About this file

This is a sworn statement form required pursuant to Kentucky Revised Statute (KRS) 45A.395 for the City of Owensboro's demolition project in Kentucky. The statement serves as a prerequisite certification that bidders and offerors must submit to be considered responsible bidders. Bidders are required to certify under penalty of perjury that neither they nor, to the best of their knowledge, anyone acting on their behalf has knowingly violated any provision of Kentucky's campaign finance laws, and that awarding a contract to the bidder will not violate such laws. The form requires the bidder or offeror to complete fields identifying the signatory's office or title, the company name, signature, and the date of execution in the format of day, month, and year.

The statement defines "knowingly" as conduct undertaken with awareness or that should have been met with awareness that such conduct violates applicable law or that a described circumstance exists. This certification is a mandatory compliance requirement for participation in the City of Owensboro's competitive bidding process and must be executed and submitted as part of the proposal package to establish bidder responsibility and eligibility for contract award consideration.
