About this file
This document is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form from the Incorporated County of Los Alamos Procurement Division. The form requires potential contractors to certify that they are not currently debarred, suspended, or otherwise excluded from public transactions, and have not been convicted of fraud, antitrust violations, embezzlement, theft, or other offenses that would compromise their business integrity within the past three years. The certification also requires bidders to confirm they are not current or former county employees, not related to county employees or officials, and have not had public contracts terminated for cause or default.
The form serves as a critical pre-qualification document for county procurement, designed to ensure that potential contractors meet ethical and legal standards for public contracting. Bidders must thoroughly disclose any potential issues that might disqualify them from consideration, with the understanding that falsification of any statement could result in immediate disqualification or contract rescission. The county reserves the right to review any certifications and may not automatically exclude a bidder based on the disclosed information, but maintains the discretion to evaluate each case individually.
