About this file
This document is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division. The form requires potential contractors or bidders to certify that they meet certain legal and ethical standards for participating in county transactions. Specifically, the certification requires companies to affirm they are not currently debarred, suspended, or ineligible from public transactions, have not been convicted of fraud or other criminal offenses within the past three years, and are not current or former county employees or immediate family members of county employees.
The document outlines detailed disqualifying conditions, including convictions related to fraud, antitrust violations, embezzlement, theft, and other offenses that might indicate a lack of business integrity. Companies must disclose any past contract terminations, criminal indictments, or ethical violations. The form warns that falsifying any statements could result in automatic disqualification from the bidding process or contract cancellation. While certification does not guarantee selection, any potentially disqualifying information will be thoroughly reviewed by the county's procurement team.
