and testing required for all equipment listed on the proposed schedule/equipment list based off of one line diagrams provided. Additionally, new replacement or added equipment to the system in various buildings were added as time permitted. The only work that was not completed under this mobilization was the 4-hour test of the Essential Electrical System (EES) and all of the MV switchgear breakers and relays. The 4-hour test of the EES was requested by VA staff to be performed in April during better weather conditions and to eliminate any potential issues that could be encountered during inclement weather. Two team members will be back on site from
4/20/23 to 4/21/23 to complete the EES test and remaining MV switchgear work. The final report will be updated with these findings and provided at that time.
Overall, the electrical system appears to be good condition and being maintained. With all older compounds, inherent issues will be present, simply due to age and environmental conditions.
Below is a list of urgent issues and recommendations and general issues that the AMP team has identified. If any issues were found during the inspections and testing, the AMP and VA Team performed corrective actions if feasible to perform during the outages. It is important to note this section does not include all of the issues, only more urgent and general issues. For a detailed list of all the findings, please review each test sheet further in this report that was created for each piece of equipment inspected. IR scans and oil sampling reports have been added to their respective equipment test sheet. All IR scan photos, regular photos, credentials and report will be provided on a storage device and will be shipped with physical report copies to the VA once AMP is approved to do so.
Urgent Issues & Recommendations
Section 2
Urgent Issues & Recommendations:
1. Bldg. 209 - MV Switchgear - MVSWGR 209: Very dirty and dusty conditions present. Air insulated MV equipment should be cleaned, inspected and exercised on annual basis to prevent tracking and flashovers which are a common cause of medium voltage air insulated equipment failures.
2. Bldg. 210 - MV XFMR - T20: Mortar and construction debris has fallen on top of the transformer and minimal protective barriers were present. Transformer cooling fins that allow insulating oil to circulate and keep the transformer cooled down are very thin.
Falling bricks or mortar could cause damage to transformer and cooling fins. If the fins are damaged, oil will leak out and air will enter the transformer causing premature failure or catastrophic failure will occur. A simple barrier or wooden structure should be built over the top of the transformer to prevent damage.
3. Bldg. 210 - SWBD-Gen-11: Edge guard for load bank conductors penetrating top mounted enclosure was found to be partially installed leaving conductors exposed to sharp edges. Edge guard needs to be installed to prevent a potential flashover. A dead front panel screw was partially screwed into the outer jacket of insulation on a conductor. The team re-routed and terminated the conductor to avoid further damage and potential flashover. Metal shavings and spare hardware found attached to bus presumably from installation. Vacuuming performed and installation hardware removed from physical contact with the energized
4. Bldg. 248 – ATS248: Transfer switch did retransfer as designed but indicator lights were indicating the switch was still on EM power which was not correct. Further troubleshooting of false power source indication will be needed.
5. Building 204 – EMP: Mfr dead front was replaced by custom plate. Thus, mfr. identifying information is not present except as noted. Outer cover securing clamps are in varying states of deteriorated functionality. Removal of dead front was not performed due to condition of installation. AMP was not able to measure the load of conductors with elevated temperature since dead front can't be removed safely. Due to age, condition and safety hazards to personnel, AMP recommends replacement of this distribution panel.
6. Building 207 – Switchboards A,B&C: High levels of humidity in equipment room are causing rust to appear on equipment. VA informed several breakers are defective so that if opened, will not re-close. Recommend replacement due to condition and exceeded useful life expectancy. Recommend installing dehumidifier to reduce humidity in equipment room and prevent future corrosion.
7. Building 249 – Transformer T8: Water leakage, and subsequently corrosion are evident, but not significant. No visible signs of oil leaks. There are (2) minor dents on cooling fins.
West side of transformer pad has accumulated vegetation debris and may enhance corrosion. Bolted connection of neutral bonding strap to cabinet was found very loose.
There are no surge arrestors installed. Per C57.104-2019, Carbon Monoxide gas is in a status 2 condition and recommend investigating the source of activity. Per C57.104-
2019, the transformer is a status 2 state with greater than normal combustible gas level.
Recommend establishing a trend as faults may be present. Duval Pentagon indicates the transformer may have Thermal Faults (> 300 C). Per C57.104-2019, the combustible gas level warrants additional oil testing and analysis. Exercise caution and evaluate transformer load conditions.
8. Building 200 – Transformer T5: Space requirements for installation present challenges for code working space compliance and inspection. Recommend notation for consideration when engineering future upgrades. Additionally, climbing the hill in snowy conditions presented safe access challenges. Similarly, safe access for site personnel could be challenging under adverse weather conditions. It was not possible to determine if fluid accumulation under the unit was water, oil, or both. Low oil level, <25⁰ C, could be indicative of leakage. Per C57.104-2019, Carbon Monoxide gas is in a status 2 condition and recommend investigating the source of activity. Per C57.104-
2019, the transformer is a status 2 state with greater than normal combustible gas level.

