About this file

This is a Request for Information (RFI) from the VA's Digital Health Office seeking information about medical calculator software capabilities to be integrated into their Clinical Decision Support (CDS) platform. The RFI (solicitation #36C10G25Q0040) requests details about Software as a Service (SaaS) products that can integrate CDS calculators and risk assessments with underlying Electronic Health Records (EHRs) to provide VA clinicians with point-of-care access to improve care quality and safety.

Key requirements include cloud-based delivery requiring no VA software downloads, FedRAMP authorization, ability to read patient data through FHIR interfaces, and options for either SaaS, managed services within VA datacenters, or microservices architecture implementation. The anticipated period of performance is one base year plus four option years, with work expected to begin in Q3 FY25. Responses are due by January 31, 2025 to William Milline and Cristine Carmody via email. The opportunity falls under NAICS code 518210 and PSC 7420. The VA notes that set-aside decisions may be made based on RFI responses, and vendors must provide details about their socioeconomic status, existing contract vehicles, FedRAMP status, pricing models, service level agreements, and technical capabilities in their responses.

Digital Health Office

Medical Calculators

Office of the Chief Technology Officer (OCTO) and the Office of Health Informatics (OHI) is a program within the Veterans Health Administration (VHA) Digital Health Office (DHO) that have developed and implemented CDS console to host clinical applications with the capability of connecting directly to the electronic health record (EHR) and other data systems capabilities to support Veterans, their families, survivors, and caregivers.

The program serves clinicians by creating a suite of context-aware, standards based Clinical Decision Support (CDS) tools that provide a common, cohesive user experience and integrate with CPRS as well as Oracle Health. VA has internally developed CDS applications, on-boarded Government Off-the-Shelf (GOTS) and looks to incorporate Commercial Off-the-Shelf (COTS) applications in the future for use in this platform.

The purpose of this RFI is to seek information from interested and capable firms who can assist the Government to integrate and implement a third-party CDS application directly into clinical workflows using the CDS Console. The expected results of this project are improved Veteran clinical outcomes and improved quality of life. As well as job satisfaction for providers using VA health information systems to ensure Veteran health and well-being.

Any responding contractor may respond to any number of questions posed in this RFI, and a submission does not mandate a response to each question asked. Any and all socioeconomic categories are encouraged to respond.

Expected time frame to begin work: Q3 FY25 Expected Period of Performance: One Year base, plus four optional year periods

(dependent on quality of responses and agency need)

The North American Industry Classification System (NAICS) for this requirement is 518210.

Be advised that set-aside decisions may be made based on the information provided in response to this RFI. Responses should be as complete and informative as possible.

Scope of Work – High Level Overview

The details of the scope of work can be found within the PWS; however, below is an overview of the primary objectives to be met:

The primary goal is to integrate CDS calculators and risk assessments with underlying EHRs and existing applications and grant access to all VA clinicians at the point of care in order to increase care quality and safety and enhance clinician efficiency.

The CDS Calculator application will read patient demographic and clinical data from the

EHR utilizing existing FHIR interfaces. EHR data will be used pre-populate relevant input fields to provide medical calculations. Initially, results will just be displayed to clinicians but will not automatically be stored or written back to the patient record in VistA through an API, though this functionality may be implemented in the future. Within the application, numerous clinical calculations and risk assessments will be supported.

The vendor will collaborate with VA OI&T to provide information necessary for the VA to integrate the CDS Clinical Calculator application within the VA CDS Platform.

Questions to be answered in response to this RFI:

i. Name of Company

ii. Address

iii. Point of Contact

iv. Phone Number

v. Fax Number

vi. Email address

vii. Company Business Size and Status

1. For VOSB and SDVOSBs, proof of verification in VetCert.

viii. NAICS code(s)

ix. Socioeconomic data

x. SAM UEI Number

b. Provide a detailed summary of your capability to meet all the requirements contained within the draft PWS, and for the following areas:

1. Detail your company’s capabilities and experience supporting federal agencies in providing a Software as a Service (SaaS) product, and how your SaaS product can satisfy the requirements listed in the PWS. Include examples of federal experience implementing your SaaS product (specific examples or references provided must include the agency, point of contact, dollar value, and contract number).

2. Detail your company’s SaaS product cloud infrastructure and accessibility from various client devices through either a thin client interface, such as a web browser (e.g., web-based email) or a program interface. The system shall not require download of software to VA computers.

The VA shall not manage or control the underlying cloud infrastructure including network, servers, operating systems, storage, or individual application capabilities. The system shall be vendor or third-party hosted and require no servers within VA. Please validate that your proposed SaaS product is SaaS in accordance with the NIST 800-145 definition and describe its hosting environment.

3. Please detail any interfacing or custom development that your SaaS product requires ahead of go-live. Can your company’s proposed SaaS product meet all PWS requirements without developing an interface or performing any kind of custom development?

4. Please describe your company’s willingness and ability to adopt and incorporate customer feedback in the product roadmap of your proposed SaaS product.

Please include details on how your company ensures that the Microservices architecture and containerization practices meet industry standards and best practices while also demonstrating the ability to scale and optimize microservices within containers to meet the evolving needs of the VA. Scalability should be achieved without compromising the system's stability, responsiveness, or security.

7. Please describe your pricing model to include the threshold for named user subscriptions vs.

an enterprise/site subscription. Is your pricing model driven by total users, concurrent users, transaction-based, number of records, etc? Are any additional services needed outside of the subscription model (e.g., configuration to meet functional requirements, data storage, data migration, additional features)? If so, please provide the items and proposed price for these additional services.

8. Please provide separate Rough Orders of Magnitude (ROMs) for the SaaS product in accordance with PWS.

9. For the SaaS portion of this effort, VA intends on providing limited payment until the SaaS product has achieved a FedRAMP Authorization and a VA Authority to Operate (ATO).

Once approved by the VA Authorizing Official, VA intends on setting up a subscription payment for the duration of the base period and option periods, if exercised.

a. Do you concur or propose different payment terms?

10. The Contractor shall comply with FedRAMP requirements as mandated by Federal laws and policies, including making available any documentation, physical access, and logical access needed to support this requirement.

a. Is your company’s SaaS product FedRAMP Authorized?

11. Following guidance from the Federal CIO, VA will utilize existing JAB ATO or agency ATO issued by another agency as a starting point for FedRAMP requirements. If neither of those exist, VA will sponsor FedRAMP ATO. VA will be using the FedRAMP baselines as a starting point, since they are specifically tailored for cloud services. Does your company’s SaaS product have an ATO from any federal agency?

12. Detail your company’s support Service Level Agreements (SLAs) for all tiers of service.

13. What was your uptime rate during the past 12 months?

14. Describe your standard release and patch cycle.

15. Has the draft PWS provided sufficient detail to describe the technical/functional requirements that encompass the software development and production operations support services to be performed under this effort.

______ YES _______ NO (if No, answer the next question)

16. If “NO”, please provide your technical comments/recommendations on elements of the draft PWS that may contribute to a more accurate proposal submission and efficient, cost-effective effort.
