About this file

This is a Vendor Questions Worksheet for RFP 26-1742 issued by the Michigan Department of Natural Resources for a Muskegon SGA-MARSH MASTER procurement per DNR specifications. The worksheet is a comprehensive questionnaire that bidders must complete as part of their proposal submission and serves as a standardized tool to collect essential information from vendors interested in contracting with the State of Michigan.

The worksheet requires bidders to provide detailed responses across multiple categories including contact information for the bidder's sole RFP contact and contract signatory, company background information such as SIGMA Vendor ID, legal business structure, state of formation, years in business, employee count, and gross annual sales for the past five years. Bidders must also disclose organizational changes, litigation history with the State of Michigan, contract defaults or terminations within the past five years, and details regarding any anticipated revenue increases exceeding 25 percent. Additionally, the worksheet collects information on relevant experience, with bidders who lack prior State of Michigan contracts required to document at least three relevant projects from the past five years that demonstrate capability to manage similar-sized contracts. The document further requires attestations regarding qualified disabled veteran status, participation in RFP development or evaluation, prevailing wage utilization, employee classification compliance, abusive labor practice prohibitions, and various state-specific certifications related to Michigan business tax compliance, Iran-linked business restrictions, environmental citizenship, convict labor compliance, and outstanding state debt or tax obligations.
