About this file

This is a Vendor's Statement form pursuant to Kentucky Revised Statutes (KRS) 45A.343, required by the City of Owensboro, Kentucky for a demolition contract opportunity. The statement serves as a compliance certification document that vendors must complete and submit as part of their bid for the demolition of one structure. The form requires vendors to disclose any violations of specified Kentucky statutes within the five-year period preceding the statement and to acknowledge their commitment to maintaining compliance throughout the contract duration. The contract involves the complete demolition, removal, and disposal of all structures and debris in accordance with applicable local, state, and federal requirements.

The vendor certification addresses compliance with Kentucky Revised Statutes covering corporate taxes (KRS 136), sales and use taxes (KRS 139), income taxes (KRS 141), wage and hour laws (KRS 337), occupational safety (KRS 338), unemployment (KRS 341), and workers' compensation (KRS 342). Vendors are required to certify that they have not been finally determined to have violated any of these provisions within the preceding five years and agree to maintain compliance for the duration of the contract. Non-disclosure of violations or failure to comply with applicable statutes provides grounds for the City of Owensboro to cancel the contract and disqualify the vendor from future City contracts for a period of two years. The form requires signatures from a duly authorized officer of the bidding company, including the company name, authorized representative name, title, and execution date.
