About this file

This is a Cost Proposal instruction document for the Drug and Alcohol Testing, Products and Services contract under the MMCAP Infuse consortium. The proposal requires vendors to submit pricing across five Excel workbook sheets covering Testing Services, DOT Testing, Fees and Additional Services, Products, and Discounts. Testing Services encompasses non-DOT regulated panels including breath alcohol, urine panels (5-14 panel options), hair panels (5-14 panel options), and saliva/oral fluid panels (5-14 panel options). DOT Testing covers DOT-regulated urine 5-panel, saliva/oral fluid 5-panel, alcohol breath testing, and alcohol saliva/oral fluid testing. Fees and Additional Services include MRO services, regulatory policy review, legal consultation, technical expert testimony, litigation assistance, reasonable suspicion training, mock audits, Substance Abuse Professional services, Employee Assistance Program services, background checks, correctional system treatment and counseling, HIPAA secure records control, data access, DER training, and random pool management. Products listed include alcohol testing devices (Alco-Screen, Q.E.D., AlcoMate), urine drug screen cups (7-15 panel options), rapid oral fluid drug tests (5-6 panel options), and harm reduction products (fentanyl, xylazine, and benzodiazepine test strips). All pricing must be rounded to the nearest penny, exclude sales and excise tax, and remain fixed for the first contract year with annual update options thereafter.

Vendors must provide both in-network and out-of-network pricing for testing services and fees, with identical pricing acceptable if no cost differentiation exists. Products must include selling unit of measure, manufacturer information, manufacturer part number, vendor item ID/SKU, FDA approval status, and CLIA waiver status. All yellow highlighted fields require completion with "N/A" for unavailable services or products and "0.00" for no-cost offerings. Additional services and products beyond those listed may be added using "ADD" designations without modifying existing line numbers. The proposal instructions emphasize that MMCAP Infuse reserves the right to award services based on member value and that additional products and services may be added upon mutual agreement during the contract term. Vendors are encouraged to include volume, prompt payment, or other member discounts with qualification requirements and percentage discounts specified on the Discounts sheet.

Instructions

Drug and Alcohol Testing, Products and Services Cost Proposal (Section 4): Instructions
All products and services you intend to offer to MMCAP Infuse Members must be listed along with their associated pricing within the sheets of this Excel workbook, including "Testing Services", "DOT Testing", "Fees and Additional Services", "Products", and "Discounts". Additional sheets/tabs should not be added and will not be scored if provided. Proposed pricing must be available to all MMCAP Infuse members. Pricing should always be rounded to the nearest penny and should not include sales or excise tax. All yellow highlighted fields should be populated for numbered lines across all tabs. If there is a service or product that you do not offer, populate any associated fields with "N/A" to represent that this is not applicable. If there is a service or product that you offer, but would be available at no cost to our Members, populate all fields and use "0.00" as the proposed price. Pricing will remain fixed for the first year of the Contract, with the option to update pricing annually. Products and services referenced in this workbook may not be all inclusive, and you are encouraged to include additional lines to represent other services or products you're also able to provide to our Members. When adding additional lines, do not modify existing "Line Number"s on each sheet, instead use "ADD" for each newly added line within the "Line Number" column. If an item or service is not included within this Cost Proposal, awarded Vendors may add additional products and/or services upon mutual agreement with MMCAP Infuse during the term of any resulting agreements. Please be sure to also review the detailed instructions included on each sheet to ensure accuracy.

Products

Drug and Alcohol Testing, Products and Services Cost Proposal (Section 4): Products
This sheet is intended for the proposed pricing on drug and alcohol testing products to be shipped to MMCAP Infuse Members. If not pre-populated, please complete fields for "Selling Unit of Measure (SUOM)", "Manufacturer Name", and "Manufacturer Part Number". Please also complete "Item lD/SKU", "Proposed Price", "FDA Approved (Y/N)", "CLIA Waived (Y/N)", and if applicable, "Additional Details". Some lines on this sheet refer to specific products, while others describe testing products that may have various sources. For testing products that don't have a defined SUOM, quantity per SUOM, manufacturer name, or manufacturer part number, please complete these fields with the information pertinent to a product you'd be able to offer Members that matches the description provided, being sure to match test type, panel size, and included drug tests as applicable. "Selling Unit of Measure (SUOM)" should describe how Members would be able to purchase this product, and it will likely be represented as one of three options: "Each (EA)", "Box (BX)", or "Case (CS)". "Quantity per SUOM" should be entered as a whole number and represent the number of tests, units or eaches included when this product is purchased. "Manufacturer Name" will reflect the name of the manufacturer for the product and may be your organization's name if self-manufactured or privately labeled. "Manufacturer Part Number" should represent the unique code used by the manufacturer to identify this item, and "Item ID/SKU" should represent the unique code you use to identify this item/product that would be visible to Members on quotes, invoices, etc. "Proposed Price" should represent the price to the Member for each product based on its SUOM. (A unit price will be automatically calculated in column J by dividing the proposed price by the quantity per SUOM. Cells in this column should not be modified.) For each product you're able to offer, please also mark in columns K and L if the product is FDA approved and/or CLIA waived by entering a "Y" for yes, or "N" for no. The "Additional Details" column should at minimum be used to include any adulterant checks included as part of the testing product, as well as any cutoff sensitivities for individual drugs tested with the product, but may also be used to provide additional context to the product or pricing if needed. Please add lines as needed, including those for any products referenced in question #34 of the Technical Proposal. Please also reference the "Instructions" sheet for additional directions that may be applicable.
