About this file

This Scope of Work document, prepared by the State of Utah Division of Wildlife Resources in conjunction with the State of Utah Division of Purchasing, outlines requirements for a 5-year contract to supply formaldehyde for fish hatchery operations. The contract calls for the delivery of up to 150 55-gallon barrels of therapeutic grade 37% formaldehyde that is FDA-approved for use with fish and eggs. The state currently uses approximately 30 barrels annually and anticipates consuming up to 150 barrels over the contract period. Deliveries must be scheduled between April 15 and October 1 each year to prevent exposure to temperatures below 40 degrees Fahrenheit during shipping. Products will be delivered to either Mantua Fish Hatchery located at 555 E Fish Hatchery Road, Mantua, UT 84324, or Glenwood Fish Hatchery at 542 S Hatchery Road, Glenwood, UT 84730. The contract award will be based on the lowest price per barrel inclusive of shipping costs.

The contract is structured as an Invitation for Bid (IFB) with award anticipated to a single responsive and responsible vendor offering the lowest cost proposal. Vendors must guarantee their pricing for a minimum of one year. Recognized product labels acceptable for this procurement include Parasite-S, Formalin-F, and Parasite-S. The formaldehyde serves a critical function in hatchery operations by preventing fungus development on eggs and fish, which is essential to the state's ability to supply fish for Utah anglers. No specific response dates, site visit requirements, or bidders' meetings are identified in the scope of work documentation provided.

Attachment: Scope of Work

Introduction Formaldehyde is needed in hatcheries as a disinfectant. It is required to treat eggs to prevent fungus from developing on eggs. On average we use about 30 55-gallon drums of

Formaldehyde a year. Over a 5-year period the division anticipates it will use up to 150 drums.

The Formaldehyde must be 37% therapeutic grade and FDA approved for use with fish and eggs. We will order Formaldehyde to ensure it is delivered between April 15th and October 1 to prevent freezing during shipping.

Background Formaldehyde is used in hatcheries to prevent the formation of fungus on eggs and fish.

Without this chemical it would be almost impossible to provide the eggs to provide the fish needed for Utah anglers.

Scope of Work To deliver as needed in 55-gallon barrels of therapeutic grade 37% formaldehyde that is FDA approved for use with eggs and fish. Some of the recognized labels are Parasite-S, Formalin-F and Parasite-S. The delivery date will be set - up to avoid the possibility of chemicals being exposed to sub-40 F temperatures. In general, yearly orders will be placed to allow delivery between April 15th and October 1. Delivery will likely be to Mantua or Glenwood Hatcheries’ addresses listed below. We currently use about 30 barrels a year, so we anticipate needing up to 150 barrels over a 5-year period. The contract will be awarded based on price/barrel, with shipping included.
