About this file

This document is a set of standard certification forms for a Disaster Recovery – Home Repair Services contract with the City of Fort Worth, likely funded through HUD (Housing and Urban Development). The forms include various compliance certifications such as Lobbying, Debarment, Drug-Free Workplace, Davis-Bacon Act, and Environmental Compliance. A Category Selection Form allows proposers to indicate their service capabilities across 13 different categories including bathroom, electrical, flooring, foundation repair, general construction, HVAC, insulation, kitchen, lead hazard control, moving and storage, plumbing, roofing, and weatherization services.

The documents require proposers to complete detailed forms certifying compliance with federal regulations, including provisions related to labor standards, equal employment opportunity, environmental safety, and ethical business practices. A Subcontractor Information Form is included for listing potential subcontractors, with all subcontractors subject to City approval. The project appears to be part of a disaster recovery initiative focusing on home repair services, with multiple potential service categories available for contractors to bid on. Proposers must provide comprehensive documentation demonstrating their qualifications, legal compliance, and ability to perform the required services under federal and local guidelines.

A flow-through entity is required to complete this line to indicate that it has direct or indirect foreign partners, owners, or beneficiaries when it provides the Form W-9 to another flow-through entity in which it has an ownership interest.
