A list of authorized prosthetics purchasing staff will be issued to the Contractor at time of award.

1.2.2 Fuel Surcharges. Fuel surcharges shall not be added to any invoice or contract prices.

1.3 BILLING PROCEDURES:

For Home Oxygen: Billing will be on a monthly basis, within ten (10) days after the end of month. Separate obligations will be issued; one for solely services provided and another for solely supplies. Contractors shall separate and track all labor vs. supply. All bills shall be properly invoiced and contain, at minimum: an itemized listing of services rendered (including number of specific items, pounds of liquid, type of container – H, M, B/C/D/E, etc.), dates of service, charges for each service, Veteran patient’s name and social security number, address, month of service, and total of bill. The Contractor will provide written documentation for services provided to the Veteran patient with each billing.

In an Excel format, the monthly billing format is the following:

For DME: A purchase order is generated and provided to the Contractor for the requested service. Upon completion of the requested service, the Contractor will charge the appropriate purchase card and provide an invoice to the VAMC Prosthetics Service within 24 hours/one business day of the date of service preferably electronically or on paper. Proof of delivery/ completion of service will be forwarded to the ordering VAMC Prosthetics Service at the time that the purchase card is processed for payment. An invoice will be provided and will contain, at minimum: an itemized listing of services rendered (including number of specific items, etc.), dates of service, charges for each service with the purchase order numbers for each service, contract number, Veteran patient’s name and social security number, address, month of service, total of bill, a copy of the simplified purchase order, appropriate HCPCSS Codes, and proof of Veteran’s acceptance of service delivery/pick-up, the model and serial number of the equipment delivered and/or picked up and whether the equipment is new or used.

Proof of Delivery. As a condition of payment, the Contractor must provide with the initial invoice for equipment delivered, a receipt bearing the signature of the VA beneficiary, family member, or legal custodian and date of delivery. Subsequent monthly invoices shall list names for the invoice period, with a certification signed by the Contractor stating that the equipment was in place for use by or services were delivered to the listed VA beneficiaries for the invoice period. Invoice for oxygen cylinders delivered or liquid refills made must be accompanied by a receipt bearing the signature of the VA beneficiary or family member and date of delivery. Payments made by the VAMC under this contract shall constitute the total cost of services provided by the contractor. The Contractor shall not seek reimbursement from the beneficiary’s family, private insurer, Medicare, or any other entity acting on behalf of the beneficiary.

1.3.1 Contacting and Tracking new starts:

1.3.1.1 A minimum of three attempts will be made to contact the Veteran/his-her family to coordinate the delivery within the contract timeframe. If the Contractor is unable to reach the Veteran or family after three attempts, the Contractor is to contact the VAMC for an alternate phone number and order disposition. The VAMC retains the right to request the Contractor to continue additional attempts, depending on the individual Veteran’s circumstances and/or cancel the order.

1.3.1.2 All Oxygen & DME patients will be tracked on a weekly basis via a new start list generated by the Contractor and provided to the VAMC COR’s by the Tuesday of the following week. The document will be updated weekly and all patient information from previous weeks will be maintained. This will be provided to the VA CORs through PKI encryption. If the contractor uses another form of e-mail encryption and requests its use, it must be user-friendly in all e-mail communications with the VA. The VA Contracting Officer will determine if the alternate encryption method requires additional costs and time for VA staff and will approve or disapprove its use prior to implementation.

1.3.2 If oxygen equipment in use on any VA beneficiary is changed upon request of the CORs, the cost of these systems will be pro-rated for the time installed.

1.3.3 When the CORs discover that the VA oxygen beneficiary under this contract has been admitted to a hospital, the Contractor will be notified if a discontinue order is indicated; not all admissions will require Contractor notification. At that time, the removal of the oxygen equipment from the VA beneficiary’s place of residence will be determined by the VA provider - a VA consult may not be generated or a consult to change/modify/discontinue services may be initiated. Each patient admission and discharge are unique.

1.3.4 When applicable, the CORs/VAMC Clinical Lead will notify the Contractor in advance of discharge and equipment will be re-delivered and services resumed as ordered by the CORs/VAMC Clinical Lead.

1.3.5 If the Contractor learns that a VA beneficiary has expired or moves before notification by the VA, the Contractor is responsible for notifying the COR. Charges will be pro-rated for the time period involved. This will also apply to services that are discontinued before the end of any month.

