About this file

This document is a Request for Statement of Qualifications (RFSQ) issued by the State of Utah Division of Purchasing for Government Employees Emergency Preparedness Items, solicitation #NS18022. The document seeks to establish an open-ended approved vendor list for emergency preparedness products, including 72-hour emergency kits, first aid kits, and other emergency preparedness items. The solicitation opened on 10/10/2017 and closed on 10/20/2017, with the intent to create a 12-month vendor list that can be periodically reopened. Two vendors, Ready Store LLC and Sunset Survival & First Aid, Inc., were awarded as qualified vendors.

The solicitation requires vendors to offer new products at a percentage discount from their catalog or price list, with shipping and handling costs included in the pricing. Vendors must provide a searchable catalog, have a local authorized representative available during business hours, offer a warranty for product defects, and comply with various technical criteria including demonstrating experience with emergency preparedness kits, providing a customer service plan, and describing unique abilities such as FEMA compliance and accommodating special needs. The procurement is specifically for government employees to purchase emergency preparedness items, with vendors' products to be identified on the state purchasing website for easy access and review.
