Questions and Answers. Government answers are highlighted in Blue.

1. Order Frequency - Do we know how frequently orders will be placed, i.e. once a month, bi-monthly, etc? And will each order include multiple shipping locations? No, but typically orders are made by the individual station on as-needed bases. Individual stations may order multiple items in September and shipped to each separate location.

1. Quantity Forecasting - Orders will be expected to be filled within 30-days. We will be able to manage inventory much more efficiently if it is possible for the VA to forecast upcoming quantities needed as far out as 60-days (or more) in advance, as an example. Will it be possible for the VA to offer any forecasting for order fulfillment in advance of orders? Awardee may work with their Contracting Officer Representative (COR) to see if estimates/ forecasting is available. They will be able to provide a list of all estimates, which include sites and month/year data.

1. Fixed Firm Pricing - Please define “Fixed Firm Pricing” and whether or not this should include shipping costs to each individual location on your list. A Firm-Fixed-Price (FFP) (FAR Subpart 16.2) contract provides for a price that is not subject to any adjustment on the basis of the contractor’s cost experience in performing the contract. This contract type places upon the contractor maximum risk and full responsibility for all costs and resulting profit or loss. Shipping is to be included in your pricing. See Attachment A- Price List.

1. Fixed Firm Pricing - Is “Firm Fixed Pricing” based on year-over-year pricing that allows for incremental annual price increases? No Or are you wanting the same pricing for each machine for years 1-5? No, you may quote at your discretion. If you awarded the contract, then your quoted pricing is considered “Firm-Fixed Pricing” and will not be subject to additional price evaluations.

1. Inflation/Cost Increases - Will there be language in the contract that protects the supplying contractor/manufacturer throughout the five-year term of the contract in the event of higher year-over-year inflation in raw materials, labor and logistics that are outside the margins of normal long-term average inflation rates like we have experienced the past couple of years? Such language could perhaps include an opportunity to re-evaluate pricing in the event of another surge in inflation. While we certainly understand the current economic market we are in, currently there are no provisions in place outside of vendors using the Producer Price Index(PPI) for option years. Price evaluations will only be completed before contract award for Base Year and all Option Year Pricing. The Government is currently looking into options/provisions for future NX Solicitations and Contracts.

1. Shipping/Logistics - If shipping is to be included, will all deliveries to all locations be considered “dock to dock” only? Most of our medical facilities and community-based outpatient clinics have a dock, but there may be a few that do not have access to one, and delivery is to the door.

1. Shipping/Logistics - Are there any additional shipping/delivery special requirements that could cause potential delays or additional expenses outside the scope of typical LTL dock to dock deliveries? These would include but are not limited to inspection points prior to delivery to ultimate destination, inside delivery, assembly, trash removal, etc. for all years of the contract? If so, please describe in detail. Although this is not an inclusive list, a facility may be located on a site that requires additional permissions to enter. Delivery may require a dolly if not dock to dock. Assembly may be in tight quarters, and trash removal may be distant in some locations. Installation is based on the manufacturers warranty requirements.

1. Maintenance or Service Plans - Outside the original manufacturers warranty, are you requesting any additional maintenance, service or parts plans to be included? Not at this time, but if you would like to have it in your proposal, that is at your discretion.

1. Payments - Can you confirm that all payments for years 1-5 will be made via Electronic Funds Transfer (EFT)? Yes.

1. Will this be to the continental US or does it include Alaska, Hawaii and other parts of the country like Puerto Rico. It’s only for the Continental US. Any orders for those locations (OCONUS) will be FOB Destination to Florida and California.

1. It seems like it would be very hard to quote prices on freight when we do not know where they are going and/or how many will be shipped. Are you wanting us to make an estimated guess? To do so, we would have to figure out how much the shipping for the farthest location possible and then add inflation over the years. This would cost the VA much more than going by the price of each order. If allowed to say that the freight is figured upon each order, we would not increase the price of shipping whatsoever and provide the Bill of Lading and any shipping documents showing the cost from the shipper is what the VA is being charged, no profit to us.. This is a Firmed Fixed Price contract that includes shipping and the Government understands that you must price accordingly.

1. Kim, from SportsArt, informed me that they had a previous NX EQ with the VA but it was a short contract. She told me that the way that freight was quoted was that it was determined upon each order, since some orders need larger trucks, lift gates etc. and that is how she is telling me to quote the shipping. I just wanted to clarify if this is the way that it should be done on this, or if you need me to get SportsArt to figure something different out. Yes, orders will be placed by individual medical centers throughout the US and the quantity can/may vary. Freight/ delivery and all associated costs shall be included in your quote. There is a section for ARO and Pricing on the Attachment A- Price List. We understand the current economic market we are living in, but please try to quote your best ARO. Shipping pricing should be included in your quote and not priced separately for this RFQ.

1. For this solicitation, are the requirements of being a medical grade treadmill meeting microamp leakage specifications, offering bi-directional belt movement (user could just turn around on a belt that just moves forward), and starting at .1 mph absolute musts? Yes, SC 1 for medical grade physical therapy is a must, and SC 2 when patients first turn on the treadmill for safety reasons is also a must be required.

1. Do you need pricing for both the T655MS and T655MD? Yes. See Attachment A- Price List.

1. What is the expected total purchases of each of the models in the 1st contract year? See estimated quantities on Attachment A- Price list. Awardee may work with their Contracting Officer Representative (COR) to see if estimates/ forecasting is available. They will be able to provide a list of all estimates, which include sites and month/year data.

1. Once awarded can contract pricing be adjusted after the 1st year or is pricing fixed for the entire contract period? The pricing is Fixed and will not be changed or subject to additional price evaluations.

1. Does freight need to be included in the price of each model or can it be priced separately based on ship to locations? Shipping must be included in pricing and cannot be priced separately.

1. Is there a minimum order number for each model during the contract period? No.

1. Will facilities require onsite training or installation once purchased? Training or installation was not required. If you would like to add it to your proposal, that is at your discretion. Please be aware that outside training or installation must follow the manufacturer's warranty requirements.

1. Based on the "Price List" attachment we noticed a quantity count was listed for each model. Are these quantities considered the minimum or maximum amount ordered per year? No. Those are yearly estimates.