B.5 STATEMENT OF WORK

III. HOME OXYGEN/RESPIRATORY CARE SERVICES

STATEMENT OF WORK: VISN 1 – VA New England Healthcare System has an ongoing requirement for Home Respiratory Care services to Veteran patients served by its eight medical centers and its associated outpatient clinics. Offeror shall ensure that in its proposal all transportation, labor, fuel, parts, ancillary supplies and supervision are inclusive in its offer. The total number of home respiratory care patients at each VA site is subject to change. The Home Respiratory Care Program, Pulmonary Section, Medical Service, at all VISN 1 facilities admits patients into the Home Respiratory Care Program after performing assessment of their clinical condition. The offeror will furnish home oxygen and respiratory equipment, conduct in-home pulmonary assessments and re-assessments by Registered Respiratory Therapists, set-up/complete overnight oximetries and provide documentation of patient education. The offeror will transact business only with the appropriate officials of VISN 1, VA New England Healthcare System, Department of Veterans Affairs, and in no case solicit services from VA beneficiaries, except to contact the VA beneficiary for delivery and/or pick up arrangements as ordered by the VAMC.

3.1. From time-to-time, the number of Veterans being served by this agreement will change. VISN 1 currently serves an estimated 3,923 Home Oxygen patients and 27,491 CPAP/BiPAP/ ASV/APAP respiratory care patients (Attachment B – several patients are in the Facility VAMC PAP Clinics/non-vendor). The volumes or amounts shown in the solicitation are estimates only and impose no obligation on the VA either minimum or maximum. The resulting contract shall be for the actual requirements of the VA as ordered by the VA during the life of the contract. The Contracting Officer’s Representative (COR) or designee will provide the Contractor with notification to initiate individual beneficiary service requirements, including the beneficiary’s oxygen prescription, equipment, supplies and services to be provided including date and place of delivery, the Contractor will confirm receipt of the request within two working hours of notification; the COR or designee will also provide notification if VA owned equipment is provided; the COR or designee will also provide notification of VA initiated discontinuance of service. The notification may be made orally but will be confirmed in writing within two (2) working days. The written documents will include the authorizing official’s signature.

Standards of Performance:

3.2. Veterans with a prescription for an oxygen concentrator shall be provided with an appropriate backup system consisting of a compressed gas source and regulator with stand or a supply to cover – at a minimum – four times the travel time to the patient’s home from the distribution site, humidifiers, all disposable supplies (e.g. cannulas/mask, tubing, connectors, etc.) for use during the event of a power failure or mechanical problem and necessitating the use of the backup cylinder. (During seasonal times when power failures increase – such as during intense winter/snowstorm and summer/hurricane storms and/or areas of New England that have weak power grids, the Contractor is responsible for providing additional back-up oxygen to those patients to cover up to 3-days of back-up oxygen – at no cost to the VA.) The minimum supply will consider each patient’s prescription flow-rate and distance from the Contractors location, and so actual supply levels may differ from patient to patient, as long as each patient has oxygen supply to last a minimum of four times the Contractors’ response time. The cost for the above-mentioned backup system would be borne by the Contractor. The cost of replacement of equipment damaged, lost or misplaced not through beneficiary’s negligence shall be borne by the Contractor. Documentation is required if the patient declines the oxygen back-up system.

Disposable Supplies – VA is not required to purchase these supplies from the Contractor. Any required, mandated VA/federal contracts or supply arrangements through the VA DLC/Denver Logistics Center will take priority over VISN supply agreements.

3.3. In accordance with the prescription received from the VAMC, the Contractor shall provide the beneficiary with the following items:

3.3.1 Humidifiers and water traps when deemed necessary. The number of humidifiers must be sufficient to ensure that they are not depleted between re-supply visits. The number of traps will be based on the patient’s flow rate.

3.3.2 Disposable oxygen cannulas on hand for a change every two weeks with a supply in reserve at all times until the Contractor’s next scheduled visit.

3.3.3 Disposable masks for change every two weeks with a supply in reserve at all times (when masks are prescribed in lieu of nasal cannulas).

3.3.4 50 ft.+ of oxygen connecting tubing shall be replaced every 3 months with an equivalent section of tubing in reserve at all times (to be included in initial setup) until the Contractor’s next scheduled visit.

3.3.5 Trach Collar and accessories (e.g. tubing and large volume nebulizer) with a supply on hand allowing for change every three (3) days and a supply in reserve at all times until the Contractors next scheduled visit.

3.3.6 The Contractor will provide and install two fire safe cannula valves (also known as thermal fuses) during the oxygen set-up on the cannula near the patient and near the oxygen device for all new and existing oxygen patients and every 6 months at minimum thereafter (unless prescribed at a different time interval). Two thermal fuses will also be installed on each and every stationary and portable oxygen delivery system and cannula in the patient’s possession and future devices delivered to the home.