9. Building 200E – MSGA Spare Breaker: Spare breaker will not close and breaker should be repaired or refurbished.
General Issues & Recommendations

General Issues & Recommendations:
1. AMP would recommend that the Togus facility consider adopting a yearly maintenance program that would test the equipment over the course of three years versus trying to perform all tri-annual testing in one trip. This would place less stress on facility staff and managers for planning all the outages out and also ease the onsite work efforts required by maintenance staff and contractor personnel. It would also allow the facility to add repairs to the following years trip without having to pay for an additional mobilization.
2. Per NFPA70E, arc flash hazard calculations need to be reviewed every 5 years or whenever there is a significant change in the electrical system. The team found that some equipment had updated labels from 2021, outdated labels from 2014 or no labels.
Overall, a short circuit, coordination study and arc flash study should be conducted for the whole compound. This process will end up generating a complete one-line diagram, verify if all equipment is properly rated for short circuits and also inform employees of appropriate incident energy so proper PPE can be utilized.
3. Install surge arrestors at all MV switches, transformers and LV switchgear and transfer switches. Surge arrestors are inexpensive and will help minimize equipment damage in the event of a lightning strike or voltage spike. The cost to repair or replace equipment typically exceeds the cost of surge arrestors.
4. A significant amount of equipment were found to not be anchored in place. Per NEC
110.13 (A) Mounting. “Electrical equipment shall be firmly secured to the surface on which it is mounted.” The VA Handbook 18-8 Seismic Design Requirements should be reviewed before addressing anchoring including factoring in seismic ratings for this specific location. Without anchoring, even modest activity could cause shifting of equipment. Some equipment could be damaged and not functional in emergency situations like generators. Other equipment that is top heavy could tip over on personnel creating safe hazards.
5. Some equipment has clear working space violations per NEC 110.26 and 110.34. Per
110.26(A)(2) Width of Working Space “In all cases, the workspace shall permit at least a
90 degree opening of equipment doors or hinged panels.” Per NEC 110.26(A)(1) and
110.34(A), there are minimum depth requirements of clear working space in order to allow personnel to have an adequate amount of space to provide servicing and testing of equipment. Several transformers have protective bollards or even landscaping in front of equipment doors that prevent doors from opening to 90 degrees or encroach on the clear working spaces. Relocating bollards, performing landscaping cleanup and trimming can be performed to feasibly eliminate some of the violations.
6. Several distribution panels, switchboards and transfer switches were found to have missing fasteners, hole plugs (KO Fillers) and bushings. Per NEC 110.12(A) Unused
Openings “Unused openings shall be closed to afford protection” Per 300.4(G) Fittings
“Where raceways contain 4 AWG or larger insulated circuit conductors, and these conductors enter a cabinet, box or raceway, the conductor shall be protected.” Missing fasteners, hole plugs and bushings should be installed in these areas which will eliminate the risk of debris or personnel from falling into equipment and prevent unintentional contact with energized parts.
7. Debris from initial and/or subsequent installation, including metal shavings, hole saw cores, stripped conductor insulation, was found in several enclosures and removed.
Recommend final inspection of installation and additional upgrade activities add focus on cleanliness, especially for removal hole saw cores and metal shavings.
8. Several locations had stored material and non-electrical equipment blocking access to installed electrical panels and equipment. This presents multiple fire and access code issues and emphasis should be provided to all staff to discontinue the practice.
9. In a few Distribution Panels conductors were sharply bent beyond allowable per NEC
300.34 Conductor Bending Radius. Though not presently observed, over time, natural hardening of the conductor insulation could cause cracking or splitting of the outer radius of the conductor. Hazards to personnel could then present. If pressed against an enclosure panel, short circuiting could occur. In each circumstance observed, the conductors were bent, arranged and restrained in a way to prevent enclosure contact, so no imminent issues were observed which would require immediate correction. We recommend future acceptance inspection ensure conductor minimum radius be maintained. We further recommend that maintenance personnel entering enclosures be informed and trained to look for these concerns to avoid potential contact with exposed conductor.
10. It was found that most of the transformers were under vacuum which could allow moisture and contamination to enter the transformer if a seal leak were to occur or while pulling oil samples. A fitting, pressure gauge and nitrogen blanket as applicable should be installed in the transformer which would place each transformer under positive pressure and for facility staff to monitor if any transformer seal leaks were to occur.
11. Develop a plan for replacing, retrofitting or refurbishing all equipment inspected and found to be past its useful life expectancy. Most notably, focus should be placed on equipment inspected and tested for all quarters equipment, building 204 EMP, building
207 switchboards A, B and C, building 248 EMDP, building 200 MV SWGR 200, MSGA &
MSGB. Equipment inspected in these locations have exceeded their useful life expectancy per IEEE 493 Gold book (shown below). These areas in particular, appear to be the most dilapidated and should have the highest priority for replacement, repair or retrofitting. It is important to note that age is a significant factor in determining useful life expectancy, but there are other factors that need to be considered when determining. For example: what site conditions is the equipment exposed to i.e. rain, snow, humidity, etc.; Has annual maintenance and inspections been performed; Is the equipment overloaded; Is the equipment load balanced; Has the equipment experienced any faults or short circuits.
